Beyond the Biomarkers: Gestation-Dependent Roles of sFLT-1 and PlGF in Human and Non-Human Mammalian Placental Development and Function.

Placental growth factor (PlGF) and soluble fms-like tyrosine kinase-1 (sFLT-1) are established biomarkers of placental dysfunction, but their functions extend beyond the maternal circulating ratio. This structured narrative review examines the FLT1-PlGF-vascular endothelial growth factor system across implantation, trophoblast differentiation, uterine vascular transformation, foetoplacental angiogenesis, exchange capacity development and late-gestational maturation. Human evidence is integrated with selected non-human mammalian studies according to gestational stage, cellular source, anatomica

Placental alkaline phosphatase promotes lipid accumulation and fibrosis in ligamentum flavum hypertrophy and represents a potential target for statin repurposing.

BACKGROUND AND OBJECTIVE: Lumbar spinal stenosis is a prevalent degenerative disorder driven primarily by ligamentum flavum hypertrophy. Emerging evidence links ectopic lipid accumulation to fibrotic remodeling and suggests a potential contribution of immune dysregulation. However, the molecular targets connecting lipid accumulation with ligamentum flavum fibrosis remain incompletely defined. We sought to evaluate the underlying mechanisms of lipid-lowering drug targets in lipid-associated ligamentum flavum fibrosis. METHODS: We applied two-sample Mendelian randomization (MR), summary-data-ba

Cellular, Acellular, and Matrix-Like Placental Products for Diabetic Foot Ulcers: A 12-Week Randomised-Trial Meta-Analysis.

Placental-derived cellular, acellular, and matrix-like products (CAMPs) are widely used as adjuncts to standard care for diabetic foot ulcers (DFUs), but trials differ in product class, comparator, and healing window. We estimated the effect of adjunctive placental-derived CAMPs on complete wound closure at a single, clinically meaningful 12-week endpoint. The review was registered in the International Prospective Register of Systematic Reviews (PROSPERO; CRD420261399700) and reported per Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020, with search reporting gu

Evaluating the efficacy and safety of Neothelium FT placental allograft in the management of hard-to-heal open wounds: a prospective case series.

OBJECTIVE: The aetiology of hard-to-heal (chronic) wounds is usually categorised as either 'diabetic foot', 'venous leg ulcer (VLU)', 'arterial', 'pressure injury', 'postoperative' and 'other'. Due to the homogeneity of patients with a diabetic foot ulcer, the majority of studies regarding the efficacy of placental allografts have been in this population of patients, with a small fraction conducted with patients with VLUs. However, insufficient research has been performed regarding advanced modalities for the treatment of other, open hard-to-heal wounds resulting from alternative aetiologies,

Maternal PM2.5 exposure induces preeclampsia-like phenotypes in mice through placental angiogenic dysfunction and lncRNA-RUNX1 dysregulation.

Preeclampsia (PE) is an idiopathic gestational disorder defined by placental structural abnormalities, systemic inflammatory activation, angiogenic factor imbalance, and endothelial dysfunction. While existing epidemiological studies have established a correlational association between maternal fine particulate matter (PM2.5) exposure and elevated PE risk, the specific underlying molecular mechanisms driving this association have not been fully elucidated. Using a C57BL/6J mouse model of maternal PM2.5 exposure via intranasal instillation (3 mg/kg body weight, every other days from gestational

A cross-sectional study of the correlation between placental thickness, gestational age, and foetal biometric parameters at selected hospitals in Rwanda.

INTRODUCTION: Foetal Biometric Parameters (FBP)-based pregnancy dating is vital for monitoring foetal growth. However, the FBP measurements that provide the most reliable gestational age estimations are taken during the first and early second trimesters ultrasound scans, which are often missed in resource-limited settings, necessitating alternatives. This study examined correlations between placental thickness, gestational age, and FBP measurements among pregnant women in the second and third trimesters at Muhima District Hospital and Remera Health Centre in Rwanda. METHODS: A cross-sectional

HRS Degradation-Induced Nicotinamide Deficiency in Placental Extracellular Vesicles Triggers Preeclampsia by Disrupting Maternal-Fetal Immune Homeostasis.

Preeclampsia (PE) is closely associated with alterations in placental extracellular vesicles (pEVs), but the mechanisms and their role in PE pathogenesis remain unclear. This study reveals that nicotinamide (NAM) levels in PE-derived pEVs (PE-EVs) are lower than in pEVs from normal pregnancies, correlating with disease severity. Functionally, NAM in pEVs inhibits Th1 differentiation via SIRT1 suppression and Th17 differentiation via macrophages. NAM-deficient pEVs exhibit reduced capacity to inhibit Th1 and Th17 cell differentiation both in vitro and in vivo, leading to PE-like symptoms. NAM i

Placental weight may influence preeclampsia or eclampsia risk through modulating lipid metabolism: a mediation Mendelian randomization analysis.

INTRODUCTION: Preeclampsia/eclampsia (PE/E) is a leading cause of maternal and perinatal morbidity and mortality and is closely linked to placental dysfunction and maternal metabolic disturbance. Although abnormal lipid profiles are consistently observed in PE/E, whether placental growth - proxied by placental weight (PW) - causally influences PE/E risk through lipid metabolism remains unclear. METHODS: We performed two-sample Mendelian randomization (MR) using summary statistics from a genome-wide association study (GWAS) of PW in term singleton pregnancies (19,861 child-mother-father trios)

Placental-Derived Connective Tissue Matrix: Cytokine Array Analysis and Pilot Study of Vocal Fold Injection During Microphonosurgery.

INTRODUCTION: Placental-derived connective tissue matrix (pd-CTM) is rich in extracellular proteins and bioactive factors that may modulate inflammation, fibrosis, and angiogenesis. To our knowledge, its molecular profile has not been characterized in the context of laryngology. The primary aim of this study was to quantify key wound-healing mediators in pd-CTM using proteomic methods. A secondary aim was to describe preliminary clinical observations following pd-CTM injection during microphonosurgical excision of benign true vocal fold lesions. METHODS: Estimated absolute concentrations (ng/

Oligomeric lactic acid nanoplastics induce intrauterine growth restriction in mice by disrupting GATA2-mediated placental vascular development.

Humans are increasingly exposed to "eco-friendly" biodegradable microplastic pollution, whose usage in packaging and medical applications is growing exponentially. The bioplastic polylactic acid (PLA) has recently been demonstrated to release large quantities of oligomeric lactic acid (OLA) nanoplastics causing adverse health effects. No research has reported on intrauterine biodistribution of OLA, and how gestational exposure may impact on early development of the fetus. Here, we reveal that OLA plastics can readily breach the placental barrier and accumulate in various fetal organs in a mous

Single-cell analysis reveals the diversity of human fetal membrane and adjacent placental cells with preterm premature rupture of membranes.

The occurrence of preterm premature rupture of membranes (PPROM) significantly impacts maternal and fetal health due to its association with the cellular composition and genetic changes of the fetal membrane. However, the specific cell type responsible for triggering PPROM and the underlying mechanisms are still largely unexplored. We employed single-cell RNA sequencing (scRNA-seq) analysis on fetal membrane along with adjacent placental tissues about two centimeters from the umbilical cord obtained from women who delivered full-term in labor (FTIL), preterm premature without rupture of membra

Targeted delivery of RGD-JEG-3-EVs for placental vascular regeneration in preeclampsia: from molecular mechanism toward therapy.

Preeclampsia (PE) is a hypertensive disorder of pregnancy associated with placental dysfunction and vascular maladaptation. Defective remodeling of maternal spiral arteries leads to placental ischemia, a key driver of the maternal systemic syndrome. Extracellular vesicles (EVs) from highly invasive choriocarcinoma trophoblasts, which exhibit pro-angiogenic properties, represent a potential therapeutic resource. However, their utility for targeted therapy in PE remains unexplored. We engineered EVs derived from RGD-JEG-3-EVs on activated vasculature. The therapeutic potential and safety were ev

Successful delivery after fertility-sparing surgery for stage I placental site trophoblastic tumor: a rare case report.

BACKGROUND: Placental site trophoblastic tumor (PSTT) is a rare subtype of gestational trophoblastic neoplasia (GTN) characterized by relative chemoresistance. The primary recommended treatment, especially for non-metastatic disease, is hysterectomy, making fertility-sparing a significant clinical challenge. There are exceedingly few reported cases of successful uterine preservation, highlighting the need for well-documented management strategies for early-stage patients with a strong desire for future fertility. CASE PRESENTATION: We report the case of a 32-year-old woman (gravida 0, para 0)
