Human Exposure to Hand-Arm Vibration from a Surgical Drill During Simulated Spine Surgery.

OBJECTIVE: During spine surgery, surgeons are exposed to vibrations from surgical drills. Increased exposure to vibration can result in neurologic, vascular, and musculoskeletal impairments. To reduce these risks, occupational health standards have been implemented to limit exposure levels. The purpose of this study was to quantify human exposure to hand-arm vibration from a surgical drill during a simulation of a common spine procedure. METHODS: Vibration measurements were collected during three 30-second surgical trials on a fresh frozen cadaver torso specimen using a standard surgical dril

Denosumab increases spine bone density in women with anorexia nervosa: a randomized clinical trial.

OBJECTIVE: Anorexia nervosa is complicated by high bone resorption, low bone mineral density (BMD), and increased fracture risk. We investigated whether off-label antiresorptive therapy with denosumab increases BMD in women with anorexia nervosa. DESIGN: Twelve-month, randomized, double-blind, placebo-controlled study. METHODS: Thirty ambulatory women with anorexia nervosa and areal BMD (aBMD) T-score <-1.0 at ≥1 sites were randomized to 12 months of denosumab (60 mg subcutaneously q6 months)(n = 20) or placebo (n = 10). Primary end point was postero-anterior (PA) lumbar spine aBMD by dual-e

Value of Evoked Potential Changes Associated with Neck Extension Prior to Cervical Spine Surgery.

BACKGROUND: Multimodal intraoperative monitoring (MIOM) is a useful tool to warn surgeons to intervene for intraoperative spinal cord injury in cervical spine surgery. However, the value of MIOM remains controversial before cervical spine surgery. OBJECTIVE: To explore the value of MIOM in early detecting spinal cord injury associated with neck extension before cervical spine surgery. METHODS AND MATERIALS: Data of 191 patients receiving cervical spine surgery with the MIOM were enrolled from June 2014 to June 2020. The subjects were divided into a group of evoked potentials (EP) changes and

Spine Fragility Fracture Prediction Using TBS and BMD in Postmenopausal Women: A Bayesian Approach.

The trabecular bone score (TBS) estimates bone microarchitecture and can be used to evaluate the risk of osteoporotic fractures independently of bone mineral density (BMD). In this retrospective case-control study, we tested and compared the ability of TBS and lumbar spine BMD (LS-BMD) to predict vertebral fragility fractures. The inclusion criteria were female sex, age range 50-90 years, menopause, and clinical risk factors for osteoporosis. Patients with secondary osteoporosis were excluded. LS-BMD and TBS were measured at the L1-L4 vertebral level. The ability of the two diagnostic systems

Comparison of Patient Preference, Understanding, and Sentiment for Minimally Invasive Versus Open Spine Surgery.

STUDY DESIGN: Retrospective questionnaire analysis. OBJECTIVE: The goal of this study was to analyze patients' understanding and preferences for minimally invasive spine (MIS) versus open spine surgery. SUMMARY OF BACKGROUND DATA: MIS surgery is increasing in prevalence. However, there is insufficient literature to evaluate how the availability of MIS surgery influences the patients' decision-making process and perceptions of spine procedures. METHODS: A survey was administered to patients who received a microdiscectomy or transforaminal lumbar interbody fusion between 2016 and 2020. All el

Acute Onset of Guillain-Barré Syndrome after Multiple Spine Surgeries: A Rare Case Report.

Guillain-Barré syndrome is an autoimmune disorder presented by ascending paralysis and areflexia. The condition has been reported after many infections, but Guillain-Barré syndrome after spine surgery is rare. We, herein, present a case of Guillain-Barré syndrome after multiple spine surgeries for degenerative lumbar scoliosis. A 60-year-old woman with degenerative scoliosis underwent surgery for the third time and developed Guillain-Barré syndrome 3 weeks after the final operation. The patient received intravenous immune globulin therapy and needed mechanical ventilation and intensive care. S

Augmenting the Cobb angle: Three-dimensional analysis of whole spine shapes using Bézier curves.

BACKGROUND AND OBJECTIVE: The identification and classification of pathological spinal deformities poses a major challenge to any diagnostician. First, available medical images are usually two-dimensional projections, obscuring elaborated spatial information. Second, several measurement techniques with different thresholds for certain clinical syndromes make it difficult to classify measured results. Here, a method is presented to augment and standardize the analysis of spinal shapes in three dimensions. METHODS: Regarding the first limitation, (semi-)automatic, three-dimensional segmentation

Posterior Endoscopic Cervical Decompression in Metastatic Cervical Spine Tumors: An Alternative to Palliative Surgery.

Metastatic spinal cord compression of the cervical spine is a well-known consequence of cancer that generally manifests as an oncological emergency. This study presents and describes an alternative to the minimally invasive posterior full-endoscopic approach for direct decompression and tumor debulking from the metastasis of hepatocellular carcinoma (HCC) in the cervical spine. A 54-year-old man presented with progressive cervical radiculopathy that had persisted for 3 months. The underlying disease was HCC. Radiographic examination revealed evidence of metastatic spinal cord compression with

Aberrant cortical spine dynamics after concussive injury are reversed by integrated stress response inhibition.

Traumatic brain injury (TBI) is a leading cause of long-term neurological disability in the world and the strongest environmental risk factor for the development of dementia. Even mild TBI (resulting from concussive injuries) is associated with a greater than twofold increase in the risk of dementia onset. Little is known about the cellular mechanisms responsible for the progression of long-lasting cognitive deficits. The integrated stress response (ISR), a phylogenetically conserved pathway involved in the cellular response to stress, is activated after TBI, and inhibition of the ISR-even wee

KIF5C deficiency causes abnormal cortical neuronal migration, dendritic branching, and spine morphology in mice.

BACKGROUND: Malformation of cortical development (MCD) includes a variety of developmental disorders that are common causes of neurodevelopmental delay and epilepsy. Most recently, clinical studies found that patients carrying KIF5C mutations present early-onset MCD; however, the underlying mechanisms remain elusive. METHODS: KIF5C expression level was examined in mouse primary cortical neurons and human ips-derived forebrain organoids. We studied the cortical neuronal migration, dendritic branching, and dendritic spine growth after knocking down the KIF5C gene by electroporation in vitro and

Navigation-Guided Subaxial Cervical Pedicle Screws in Revision Spine Surgery: 2-Dimensional Operative Video.

Traditionally, lateral mass screws (LMSs) have been the mainstay of posterior fixation in the subaxial spine. Although LMSs provide adequate fixation, cervical pedicle screws (CPSs) facilitate high fusion rates (90.5%) and provide for greater bone purchase, better reduction, lower rates of screw loosening or pull out, 2 times greater biomechanical advantage, superior stabilization, decreased development of pseudarthrosis, and decreased revision surgeries compared to LMSs.1-4 In addition, CPSs can be a powerful bail-out option after lateral mass construct failure. Navigation-guided CPS placemen

Cervical Spine Reconstruction with Chest Tube Technique After Metastasis Resection: A Single-Center Experience.

BACKGROUND: The silastic tube technique, in which a chest tube is placed into the vertebral body defect and impregnated with polymethyl methacrylate, showed good results in patients with lumbar and thoracic neoplastic diseases. There has been only 1 study about the effectiveness and safety of this technique in patients with cervical metastases. We aimed to report our experience in using this technique to reconstruct the spine after corpectomy for cervical metastasis. METHODS: All patients with cervical spinal metastasis who underwent surgical treatment using a chest tube impregnated with poly

Vitamin D levels and pain outcomes in adolescent idiopathic scoliosis patients undergoing spine fusion.

PURPOSE: Prior research has indicated adolescent idiopathic scoliosis (AIS) patients have lower bone mineral density and lower vitamin D levels than healthy peers. Vitamin D deficiency has been associated with higher levels of pain. This study investigated whether vitamin D-deficient AIS patients had higher pain before or immediately after posterior spine fusion (PSF) surgery. METHODS: 25-Hydroxy vitamin D levels were tested in all AIS patients at their pre-operative appointment. Patients were grouped by serum 25-hydroxy vitamin D level: deficient, < 20 ng/mL; insufficient, 20-29 ng/mL; suffi
