Volitional processes in changing physical activity: A randomized controlled trial with individuals with knee osteoarthritis.

OBJECTIVE: A health action process approach (HAPA)-based intervention was designed to support moderate-to-vigorous physical activity (MVPA) in individuals with osteoarthritis of the knee (OAK). In secondary analyses of the randomized controlled trial "preventing the impairment of primary osteoarthritis by high-impact long-term physical exercise regimen-psychological adherence program," we examined long-term effects of the intervention on HAPA determinants and MVPA, and explored the former as mediators of change. METHOD: N = 241 individuals with OAK (63% women, aged 44-80 years) were randomly

Pharmacogenetic actionability and medication prescribing in people with cystic fibrosis.

Although major advancements have been made in the therapeutics for people with cystic fibrosis (PwCF), many still require the use of multiple medications to manage acute exacerbations of disease and maintain health. Iterative trial and error processes of pharmacotherapeutic management can be optimized by assessing and incorporating pharmacogenetics. For 82 PwCF, we reviewed 2 years of medication use and response history and interrogated metabolizer status for common pharmacogenes, revealing 3336 medication exposure events (MEEs) to 286 unique medications. As expected, the more frequent MEEs we

A QI Initiative to Reduce Hospitalization for Children With Isolated Skull Fractures.

BACKGROUND AND OBJECTIVE: Although children with isolated skull fractures rarely require acute interventions, most are hospitalized. Our aim was to safely decrease the hospitalization rate for children with isolated skull fractures. METHODS: We designed and executed this multifaceted quality improvement (QI) initiative between January 2008 and July 2015 to reduce hospitalization rates for children ≤21 years old with isolated skull fractures at a single tertiary care pediatric institution. We defined an isolated skull fracture as a skull fracture without intracranial injury. The QI interventio

Spouses' attempts to regulate day-to-day dietary adherence among patients with type 2 diabetes.

OBJECTIVE: To investigate daily dietary adherence and diabetes-specific distress among older adults with type 2 diabetes mellitus (T2DM) as a function of spouses' diet-related support and diet-related control (persuasion and pressure) and whether these daily processes differ among couples who do and do not appraise responsibility for managing T2DM as shared. METHODS: End-of-day diaries were completed by 126 couples in which one partner had T2DM (patient) and the other did not (spouse). Using electronic diary methods, each partner independently recorded data for 24 consecutive days (patients r

La Familia: methodological issues in the assessment of perinatal social support for Mexicanas living in the United States.

Do Mexicanas receive social support from a close network of family and friends during the perinatal period? To answer this question, a longitudinal ethnographic study followed 28 urban Mexican-origin women living in the US from their last trimester of pregnancy through their first month post-partum. A total of 93 interviews with Mexicanas focused on health and social support. All of the women lived in a large western city in the US but varied in their acculturation and income levels. Analyses identified four social support themes from women's experience (the emic analysis) and four social supp

Understanding health care practitioners' criteria regarding the decision to treat men with premature ejaculation.

BACKGROUND: The extent to which health care practitioners (HCP) follow clinical practice guidelines (CPG) regarding diagnostic criteria can affect treatment outcomes, yet little is known about the factors HCPs take into consideration regarding their decision to treat men with symptoms of premature ejaculation (PE), a field where professional definitions vary considerably. AIM: To describe HCP priorities regarding the use of PE diagnostic criteria in their decision-to-treat and, further, to assess whether HCP characteristics regarding specialization, professional identity, and experience impac

Getting evidence into clinical practice: protocol for evaluation of the implementation of a home-based cardiac rehabilitation programme for patients with heart failure.

INTRODUCTION: Cardiac rehabilitation (CR) improves health-related quality of life and reduces hospital admissions. However, patients with heart failure (HF) often fail to attend centre-based CR programmes. Novel ways of delivering healthcare, such as home-based CR programmes, may improve uptake of CR. Rehabilitation EnAblement in CHronic Heart Failure (REACH-HF) is a new, effective and cost-effective home-based CR programme for people with HF. The aim of this prospective mixed-method implementation evaluation study is to assess the implementation of the REACH-HF CR programme in the UK National

Heterogeneity in Vision, Hand Function, Cognition, and Health Literacy Among Older Veterans: Impacts, Outcomes, and Clinical Recommendations for First-Time Hearing Aid Users.

BACKGROUND: Age-related changes (both normal and pathological), and health literacy are relevant to audiological practice. Changes associated with the musculoskeletal, vascular, and nervous systems drive manual, visual, and cognitive function. These in turn affect the capabilities required for effective hearing aid (HA) skill acquisition, use, and management. Meanwhile, health literacy influences the ability to gain access to, understand, and use information, which is important for promoting and maintaining HA use and management. Understanding the interindividual variability of these variables

Global, regional, and national estimates and trends in stillbirths from 2000 to 2019: a systematic assessment.

BACKGROUND: Stillbirths are a major public health issue and a sensitive marker of the quality of care around pregnancy and birth. The UN Global Strategy for Women's, Children's and Adolescents' Health (2016-30) and the Every Newborn Action Plan (led by UNICEF and WHO) call for an end to preventable stillbirths. A first step to prevent stillbirths is obtaining standardised measurement of stillbirth rates across countries. We estimated stillbirth rates and their trends for 195 countries from 2000 to 2019 and assessed progress over time. METHODS: For a systematic assessment, we created a dataset

A Multicomponent Strategy to Improve HIV Pre-Exposure Prophylaxis in a Southern US Jail: Protocol for a Type 3 Hybrid Implementation-Effectiveness Trial.

BACKGROUND: Pre-exposure prophylaxis (PrEP) is an effective approach for preventing HIV infection, but it is underutilized by populations who may benefit the most, including people living in the Southern United States and those involved in the criminal legal (CL) system. Improving the access and use of PrEP for these groups could decrease HIV-related health disparities. Beyond individual outcomes, HIV prevention for CL-involved people can have a significant public health impact on HIV incidence due to a high turnover between jails and the community. OBJECTIVE: We will develop, implement, and

The Emerging Role of Prolactin as a Biomarker for Reproduction, Health, and Welfare in Wildlife Species: Elephants as a Model.

Prolactin is a key hormone that plays significant roles in reproduction, extending far beyond its well-known function in mammalian lactation. It is vital for reproductive success across diverse taxa, including birds, fish, reptiles, and amphibians, where it regulates critical reproductive behaviors and physiological processes like parental care, pregnancy maintenance, embryonic diapause, immunity, osmoregulation, metamorphosis, and molting. Environmental cues often fine-tune its actions to ensure reproduction occurs under optimal conditions. Conversely, dysregulation of prolactin can severely

Is Discordance Between Recommended and Actual Postacute Discharge Setting a Risk Factor for Readmission in Patients With Congestive Heart Failure?

Background Readmissions in patients with congestive heart failure are common and often preventable. Limited data suggest that patients discharged to a less intensive postacute care setting than recommended are likely to readmit. We examined whether postacute setting discordance (discharge to a less intensive postacute setting than recommended by a physical and occupational therapist) was associated with hospital readmission in patients with congestive heart failure. We also assessed sociodemographic and clinical predictors of setting discordance. Methods and Results Retrospective analysis of a
