Cerebral infarct complicating traumatic pneumatocele: a rare sequela following blunt chest trauma.

Systemic air embolism is known to rarely complicate blunt chest trauma. However, cerebral infarction caused by air emboli possibly originating from a traumatic pneumatocele has not been previously reported. We report a case of a 46-year-old woman who sustained blunt chest trauma with multiple rib and clavicular fractures, hemothorax and a huge, tense traumatic pneumatocele. She subsequently developed clinical and radiologic features of cerebral infarction. The cerebral infarct is likely to be secondary to cerebral air embolism originating from a traumatic pneumatocele.

[Surgery of traumatic tracheal and tracheobronchial injuries].

Tracheal injuries are altogether rare events and can be divided into three broad categories: tracheobronchial injuries caused by external violence, iatrogenic ruptures of the trachea and inhalation trauma. Successful management of tracheobronchial injuries requires a fast and straightforward diagnostic evaluation. In all severely injured patients with cervicothoracic involvement an injury of the tracheobronchial system should be actively excluded. Although it is commonly agreed that posttraumatic injuries require surgical intervention the management of iatrogenic injuries is presently shifting

Neo-coarctation after the arterial switch operation.

Neo-coarctation following arterial switch operation (ASO) for transposition of the great arteries (TGA) is a complication that is not regularly described, but may occur. We describe five patients who developed a neo-coarctation after operation. They were diagnosed with TGA, either with or without ventricular septal defect without signs or symptoms of a coarctation. Except for one patient, all patients were reoperated for a neo-coarctation within one year after the ASO. Several explanations are discussed as a possible cause for this phenomenon.

Asymptomatic paraganglioma of the posterior mediastinum misdiagnosed until operation.

A 23-year-old female patient was treated for a posterior mediastinal tumor of unknown histology. Her systemic blood pressure increased rapidly to a critical level immediately after starting surgical manipulation of the tumor, which was suggestive of a hyperfunctioning pheochromocytoma. The tumor was removed after controlling the blood pressure and was histologically diagnosed as an extra-adrenal pheochromocytoma. Further postoperative investigations showed the tumor to be a sporadic type through further postoperative investigations and the patient had subsequently an uneventful course.

Schwannoma mimicking liver tumor.

Thoracic neurogenic tumors typically originate from the posterior mediastinum and the intercostal nerves. No report of a chest wall schwannoma extending toward the subphrenic areas and making a significant indentation into liver parenchyma exists to date. We present a liver tumor-mimicking schwannoma of the intercostal nerves. A 58-year-old woman presented with a painful lesion in the right subphrenic area and abdominal pain in the right upper quadrant for two months. Abdominal ultrasonography and magnetic resonance imaging revealed a tumor, 9.1 x 7.1 x 8.9 cm in size, with an inner cystic cha

Three-dimensional CT image analysis of a tracheal bronchus in a patient undergoing cardiac surgery with one-lung ventilation.

The incidence of a tracheal bronchus--that is, a congenitally abnormal bronchus originating from the trachea or main bronchi--is 0.1%-2%. Serious hypoxia and atelectasis can develop in such patients with intubation and one-lung ventilation. We experienced a remarkable decrease in peripheral oxygen saturation (SpO2) and a rise in airway pressure during placement of a double-lumen endobronchial tube in a patient with patent ductus arteriosus and tracheal bronchus. Substitution of the double-lumen tube with a bronchial blocker tube provided secure isolation of the lung intraoperatively. A type I

Esophageal tear in a patient undergoing stereotactic brain biopsy under general anesthesia.

Injuries of the esophagus with resultant mediastinitis have been reported following endotracheal intubation. Herein, we report a case of esophageal perforation that resulted from difficulty with intubation in a patient with a stereotactic head frame. A 52-year-old woman underwent a stereotactic brain biopsy of a left temporal tumor. After a stereotactic head frame was applied, intubation for anesthesia required three attempts. On postoperative day 2, she complained of worsening dysphagia and chest pain. A 4-mm tear in the right posterior cervical esophagus was discovered and repaired. Esophage

Natural history of spinal anomalies and scoliosis associated with esophageal atresia.

OBJECTIVE: Clinical characteristics of spinal deformities that commonly occur in patients with esophageal atresia (EA) are unclear. The objective of this study was to assess the incidence and natural history of spinal anomalies and scoliosis in patients with EA. METHODS: A population-based cohort of 100 adults who had an operation for EA in our hospital were examined clinically and radiographically for spinal deformities. The results were compared with data obtained from normal population-based controls. RESULTS: Vertebral anomalies were observed in 45 patients, predominating in the cervical

Giant primary mediastinal hydatid cyst causing Horner's syndrome: report of a case.

As the liver and the lungs are the most common sites for hydatid cysts, a primary mediastinal involvement is quite rare. The symptoms related to primary mediastinal hydatid cysts usually depend on the size, location and compression to nearby structures. Presenting a 51-year-old male patient with the symptoms of Horner's Syndrome, we draw attention to a rare complication of a giant primary mediastinal hydatid cyst and its challenging management.

Inverted L-shape sternotomy as a minimally invasive approach: re-do cardiac surgery for papillary muscle rupture.

We report a successful treatment of the complete papillary muscle rupture occurring 16 months after coronary artery bypass grafting (CABG). A 57-year-old man was admitted for the sudden onset of chest pain and cardiogenic shock. Emergency cardiac catheterization revealed severe mitral regurgitation and total occlusion in the right coronary artery, which was successfully revascularized by percutaneous coronary intervention under intra-aortic balloon pumping. The right internal thoracic artery grafted to the left anterior descending artery in the previous CABG was functioning well. An echocardio

Successful management of severe chylothorax with argon plasma fulguration and fibrin glue in a premature infant.

Post-traumatic neonatal chylothorax is a rare entity. Management includes medical and surgical treatment. We describe here a newborn who developed a severe chylothorax after repair of an oesophageal atresia. The chylothorax was treated successfully by a combination of argon beam coagulation of the mediastinum and fibrin glue application. However, the patient developed complete thrombosis of the left femoral vein with clot extension to the inferior vena cava which resolved after infusion of recombinant tissue plasminogen activator (rt-PA). The use of argon plasma coagulation and mediastinal fib

Operative stabilization of a flail chest six years after injury.

We report a case of operative stabilization of an incompetent upper chest wall 6 years following flail chest. The indications for stabilization were chronic pain and dyspnea associated with rib malunion and loss of hemithorax volume. At operation, multiple pseudoarthroses were encountered and partial resection of ribs three and four was required. Malleable plates were used to bridge the gaps created by the resection and were secured in place with sternal wire. The patient reported a dramatic relief of symptoms and, at 18 months postoperatively, continues to work full-time on his cattle ranch e

Esophagogastrectomy via left thoracophrenotomy.

Esophagogastrectomy is generally considered to be the treatment of choice for resectable tumors of the esophagus. Although many approaches and techniques have been advocated, since April 1983 we have used a left thoracophrenotomy approach for most lesions of the lower two thirds of the esophagus and gastric cardia. Stapling instruments have been used for mobilization of the stomach and fashioning of the esophagogastric anastomosis. One-hundred fifteen patients undergoing resection of malignant tumors with this technique were retrospectively reviewed. Perioperative mortality was 8.7% (10/115).

Pulmonary Resection for Bronchiectasis: Perioperative and Follow-Up Outcomes in a VATS-Dominant Cohort.

Background/Objectives: Surgical resection remains an important treatment option for selected patients with bronchiectasis who have persistent symptoms, recurrent pulmonary infections, or hemoptysis despite medical therapy. This study evaluated perioperative and follow-up outcomes after pulmonary resection for bronchiectasis in a predominantly video-assisted thoracoscopic surgery (VATS)-treated cohort. Methods: This retrospective single-center cohort included patients who underwent pulmonary resection for bronchiectasis between January 2009 and January 2026. Baseline and follow-up data were ana

Minimally-invasive tricuspid valve operations: three decades of Leipzig experience.

OBJECTIVES: Data on minimally invasive tricuspid valve surgery for severe tricuspid regurgitation remain limited, particularly regarding long-term outcomes. METHODS: This retrospective single-center study included adult patients who underwent minimally invasive tricuspid valve surgery through right mini-thoracotomy for severe tricuspid regurgitation as the primary indication between 1996 and 2024. Patients with endocarditis or tricuspid stenosis, as well as those undergoing left-sided valve surgery, were excluded. The primary endpoint was all-cause mortality during follow-up. RESULTS: A tota

Clostridium septicum Aortitis and Mediastinitis Complicating Perforated Appendicitis in a Patient with Chronic Myelomonocytic Leukemia: A Case Report.

Clostridium septicum is an aerotolerant anaerobe classically linked to occult colorectal malignancy and hematologic disease, and a rare but frequently fatal cause of nontraumatic infective aortitis; it also causes gas gangrene, osteomyelitis and spondylodiscitis. Survival depends on radical surgical debridement, as antibiotics alone are almost uniformly fatal. We describe a 71-year-old man with untreated chronic myelomonocytic leukemia (CMML) who presented with back pain, vomiting and rigors. Computed tomography showed perforated appendicitis with extraluminal air together with mediastinitis a
