Shoulder Hemiarthroplasty After Previous Pectoralis Major Transfer for Irreparable Subscapularis Tear: A Case Report.

CASE: A 30-year-old woman with an irreparable subscapularis tear presented with persistent painful anterior instability despite several open and arthroscopic stabilization procedures. A pectoralis major tendon transfer (PMTT) was performed. The patient subsequently developed progressive glenohumeral arthritis over the next 10 years, ultimately necessitating shoulder arthroplasty. CONCLUSIONS: PMTT provides valuable anterior soft-tissue reconstruction stabilization for subscapularis insufficiency in a multioperated shoulder. Ten years later, the transfer was found to be intact and managed like

The lower trapezius transfer: a systematic review of biomechanical data, techniques, and clinical outcomes.

BACKGROUND: Lower trapezius (LT) transfers were originally described to restore external rotation (ER) in the management of brachial plexus palsy; however, there is recent interest in the role of this transfer to restore shoulder function, specifically ER, in patients with a massive irreparable rotator cuff tear (RCT). The purpose of this systematic review is to summarize the current literature pertaining to LT transfers, including biomechanics, techniques, and clinical outcomes for patients with brachial plexus palsy and massive RCTs. METHODS: MEDLINE, EMBASE, and PubMed were searched for bi

Arthroscopic superior capsule reconstruction with Teflon felt synthetic graft for irreparable massive rotator cuff tears: clinical and radiographic results at minimum 2-year follow-up.

BACKGROUND: Superior capsule reconstruction (SCR) was developed to improve shoulder function and relieve pain in patients with irreparable rotator cuff tears. Here, we investigated the clinical and radiographic outcomes and postoperative complications of SCR using a Teflon graft for reconstruction. METHODS: Thirty-five consecutive patients with irreparable rotator cuff tears underwent SCR with Teflon grafts. The American Shoulder and Elbow Surgeons score, active shoulder elevation, shoulder muscle strength, visual analog scale pain scores, acromiohumeral distance, and postoperative complicati

Glenohumeral osteoarthritis: frequency of underlying diagnoses and the role of arm dominance-a retrospective analysis in a community-based musculoskeletal practice.

Patients with shoulder pain are commonly seen in adult primary care, and are typically referred to musculoskeletal specialists when their symptoms are persistent. Rotator cuff disorders (RCD) most commonly underlie non-traumatic shoulder pain in adults, and glenohumeral osteoarthritis (GHOA) has been considered to be relatively uncommon (Nakagawa et al., J Shoulder Elb Surg 8:580-584, 1999). Our impression has been that GHOA affects significant numbers of patients, is less well studied, and may be under recognized. Among adults referred to a musculoskeletal practice for shoulder pain evaluatio

Double-Layer Rotator Cuff Repair: Anatomic Reconstruction of the Superior Capsule and Rotator Cuff Improves Biomechanical Properties in Repairs of Delaminated Rotator Cuff Tears.

BACKGROUND: Delamination in rotator cuff tears has been identified as a prognostic factor for negative outcome after repair, with a reported prevalence between 38% and 88%. PURPOSE: To compare biomechanical properties of 3 repair techniques for delaminated rotator cuff tears. STUDY DESIGN: Controlled laboratory study. METHODS: Eighteen fresh-frozen cadaveric shoulders were used to evaluate rotator cuff footprint reconstruction, contact area and pressure, displacement under cyclical loading, and load to failure of 3 double-row repair configurations: double-row suture repair with medial row k

[Functional evaluation of the supraspinatus tendon repair comparing mini-open and open techniques].

OBJECTIVE: To compare two surgical techniques to address the painful shoulder syndrome with partial or total supraspinous tendon tear. METHODS: Patients with a diagnosis of complete supraspinous tendon tear who required surgical repair and subacromial decompression. One group was treated with acromioplasty and tendon plasty using an open approach (G1), and the second one underwent arthroscopy with tendon plasty using a minimally-invasive approach (G2). The surgical approach was not decided at random, but was based on the surgeons preference. The major outcome was functional assessment at post

Biomechanics of reverse total shoulder arthroplasty.

Reverse total shoulder arthroplasty is an effective procedure for treatment of glenohumeral joint disease among patients with severe rotator cuff deficiency. Improvements in prosthetic design are the result of an evolved understanding of both shoulder and joint replacement biomechanics. Although modern generations of the reverse shoulder prosthesis vary in specific design details, they continue to adhere to Grammont's core principles demonstrated by his original Delta III prosthesis. This review article discusses the biomechanics of reverse total shoulder arthroplasty with a focus on elements

Percutaneous humeral derotational osteotomy in obstetrical brachial plexus palsy: a new technique.

Thirty three per cent of children with obstetrical brachial plexus palsy with incomplete neurological recovery develop shoulder internal contracture associated with osseous deformity. Some of the older children are treated by humeral derotational osteotomy. The classical technique of open approach to the humeral diaphysis and plate fixation imposes a longitudinal scar and carries significant risks (nonunion, nerve palsy); a secondary procedure for plate removal is necessary in a significant proportion of patients. The authors report a new technique of percutaneous humeral osteotomy with osteos

Arthroscopic pancapsular plication for multidirectional shoulder instability in overhead athletes.

Treating shoulder multidirectional instability with an open stabilization procedure has been reported to have good results. However, few studies exist of arthroscopic plication, especially in overhead athletes. The purpose of this study was to evaluate the clinical outcomes of arthroscopic pancapsular plication for multidirectional instability in overhead athletes.Twenty-three athletes with symptomatic multidirectional instability were treated with arthroscopic pancapsular plication and evaluated at a mean follow-up of 36.3 months (range, 24-61 months). Mean patient age was 23.3 years (range,

Is external rotation the correct immobilisation for acute shoulder dislocation? An MRI study.

INTRODUCTION: Anterior dislocation of the shoulder is frequent, with high rates of recurrence. Immobilization in external rotation (ER) seems to improve results, although few studies have actually demonstrated this. The present MRI study examined the impact of ER on labral and capsular ligamentous complex lesions after primary dislocation. MATERIAL: A prospective study was started up on January 1st, 2007. Inclusion criteria were: acute initial anteromedial dislocation of the shoulder, without past history of shoulder trauma. There were 23 such patients, with a mean age of 37 years. METHODS:

Incidence of glenohumeral instability in collegiate athletics.

BACKGROUND: Glenohumeral instability is a common injury sustained by young athletes. Surprisingly, little is known regarding the incidence of glenohumeral instability in collegiate athletes or the relevant risk factors for injury. A better understanding of the populations most at risk may be used to develop preventive strategies. HYPOTHESIS: The incidence of glenohumeral instability in collegiate athletics is high, and it is affected by sex, sport, type of event, and mechanism of injury. STUDY DESIGN: Descriptive epidemiologic study. METHODS: The National Collegiate Athletic Association inj

[Brachial plexus neuropathy (stinger syndrome) occurring in a patient with shoulder laxity].

The stinger syndrome is a common neuropathy caused by traction or compression of the brachial plexus. In general, it is seen in young adults involved in sport activities and a major contact trauma is the rule. An 11-year-old boy with bilateral glenohumeral joint laxity had pain in the left shoulder, numbness and decreased strength in the left arm that developed after striking against a wall while running, with the left shoulder in extension and the neck in minimal lateral flexion to the contralateral side. Physical examination showed extreme anteroinferior passive translocation of the humeral

Severe humeral erosion in a bedridden patient: a comprehensive intervention to prevent bone fracture.

This article reports a 1-yr prospective study related to a bedridden patient with rotator cuff tear arthropathy featuring severe erosion of the proximal part of the humerus. To prevent spontaneous humeral fracture, a comprehensive intervention was planned. This included drug therapy for osteoporosis and pain, nurse team training on patient handling techniques, and patient and caregiver education. After 1 yr, clinical and radiologic findings are stable.

[Synovial chondromatosis as an unusual cause for subacromial impingement in a tennisplayer].

On the one hand synovial chondromatosis is a rare disorder and usually involves the knee and hip but rarely the shoulder joint. On the other hand bursal causes of impingement are rare too. We report a case of synovial osteochondromatosis presenting as chronic impingement syndrome in a 44 year old hobby tennis player. In the course of four years impingement problems and a "tumorous" growth on the outer surface attacked attraction to the patient. An MRI presented several nodular structures with sclerosis in the anteriomedial region of the shoulder joint. The mass was completely removed by operat
