Acherontiscus caledoniae: the earliest heterodont and durophagous tetrapod.

The enigmatic tetrapod Acherontiscus caledoniae from the Pendleian stage of the Early Carboniferous shows heterodontous and durophagous teeth, representing the earliest known examples of significant adaptations in tetrapod dental morphology. Tetrapods of the Late Devonian and Early Carboniferous (Mississippian), now known in some depth, are generally conservative in their dentition and body morphologies. Their teeth are simple and uniform, being cone-like and sometimes recurved at the tip. Modifications such as keels occur for the first time in Early Carboniferous Tournaisian tetrapods. Achero

Sagittal Craniosynostosis with Uncommon Anatomical Pathologies in a 56-Year-Old Male Cadaver.

Sagittal craniosynostosis (CS) is a pathologic condition that results in premature fusion of the sagittal suture, restricting the transverse growth of the skull leading in some cases to elevated intracranial pressure and neurodevelopmental delay. There is still much to be learned about the etiology of CS. Here, we report a case of 56-year-old male cadaver that we describe as sagittal CS with torus palatinus being an additional anomaly. The craniotomy was unsuccessful (cephalic index, CI = 56) and resulted in abnormal vertical outgrowth of the craniotomized bone strip. The histological analysis

Stimulation Effect of Inter-subject Variability in tDCS-Multi-scale Modeling Study.

Transcranial direct current stimulation (tDCS) is an emerging non-invasive neuromodulation method that is convenient and popular in clinical use. However, there is a practical issue in applying tDCS; it is difficult to optimize the montage for each individual because of inherent inter-subject variability. Thus, the stimulation effect of such individual anatomical head variation has been investigated using anatomically realistic models. In this work, we developed a multi-scale computational model, which combined head models based on magnetic resonance imaging (MRI) and multi-compartmental neuro

Focused ultrasound in neurosurgery: a historical perspective.

Focused ultrasound (FUS) has been under investigation for neurosurgical applications since the 1940s. Early experiments demonstrated ultrasound as an effective tool for the creation of intracranial lesions; however, they were limited by the need for craniotomy to avoid trajectory damage and wave distortion by the skull, and they also lacked effective techniques for monitoring. Since then, the development and hemispheric distribution of phased arrays has resolved the issue of the skull and allowed for a completely transcranial procedure. Similarly, advances in MR technology have allowed for the

A novel nasoseptal flap harvesting technique in revision expanded endoscopic transsphenoidal approaches.

OBJECTIVES: To describe the technique of harvesting the nasoseptal flap (NSF) in revision-expanded endoscopic approaches (EEA). STUDY DESIGN: We retrospectively analyzed four cases of endoscopic skull base reconstruction (ESBR) following revision EEA done for pituitary adenoma recurrence. The presence of an intact mucoperiosteum between the nasal septum and the roof of the choana as judged on a preoperative endoscopic and radiological assessment was considered to be sufficient for the presence of a viable pedicle. By strategic placement of the incisions, the entire bilateral posterior nasal s

Determining fitting ranges of various bone conduction hearing aids.

OBJECTIVES: To define fitting ranges for nine bone conduction devices (BCDs) over different frequencies based on the device's maximum power output (MPO) and to validate the assessment of MPO of BCDs in the ear canal. BACKGROUND: Maximum power output (MPO) is an important characteristic when fitting BCDs. It is the highest output level a device can deliver and is one of the major determinants of a device's fitting range. A skull simulator can be used to verify MPO of percutaneous BCDs. No such simulator is available for active and passive transcutaneous devices. DESIGN: The MPO of nine differ

Intracranial Extracranial Huge Meningioma: Report of a Case.

Meningioma accounts for 15% to 20% of all intracranial neoplasms. Intracranial meningioma is a common brain tumor but huge meningiomas with extracranial extensions are extremely rare. Various symptoms, including neurological deficits and epileptic seizures are very frequent in these cases. Surgery still remains the principal form of the treatment and must be preceded by appropriate preoperative diagnostics. We report an unusual case of a huge meningioma of the brain (intracranial-extracranial extension) invading and destroying the skull bone and subcutaneous and cutaneous tissue. A 44-year old

Rare case of a primary intracranial chondrosarcoma.

BACKGROUND: Intracranial chondrosarcomas comprise 6% of all skull base neoplasms and account for 0.15% of all intracranial tumors. They are potentially fatal, invading the brain and elevating intracranial pressure by progressive enlargement. Diplopia and headache are the most common clinical symptoms. Previous data indicate a particularly aggressive behavior of intracranial chondrosarcomas. CASE REPORT: A 17-year-old female patient presented to the hospital with focal seizures. A magnetic resonance imaging (MRI) scan revealed a brain tumor located in the right meso-temporal region. Total tumo

Endoscopic endonasal surgical approach to the oculomotor trigone from the cavernous sinus.

Knowledge of anatomy visualized endoscopically is necessary to perform endoscopic surgical procedures safely. The cavernous sinuses are complicated structures with major blood vessels and nerves seated deeply in the center of the skull base. Anatomical orientation during surgery is essential for deep and narrow skull base surgery. While performing surgery involving the cavernous sinuses, understanding of the structures identifiable via a transsphenoidal view can allow comprehension of the relationship between a lesion and the surrounding structures, thus preventing intraoperative complications

Small-cell neuroendocrine carcinoma of the esophagus: an autopsy case report.

Small-cell neuroendocrine carcinoma is a well-known aggressive neoplasia, which is usually associated with a poor prognosis. The lung is the most common primary site, but other organs may be involved, especially those of the digestive tract. The authors report the case of a 71-year-old Caucasian, male patient who was admitted because of congestive heart failure and loss of vision accompanied by right proptosis. Skull and sinuses computed tomography showed a tumoral mass involving the posterior region of the right eye, local bones, and paranasal sinuses. Because of severe hemodynamic instabilit

Non-traumatic spontaneous acute epidural hematoma in a patient with sickle cell disease.

A 19-year-old female with sickle cell anemia (SCD) was referred to our hospital after two days of hospitalization at another hospital for a headache crisis. This headache crisis was due to a raised intracranial pressure; these symptoms were noted and included in her comprehensive list of symptoms. There was an acute drop in the hemoglobin and hematocrit levels. The cranial CT scan demonstrated a left fronto-parietal acute epidural hematoma (AEH) and a calvarial bone expansion, which was suggestive of medullary hematopoiesis. The patient underwent emergent craniotomy and evacuation of the hemat

The discovery of stapes.

Giovanni Filippo Ingrassia revisited and redefined some of Galeno's reports, and was recognized as one of the leading Italian Physicians of the 16th century. Ingrassia principally studied the skull, and gave very important contributions to otorhinolaryngology, including the discovery of the stapes. He also isolated the inferior nasal concha from the maxillary bone, described the frontal sinus, the pterygopalatine fossa and several foramina of the skull. Ingrassia firstly attributed a sensorial function to the middle ear bones, which he called fifth particular function. He also added some detai

A new neosuchian with Asian affinities from the Jurassic of northeastern Brazil.

Phylogenetic relationships near the origin of extant crocodylians are weakly supported, and this lack of resolution makes for poor estimates of taxonomic and morphological diversity. Previously known taxa are found throughout the Cretaceous in Laurasia and at a few sites from Brazil, Australia, and northern Africa. Here, we report Batrachomimus pastosbonensis gen. et sp. nov. from the Late Jurassic of northeastern Brazil, which is deeply nested within Neosuchia and associated to the Cretaceous Asian paralligatorids. The new taxon predates all other members of Paralligatoridae and its immediate

Endoscopic endonasal management of congenital intranasal meningocele in a 2-month-old infant.

INTRODUCTION: Nasal meningocele, which is usually congenital, is a rare anomaly resulting from meningeal herniation into the nasal cavities through a bone defect in the skull base. CASE REPORT: An 8-day-old boy was referred with respiratory disturbance and nasal obstruction. Examination showed a cyst-like grayish swelling filling the right nasal cavity. CT scan showed opacity, of fluid-like density, filling the right nasal fossa, in contact with a small bony defect in the right cribriform plate. MRI ruled out herniated brain parenchyma and enabled diagnosis of meningocele. The patient was ope

A case of intraosseous microcystic meningioma without a mass lesion.

Both intraosseous and microcystic meningiomas are rare tumor types. We report the case of a 66-year-old woman with intraosseous microcystic meningioma without a mass lesion. She presented with a rare intraosseous microcystic meningioma manifesting as pain. Radiological examination revealed an osteolytic lesion in the right parietal bone. Magnetic resonance (MR) images showed iso- to hypointensity on T1-weighted images and hyperintensity on T2-weighted images corresponding to the lesion. T1-weighted MR imaging with gadolinium enhancement better defined the marginal area. The inner table of the

Practical and technical aspects of trans-sphenoidal surgery.

Trans-sphenoidal surgery was first described more than a century ago. Today, this approach occupies a crucial place in the armamentarium of the neurosurgeon for the management of sellar, suprasellar, and parasellar pathological conditions. Over the years, the procedure has witnessed multiple modifications, benefitting from technological advances and from innovative ideas of pioneering neurosurgeons and otolaryngologists. The introduction of the microscope and then the endoscope allowed progressive improvement of visualization, illumination, and magnification in this restricted surgical corrido

No evidence for an afrotherian-like delayed dental eruption in South American notoungulates.

The fossil South American ungulates are of great interest relative to the new phylogenetic framework elaborated for living placental mammals. In particular, studies on these endemic taxa can allow for testing congruence between southern placental phylogeny and plate tectonics, beyond what has already been suggested in the Atlantogenata hypothesis based on extant afrotherians and xenarthrans. The presence of delayed dental eruption relative to skull growth is one feature characterizing the extant afrotherians and possibly the xenarthrans. Late dental eruption has been mentioned previously in So

[A murder case from 900 years ago? Analysis of extensive cranial trauma observed in a historical skeleton recovered in central Poland].

This work presents the results of study of a human skeleton from the early Middle Ages recovered in Pecławice (province of Łódź), presenting signs of extensive cranial trauma suffered perimortem. The skeleton belonged to a 20-30 year-old male of sturdy build, with prominent bone processes, marked right-side asymmetry of the bones and joints of the upper extremities, and tallness (stature well above average for early medieval times). Except for the skull, the skeleton lacks any pathologic or traumatic lesions. The right side of the skull bears signs of three extensive injuries involving the fro

Primary meningeal T-cell lymphoma at the clivus mimicking a meningioma.

BACKGROUND: Most primary lymphomas of the central nervous system (CNS) are of B-cell origin and are found intra-axially, with a few reported cases of skull base tumors involving the upper clivus or sellar region or both. In this case, a tumor resembling a clivus meningioma without osseous involvement was surgically removed and turned out to be a primary T-cell lymphoma. CASE REPORT: A 60-year-old woman presented with slight right-sided abducens nerve palsy. Cranial imaging revealed an extra-axial mass at the caudal clivus resembling a meningioma. The tumor was removed surgically; smear prepar
