Unusual histopathological findings in appendectomy specimens with clinical diagnosis of acute appendicitis: A retrospective cohort analysis.

INTRODUCTION: While appendicitis is considered one of most common acute surgical conditions, several studies have reported abnormal histopathological findings in appendectomy specimens; however, sending all appendices to histopathology is not yet routinely done. Here we report many unusual findings. Those unusual findings played a role not only in confirming acute appendicitis as a cause of the presentation in some cases but also discovering etiologies that mimic it with great impact on its management. METHODS: Between January 2011 and December 2017, a total of 1510 patients were operated with

Enhancing bone metastasis CT report analysis: a comparison of local and proprietary large language models for privacy and resource efficiency.

BACKGROUND: Computed tomography (CT) reports for patients with bone metastases include key clinical details, but they are usually written as free text. Different doctors write reports in different ways, which makes it hard to extract information in a consistent way. Large language models (LLMs) may help pull out important details automatically. But using external models can raise data privacy and management concerns in hospitals. This study looked at whether proprietary models and locally deployed open-source LLMs can help extract key diagnostic information and create structured reports in set

Differentiating Nontuberculous Mycobacterial Pulmonary Disease from Pulmonary Tuberculosis in Resource-Limited Settings: A Pragmatic Model for Reducing Misguided Antitubercular Treatment.

Background: Differentiating nontuberculous mycobacterial pulmonary disease (NTM-PD) from pulmonary tuberculosis (PTB) remains challenging due to overlapping clinical features, particularly in resource-limited settings where diagnostic errors are frequent. This retrospective case-control study (January 2023-June 2024) aimed to identify key clinical predictors and develop a diagnostic model to distinguish NTM-PD from PTB. Methods: Patients initially presumed to have PTB (meeting clinical-radiological criteria but lacking bacteriological confirmation at admission) at a tertiary tuberculosis hospi

Subclassification of second-degree tears at delivery: creation and reported outcomes.

BACKGROUND: Perineal tears at delivery are common. The current WHO classification system compacts all the varying extents of second-degree tears into one code. Some tears lead to long-term injuries. The correct identification and classification of disease is necessary for correct clinical management as well as for research. Regulatory standards govern care practices. This article describes the process of creating and testing new subclassifications for second-degree tears at delivery. METHODS: The development and implementation of new subclassifications of second-degree perineal tears after de

Investigation of serum biomarkers in rheumatoid and psoriatic arthritis patients for disease-specific signatures.

BACKGROUND: Rheumatoid arthritis (RA) and Psoriatic arthritis (PsA) are systemic auto-immune diseases of unknown aetiology that lead to systemic inflammation and synovial joint destruction. Identification of specific serum proteins that selectively regulate these diseases, or which precede disease development could have great potential as disease biomarkers and predictors. METHODS: Serum levels of C-reactive protein (CRP), sICAM-1, sVCAM-1, Serum amyloid A (SAA), Matrix metalloproteinases (MMPs 1, 3 and 9) and metabolic markers: Active Glucose-dependent Insulinotropic polypeptide (GIP), activ

Foveal Hypoplasia Grading with Optical Coherence Tomography: Agreement and Challenges Across Experience Levels.

Background/Objectives: The diagnosis and prognosis of arrested foveal development or foveal hypoplasia (FH) can be made using the Leicester grading system for FH and optical coherence tomography (OCT). In clinical practice, ophthalmologists and ophthalmic health professionals with varying experience consult patients with FH; however, to date, the FH grading system has only been validated amongst experts. We compare the inter-grader and intra-grade agreement of healthcare professionals against expert consensus across all grades of FH. Methods: Handheld and table-mounted OCT images (n = 341) wer

Epidemiological, clinical, biological and evolutionary profile of scorpion envenomed children under one year to 15 years in the Souss Massa region of Morocco.

Morocco is one of the main countries affected in North African with the scorpion envenomations. Faced with the threat, significant morbidity and a major risk of death especially in children, a detailed identification of scorpionic profile of stings remains important for health authorities at national or even regional level. The current study aims to establish the epidemiological, clinical, biological and evolutionary data of the scorpionism by analyzing 383 cases of scorpion stings in children from three age groups (<1 year, 1-5 years and >5 years), admitted at the Regional Hospital Hassan II-

Evaluation of AQP4 functional variants and its association with fragile X-associated tremor/ataxia syndrome.

INTRODUCTION: Fragile X-associated tremor/ataxia syndrome (FXTAS, OMIM# 300623) is a late-onset neurodegenerative disorder with reduced penetrance that appears in adult FMR1 premutation carriers (55-200 CGGs). Clinical symptoms in FXTAS patients usually begin with an action tremor. After that, different findings including ataxia, and more variably, loss of sensation in the distal lower extremities and autonomic dysfunction, may occur, and gradually progress. Cognitive deficits are also observed, and include memory problems and executive function deficits, with a gradual progression to dementia

Shared Molecular Signatures Across Zika Virus Infection and Multiple Sclerosis Highlight AP-1 Transcription Factor as a Potential Player in Post-ZIKV MS-Like Phenotypes.

Zika virus (ZIKV) is an arbovirus of the Flaviviridae genus that has rapidly disseminated from across the Pacific to the Americas. Robust evidence has indicated a crucial role of ZIKV in congenital virus syndrome, including neonatal microcephaly. Moreover, emerging evidence suggests an association between ZIKV infection and the development of an extensive spectrum of central nervous system inflammatory demyelinating diseases (CNS IDD), such as multiple sclerosis-like clinical phenotypes. However, the underlying mechanisms of host-pathogen neuro-immune interactions remain to be elucidated. This

Obstetric anal sphincter injuries and other delivery trauma: a US national survey of obstetrician-gynecologists.

INTRODUCTION AND HYPOTHESIS: Obstetric lacerations complicate the majority of deliveries. The application of standardized guidelines for assessing delivery trauma has not been assessed thoroughly in the United States. We recently identified gaps in US midwives' clinical assessment of delivery trauma. We conducted a cross-sectional national survey of practicing obstetricians in the USA to characterize their classification of obstetric lacerations. We hypothesized that attending obstetricians' identification and diagnosis of delivery trauma would be similar to our findings for midwives with freq

Retinal Microvascular Alterations as the Biomarkers for Alzheimer Disease: Are We There Yet?

BACKGROUND: Alzheimer disease (AD) is a heterogeneous and multifactorial disorder with an insidious onset and slowly progressive disease course. To date, there are no effective treatments, but biomarkers for early diagnosis and monitoring of disease progression offer a promising first step in developing and testing potential interventions. Cerebral vascular imaging biomarkers to assess the contributions of vascular dysfunction to AD are strongly recommended to be integrated into the current amyloid-β (Aβ) [A], tau [T], and neurodegeneration [(N)]-the "AT(N)" biomarker system for clinical resea

Igniting Harmonized Digital Clinical Quality Measurement through Terminology, CQL, and FHIR.

BACKGROUND: Electronic clinical quality measures (eCQMs) seek to quantify the adherence of health care to evidence-based standards. This requires a high level of consistency to reduce the effort of data collection and ensure comparisons are valid. Yet, there is considerable variability in local data capture, in the use of data standards and in implemented documentation processes, so organizations struggle to implement quality measures and extract data reliably for comparison across patients, providers, and systems. OBJECTIVE: In this paper, we discuss opportunities for harmonization within an
