[Refsum's disease: evolution 35 years after diagnosis].

Refsum's disease (Heredopathia atactica polyneuritiformis) is an autosomal recessive disease caused by a defective alpha oxidation of a C20 fatty acid: the phytanic acid. Deficiency of a peroxysomal enzyme called "Phytanoyl-Co-A alpha hydroxylase" leads to an accumulation of phytanic acid. The clinical picture include retinitis pigmentosa, peripheral neuropathy, ataxia and elevated cerebrospinal fluid protein concentration. Firstly described in 1946 by Sigvald Refsum, dietary treatment leads to an improvement of neurological symptoms but does not affect retinal changes. To our knowledge, there

Von Hippel-Lindau disease in a large British family: clinicopathological features and recommendations for screening and follow-up.

We describe the clinical and pathological features in nine members of a large British family with von Hippel-Lindau disease. Twenty living family members underwent detailed assessment for features of the disease, including retinal examination and computed tomography of the head and abdomen. The hospital records of three deceased family members were reviewed and detailed information about a fourth was obtained from members of the family. As a result of this study, three asymptomatic family members were found to have features of the disease and all three living affected family members were found

Polygenic Liability to Alzheimer's Disease Is Associated with a Wide Range of Chronic Diseases: A Cohort Study of 312,305 Participants.

BACKGROUND: Alzheimer's disease (AD) patients rank among the highest levels of comorbidities compared to persons with other diseases. However, it is unclear whether the conditions are caused by shared pathophysiology due to the genetic pleiotropy for AD risk genes. OBJECTIVE: To figure out the genetic pleiotropy for AD risk genes in a wide range of diseases. METHODS: We estimated the polygenic risk score (PRS) for AD and tested the association between PRS and 16 ICD10 main chapters, 136 ICD10 level-1 chapters, and 377 diseases with cases more than 1,000 in 312,305 individuals without AD diag

High-Altitude Retinopathy Presenting as Blue Spots with Prolonged Recovery: A Case Report.

Smith, Ben, Nicola Wetherill, and Daniel S. Morris. High-altitude retinopathy presenting as blue spots with prolonged recovery: a case report. High Alt Med Biol. 26:99-101, 2025.-High-altitude retinopathy (HAR) is a common ocular manifestation of high-altitude illness that is usually asymptomatic but can present with reduced or distorted central vision and signs of retinal vascular engorgement and retinal hemorrhages of varying severity. Typically, symptoms are very mild, such as a slight decrease in visual acuity or a small scotoma. There is no treatment for this condition, and most cases res

Microcephaly with or without chorioretinopathy, lymphedema, or mental retardation (MCLMR)- the new lacunae: a case report.

BACKGROUND: Microcephaly with or without chorioretinopathy, lymphedema, or mental retardation is a rare autosomal dominant disease caused by mutations in KIF11 which disrupt EG5 protein function, impacting the development and maintenance of retinal and lymphatic structures due to its expression in the retinal photoreceptor cilia. The primary ocular finding in MCLMR is chorioretinopathy. Additional features can include microphthalmia, angle-closure glaucoma, persistent hyperplastic primary vitreous, cataract, pseudo-coloboma, persistent hyaloid artery, and myopic or hypermetropic astigmatism. Microglial and macroglial dynamics in a model of retinitis pigmentosa.

In retinal degenerative diseases, such as retinitis pigmentosa (RP), the characteristic photoreceptor cell death is associated with changes of microglia and macroglia cells. Gene therapy, a promising treatment option for RP, is based on the premise that glial cell remodeling does not impact vision rescue. However, the dynamics of glial cells after treatment at late disease stages are not well understood. Here, we tested the reversibility of specific RP glia phenotypes in a Pde6b-deficient RP gene therapy mouse model. We demonstrated an increased number of activated microglia, retraction of mic

Case Report: Bilateral Valsalva Retinopathy in an Acutely Anemic Patient.

SIGNIFICANCE: The Valsalva maneuver is known to cause pre-retinal hemorrhages, which have a predilection to affect the macula. Certain conditions may increase the risk of developing Valsalva retinopathy including anemia. Knowing these associations can aid in uncovering underlying systemic conditions for which a patient may not otherwise be aware. PURPOSE: This case report reviews the presentation of Valsalva retinopathy and the impacts of acute anemia on retinal findings. Recommendations for management are also reviewed and reported. CASE REPORT: A 30-year-old Hispanic man presented to the e

Neovascular Glaucoma: An Update.

Neovascular glaucoma (NVG) is a severe type of secondary glaucoma with devastating complications and generally poor visual prognosis. NVG is defined by the development of pathological neovessels over the iris and the iridocorneal angle that can block the outflow of aqueous humor, causing elevation of intraocular pressure (IOP). The pathogenesis of NVG is, in most cases, associated with ischemia of the posterior segment, which is most frequently associated with proliferative diabetic retinopathy or central retinal vein occlusion. The advanced stages of NVG are by iris and angle neovascularizati

Pentosan polysulfate maculopathy.

Pentosan polysulfate sodium (PPS), a semisynthetic sulfated polysaccharide, is the only FDA-approved oral therapy for interstitial cystitis. Recent studies have described a progressive, vision-threatening macular condition associated with long-term PPS use. We reviewed all publications concerning PPS maculopathy to consolidate known clinical features and to evaluate the strength of this association. Current literature supports a strong dose-dependent association between PPS exposure and a progressive maculopathy impacting the retinal pigment epithelium (RPE) and RPE-photoreceptor interface tha

[Spectrum of diseases associated with paracentral acute middle maculopathy].

Paracentral acute middle maculopathy (PAMM) is a rare disease affecting middle layers of the central retina that is seen as a focal hyperreflective band-like lesion on optical coherence tomography. Paracentral acute middle maculopathy is associated mainly with vascular diseases. Optical coherence tomography angiography (OCTA) findings confirm the ischemic nature of paracentral acute middle maculopathy.

Most cases labeled as "retinal migraine" are not migraine.

BACKGROUND: Monocular visual loss has often been labeled "retinal migraine." Yet there is reason to believe that many such cases do not meet the criteria set out by the International Headache Society (IHS), which defines "retinal migraine" as attacks of fully reversible monocular visual disturbance associated with migraine headache and a normal neuro-ophthalmic examination between attacks. METHODS: We performed a literature search of articles mentioning "retinal migraine," "anterior visual pathway migraine," "monocular migraine," "ocular migraine," "retinal vasospasm," "transient monocular vi

[Vitrectomy in optic nerve disc pit with maculopathy].

There is described a rare case of a congenital optic disc pit of the 20-years old man that was accompanied by a cyst-like change in the macula with serous retinal detachment. Visual impairment in his right eye was treated with pars-plana-vitrectomy with surgical posterior vitreous detachment and gas tamponade. After temporary pneumatic displacement of the subretinal fluid inferiorly the corrected visual acuity improved to 6/9. Schisislike changes persisted in the macular area. Our findings could support the view on the two-layer structure of optic disk pit maculopathy and an important role of

Senior's syndrome (hereditary renal retinal dysplasia) associated with cataracts.

Senior's syndrome includes nephronophthisis with retinitis pigmentosa. In our patient, there was a combination of bilateral retinitis pigmentosa, cataracts, nephronophthisis, osteomalacia, growth and mental retardation, arterial hypertension, and aortic insufficiency. This case was unusual because all the features of Senior's syndrome existed along with bilateral cataracts from childhood.

Perfluorocarbon liquids in the management of complicated retinal detachments.

Low viscosity perfluorocarbon liquids have unique physical properties making them ideal for the temporary tamponade, mechanical fixation, and manipulation of the retina. These substances are optically clear, have a specific gravity greater than that of water, and have interfacial tension properties similar to those of silicone oil. The temporary mechanical fixation of the retina by perfluorocarbon liquids facilitates the removal of epiretinal membranes and release of tractional forces. Intraoperative flattening of the retina under perfluorocarbon liquid eliminates the need for a posterior reti

Solitary retinal astrocytoma.

A two-year-old female child presented with a left retinal mass in front of the optic disc and total serous retinal detachment. Enucleation was performed for suspected retinoblastoma. Histopathologically the tumour was composed of astrocytic giant cells with a few spindle-shaped astrocytes. Presence of multiple necrotic foci in the tumour resembling micro-abscesses and extension of tumour cells beyond lamina cribrosa were additional unusual features.

[Retinitis pigmentosa with uveal effusion syndrome, a case report].

We reported a 65 year old female patient with retinitis pigmentosa accompanied by unilateral uveal effusion syndrome. Reduction in visual acuity and loss of depth of the anterior chamber were followed by peripheral choroidal detachment and then by retinal detachment. Subretinal fluid shifted to the lower part of the fundus for a short period and easily shifted to the posterior pole in a supine position. The axial length of both eyes was 21.3 mm. Fluorescein angiography revealed no definite points of leakage or abnormally dilated vessels. Chorioretinal detachment gradually disappeared with syst
