A standardised protocol for the acute management of corrosive ingestion in children.

Oesophageal strictures developing after caustic ingestion in children are a serious problem, and several protocols to prevent stricture formation have been proposed. A prospective clinical trial was conducted for preventing strictures in caustic oesophageal burns in a single clinic, and the results are presented. All children with caustic ingestion who had oesophagoscopy for diagnosing the severity of the burn were included in the study. Eighty-one children were included in the series, with ages ranging between 3 months and 12 years. The patients were given nothing by mouth until oesophagoscop

Cardiac diphtheria in a previously immunized individual.

A previously healthy 19-year-old Asian female without significant past medical history presented to the emergency room complaining of a sore throat, difficulty in swallowing, fever, swollen neck, malaise, and myalgia for three to four days. The patient was initially seen at an outside hospital, evaluated by an ear, nose, and throat physician (ENT), and was found to have desquamative pharyngitis. The patient was transferred to our hospital after she continued to experience progressively worsening shortness of breath and went into acute respiratory distress. The patient was found to have larynge

Transient Foix-Chavany-Marie syndrome after surgical resection of a right insulo-opercular low-grade glioma: case report.

OBJECTIVE AND IMPORTANCE: We describe an atypical case of transient Foix-Chavany-Marie syndrome, or faciopharyngoglossomasticatory diplegia with automatic voluntary dissociation, occurring after surgical resection of a right insulo-opercular glioma. CLINICAL PRESENTATION: A 26-year-old right-handed man experienced partial seizures that were poorly controlled by antiepileptic drugs during a 2-year period as a result of a right insulo-opercular low-grade glioma, leading to the proposal of surgical resection. In addition, 1 year before the operation, the patient experienced a severe brain injury

Clinical benefits in endoscopic thyroidectomy by the axillary approach.

BACKGROUND: Surgical treatments for thyroid diseases require skin incisions that can result in prominent scars, complaints resulting from adhesions, hypesthesia, and paresthesia in the neck. We have developed an endoscopic thyroidectomy using an axillary approach. In this article, we compare our original technique with conventional open surgery from the aspects of surgical invasiveness and patients' complaints after surgery. STUDY DESIGN: Each procedure was performed in 20 patients with follicular tumors. The two groups were similar for age, gender, and the mean diameter of the thyroid tumor.

Laparoscopic Heller myotomy with fundoplication for achalasia.

BACKGROUND AND PURPOSE: Laparoscopic Heller cardiomyotomy for the treatment of achalasia can be performed safely. The application of this minimally invasive approach has not been reported in Taiwan. This study assessed the results obtained using this new method in Taiwanese patients. METHODS: From January 1998 to December 2000, we recruited 10 patients (3 men, 7 women; average age 37.3 yr) with achalasia who underwent laparoscopic cardiomyotomy and fundoplication. Before and after surgery, the severity of three symptoms (dysphagia, regurgitation, chest pain) was evaluated by symptom scores (0

Dysphagia and aspiration with unilateral vocal cord immobility: incidence, characterization, and response to surgical treatment.

To objectively determine the incidence of dysphagia associated with unilateral vocal cord immobility (UVCI) and to evaluate the potential for response to vocal cord medialization, we made videofluoroscopic swallowing (VFS) recordings of patients with newly diagnosed UVCI and prospectively analyzed them in a blinded fashion using the Penetration-Aspiration Scale (PAS) and pharyngeal transport function measures. A subset of patients underwent vocal cord medialization and were evaluated with a postoperative VFS study. Comparison was made between preoperative and postoperative VFS results to study

Safety of inhaled corticosteroids in children.

Inhaled corticosteroids (ICS) are now first-line therapy for persistent asthma in children. The major safety concerns of long-term ICS therapy for childhood asthma are potential effects on adrenal function, growth, and bone mass. Dosage, type of inhaler device, and individual drug characteristics influence systemic effects of ICS. Sensitive measures of basal adrenal function can show statistically significant changes during ICS therapy, but these do not accurately predict clinically meaningful adrenal axis suppression. Adrenal insufficiency is rare and confined to children receiving high doses

Improving the functional outcome of Tucker's reconstructive laryngectomy.

BACKGROUND: The goal of reconstructive laryngectomy addressing glottic carcinoma, as described by Tucker, is twofold: achieve local control and preserve as much as possible the physiological laryngeal function. METHODS: The study consisted of long-term assessment of oncological, deglutitory, and vocal outcome in 34 patients following Tucker's reconstructive laryngectomy. Postoperative follow-up included fiberscopic examination, videoradiography, and voice assessment after speech rehabilitation. RESULTS: The 5-year overall survival rate was 92% +/- 5%. Fiberscopic examination and videoradiogr

Detection of caustic oesophageal injury with technetium 99m-labelled sucralfate.

BACKGROUND/PURPOSE: Radiolabelled sucralfate has been used to show the extent and severity of reflux oesophagitis, peptic ulceration, and inflammatory bowel disease. Endoscopy under general anaesthetic has been the preferred method to assess the injury after caustic ingestion. The aim of this study was to assess whether sucralfate has an affinity for the chemically injured oesophageal mucosa and, if so, to assess the accuracy of radiolabeled sucralfate as an indicator of presence and extent of oesophageal injury. METHODS: A prospective study was conducted of 22 patients with mean age of 30 mo

Value of pharyngography in patients without suprasternal symptoms.

The purpose of the present paper was to determine if pharyngeal or cervical oesophageal lesions may present with distal symptoms. All patients presenting for barium swallow underwent examination of the pharynx and oesophagus. The pharyngeal examination included spot films of the pharynx as well as views of the pharyngo-oesophageal segment filmed at three frames per second. During the 18-month period of the present study interrogations were carried out to identify patients without symptoms in the cervical or suprasternal region. One hundred and twelve patients were identified; 58 were male and

Pharyngo-oesophageal dysphagia: surgery based on clinical and manometric data.

High or pharyngo-oesophageal dysphagia (PD) is defined as difficulty in initiating the act of swallowing within 1s. It involves the mechanisms controlling the tongue, pharynx and upper oesophageal sphincter (UOS) and is associated with a wide variety of local, neurologic and muscular disorders, and can also occur after surgery in the area and in response to gastro-oesophageal reflux (GOR). Our study aims at defining the criteria for surgery in PD and to evaluate the clinical results of such treatment. Twenty-three patients who underwent surgery were evaluated with pharyngo-oesophageal motility

Potential success of near-total laryngectomy despite postoperative radiation therapy.

Advanced laryngeal cancers frequently require total laryngectomy (TL). Some of these cancers, however, are suitable for near-total laryngectomy (NTL). We review our experience with NTL over a 14-year period and compare the functional results with those obtained over the same period using TL followed by tracheoesophageal puncture (TEP). One particular interest was the results achieved when surgery was preceded or followed by radiation therapy. From January 1980 through December 1994, 22 patients underwent NTL. The mean age of the 19 men (86.4%) and 3 women (13.6%) was 61.1 +/- 9.9 years. Follow

A large volume spacer significantly reduces the effect of inhaled steroids on bone formation.

Inhaled steroids are increasingly advocated as first line treatment for mild asthma. Some studies suggest that inhaled steroids suppress bone formation as reflected by a fall in plasma osteocalcin. Spacers have been shown to increase the proportion of inhaled aerosol that is deposited in the lungs and to reduce the amount swallowed. We measured plasma osteocalcin levels to determine the effect on bone formation of inhaled beclomethasone dipropionate (BDP) with and without a 750 ml spacer in a double-blind, randomised, placebo-controlled, cross-over study. Twenty-six healthy male volunteers too

[The experience of early rehabilitation].

Functional laryngectomies permit a more or less ideal preservation of laryngeal functions whose recovery, especially in les conservative operations, occurs very slowly and depends on several conditions: post operative course, sensitivity and motility of the hypopharynx, patient's ability to restore swallowing mechanisms. The Authors relate their experience concerning use of a rehabilitative program partially based on the experiences of some French logopedic schools and partially original. They illustrate the steps and goals of this program which starts on the fifth post-operative day with resp

Complications, Morbidity, and Rehabilitation After Treatment of Gingivobuccal Cancers.

Gingivobuccal malignancies involve structures essential for speech, swallowing, and facial contour. The management of these cancers requires a multidisciplinary approach, which is often associated with significant morbidities. The most concerning complications are trismus, osteoradionecrosis, oral incompetence, hardware-related complications and xerostomia, all of which can profoundly impair the quality of life. Recent surgical and radiation technique advancements, along with consistent implementation of rehabilitative protocols, have improved outcomes leading to better quality of life. This a

Feeding disorders in children with congenital heart disease: clinical implications and the role of the speech and language pathologist.

Medical and technological advances have increased the survival rate of children with congenital heart disease, which has led to a deeper understanding of various aspects of their subsequent development. Cardiovascular involvement has a direct impact on feeding, causing sucking and swallowing dysfunctions from birth that can trigger a series of consequences such as malnutrition, dysphagia, and clinical discomfort. This problem not only affects the physiological sphere but also the emotional and family dimensions. This article summarizes key findings in the growing literature on the impact of he

Orthodontics and Myofunctional Therapy: A critical review of the best complementary treatments of orofacial dysfunctions (Part 1).

AIM: This critical review article illustrates the anatomo-physiological complexity of the orofacial district that makes orthodontic and myofunctional therapies naturally complementary. METHODS: The evolution of the face and mouth's structures and functions intimately connects not just bones and muscles but several functions: breathing, sucking, swallowing, chewing, biting, protective mechanisms such as spitting or coughing, and parafunctions such as clenching or nocturnal bruxism. These functions (among others) are mediated by an extremely complex neural system that involves, affects, or is a

Modification of supraglottic partial laryngectomy to preserve hyoid bone: a cadaveric study.

BACKGROUND: Supraglottic partial laryngectomy (SGPL) preserving the hyoid bone is expected to maintain laryngeal elevation by the suprahyoid muscles. However, the surgical technique has not been adequately validated. We describe the procedure, which preserves the hyoid body and suprahyoid muscle complex to maintain laryngeal elevation and provides access to the pharynx. METHOD: Using cadaveric studies, we modified the SGPL procedure with a goal of preserving the hyoid bone, and performed the procedure in a 67-year-old man with large cell neuroendocrine carcinoma. CONCLUSION: This modified SG

The Role of Ultrasound in the Evaluation of Tardive Dyskinesia: A Case Series.

BACKGROUND: Despite efforts to visualize all the movements of tongue and oropharynx in individuals with focal movement disorders (specifically tardive dyskinesia (TD)), clinicians can miss the complete picture and additional tools may be required to reach an accurate diagnosis. CASES: We present three cases with TD where ultrasound assisted in diagnoses. These individuals had difficulty swallowing and abnormal sensations in the tongue, which remained undiagnosed until we performed ultrasound of oropharynx which allowed for characterization of these movements. DISCUSSION: Ultrasound is an ide
