Pathophysiology of congenital diaphragmatic hernia. XI: Anatomic and biochemical characterization of the heart in the fetal lamb CDH model.

AIM: The purpose of this study was to determine whether the presence of bowel in the chest during development in the fetal lamb model of congenital diaphragmatic hernia (CDH) results in structural and/or biochemical hypoplasia of the left venticle. METHODS: The model was created at 80 days' gestation and delivered at term. The hearts were fixed in 4% formaldehyde solution, components weighed, and right ventricular (RV) and left ventricular (LV) wall thicknesses and both aortic (Ao) and pulmonary artery (PA) root diameters were measured. Fresh specimens were analyzed for protein, DNA, hydroxyp

Factors predisposing to ventricular tachyarrhythmia leading to appropriate ICD intervention in patients with coronary artery disease or non-ischaemic dilated cardiomyopathy.

BACKGROUND: In order to achieve optimal outcomes when treating ventricular tachyarrhythmias with implantable devices, it is extremely important to identify parameters predisposing to arrhythmia. In view of current restrictions in healthcare funding, there is a growing demand for additional predictors of arrhythmia that would allow better patient selection for implantable cardioverter-defibrillator (ICD) use for primary prevention of sudden cardiac death (SCD). AIM: To identify parameters predisposing to ventricular tachyarrhythmia/appropriate ICD intervention in ICD recipients. METHODS: We a

Hemodynamic assessment of critically ill patients using a miniaturized transesophageal echocardiography probe.

INTRODUCTION: Hemodynamic management in intensive care patients guided by blood pressure and flow measurements often do not sufficiently reveal common hemodynamic problems. Trans-esophageal echocardiography (TEE) allows for direct measurement of cardiac volumes and function. A new miniaturized probe for TEE (mTEE) potentially provides a rapid and simplified approach to monitor cardiac function. The aim of the study was to assess the feasibility of hemodynamic monitoring using mTEE in critically ill patients after a brief operator training period. METHODS: In the context of the introduction of

Cardiac Mitochondrial Dysfunction Following Bleomycin-Induced Acute Lung Injury in Rats.

BACKGROUND: Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are frequently associated with cardiac complications, including myocardial injury and right ventricular dysfunction. However, the mechanisms linking pulmonary injury to cardiac dysfunction remain incompletely understood. In this study, we investigated ventricular mitochondrial respiratory function during the acute phase of bleomycin-induced ALI. METHODS: ALI was induced in male and female rats by intratracheal bleomycin (2.5 mg/kg); saline served as a control. Circulating cardiac troponin I (cTnI) was measured

Cardiorespiratory coupling during high-frequency oscillatory ventilation with volume guarantee in extremely preterm infants.

UNLABELLED: This study aims to evaluate associations between PCA-derived cardiovascular components and respiratory burden during high-frequency oscillatory ventilation with volume guarantee (HFOV-VG). This retrospective cohort study included infants born before 30 weeks of gestation who received HFOV-VG and underwent targeted neonatal echocardiography during respiratory support. Echocardiography assessed ventricular systolic function, cardiac output, myocardial deformation, and pulmonary vascular loading. Principal component analysis (PCA) derived integrated cardiovascular components. Multivar

Characterization and z-score calculation of cardiovascular magnetic resonance imaging parameters in patients after the Fontan operation: A Fontan Outcome Registry using Cardiovascular Magnetic Resonance Examinations study.

BACKGROUND: Cardiac magnetic resonance (CMR) offers valuable hemodynamic insights post-Fontan, but is limited by the absence of normative single ventricle data. The Fontan Outcomes Registry using CMR Examinations (FORCE) is a large international Fontan-specific CMR registry. This study used FORCE registry data to evaluate expected CMR ventricular size/function and create Fontan-specific z-scores adjusting for ventricular morphology (VM) in healthier Fontan patients. METHODS: "Healthier" Fontan patients were defined as patients free of adverse outcomes, who are New York Heart Association class

Case report: A complex case of valve-in-valve TAVI and left bundle branch pacing for severe aortic regurgitation with partially corrected type A aortic dissection and low ejection fraction.

BACKGROUND: Aortic regurgitation is a major concern following transcatheter aortic valve implantation (TAVI), as even low-grade regurgitation is associated with increased mortality. This is of particular concern to patients with pre-existing aortic disease who are at increased risk of TAVI valve slippage. Furthermore, conduction system disturbances after TAVI, namely left bundle branch block (LBBB), may have an additional detrimental effect on cardiac function. CASE PRESENTATION: This report documents a successful treatment strategy in a frail patient with a bicuspid aortic valve and aortic d

Description of a new clinical syndrome: thoracic constriction without evidence of the typical funnel-shaped depression-the "invisible" pectus excavatum.

Pectus excavatum (PE) is a congenital malformation with a funnel-shaped depression of the sternum that can lead to cardiac symptoms. However, there are patients with thoracic constriction (defined as elevated Haller-Index > 3.25 determined by cardiac magnetic resonance imaging (CMR)) without visible evidence of PE, leading to similar complaints. Between January 2004 till June 2020, patients who underwent CMR for further evaluation of the heart, due to cardiac symptoms were enrolled and compared to controls. Biventricular global strain analysis was assessed using feature tracking (CMR-FT). ECG

Long-term outcome of patients undergoing surgical repair of isolated pulmonary valve stenosis. Follow-up at 20-30 years.

In a group of 191 consecutive patients who underwent operation at the Mayo Clinic for isolated pulmonary valve stenosis between 1956 and 1967, late results were excellent. In patients surviving operation before the age of 21 years, long-term survival was similar to that in an age- and sex-matched control population. In the other patients, late survival and functional status were good, but survival was poorer than in the control population. This late attrition, most likely due to the sequelae of long-standing right ventricular hypertrophy, suggests the need for continued surveillance. Late sudd

Persistent left superior vena cava is associated with complex atrial tachyarrhythmias in repaired tetralogy of Fallot: evidence for a right-sided arrhythmogenic substrate.

BACKGROUND: Atrial fibrillation (AF) is an important late complication in patients with repaired tetralogy of Fallot (TOF); however, its clinical determinants and arrhythmogenic substrates remain uncertain. Identifying high-risk anatomical profiles is essential for optimal rhythm management and tailored ablation strategies. METHODS: We retrospectively analyzed 137 consecutive patients with repaired TOF followed at the University of Tsukuba Hospital (2013-2024). Clinical characteristics, echocardiographic parameters, and procedural findings were compared between patients with and without AF. L

INTRODUCTION: Antemortem diagnosis of pulmonary tumor thrombotic microangiopathy (PTTM) is challenging because of rapidly worsening respiratory failure. Vascular endothelial growth factor (VEGF) is involved in PTTM pathogenesis; however, the clinical picture of PTTM in patients with cancer receiving anti-VEGF treatment is unknown. CASE PRESENTATION: A 40-year-old man with advanced gastric adenocarcinoma on paclitaxel plus ramucirumab developed a dry cough and, after 2 months of a stable period, dyspnea on exertion. Chest computed tomography (CT) showed bilateral diffuse patchy ground-glass op

Migration of a ventriculoperitoneal shunt to the mediastinum: A case report and literature review.

RATIONALE: Ventriculoperitoneal shunt (VPS) surgery, a common neurosurgical procedure for hydrocephalus, carries risks of complications such as shunt obstruction and infection. Migration of the shunt into the mediastinum, however, is an uncommon complication, particularly in adults. PATIENT CONCERNS: A 56-year-old male patient was admitted to our hospital with a complaint of "progressive decline in mobility over 3 months, significantly impairing daily activities." He had previously undergone bilateral VPS surgery and was admitted with a suspected shunt obstruction. DIAGNOSES: Chest radiograp

Right Atrial Appendage for Aortic Valve Reconstruction.

BACKGROUND: Congenital aortic valve disease, often bicuspid, is a common problem. Current repair techniques, including valve replacement or use of pericardium for repair, are imperfect and limited by lack of ideal tissue. Based on our experience using live right atrial appendage (RAA) tissue for pulmonary valve reconstruction, we describe preliminary results using this tissue for aortic valve reconstruction. METHODS: Six patients with unicuspid or bicuspid aortic valves and predominant aortic insufficiency were referred for intervention. Intraoperative assessment of the native valve was follo

Role of Left Ventricular Dysfunction in Systemic Sclerosis-Related Pulmonary Hypertension.

BACKGROUND: In systemic sclerosis (SSc), pulmonary hypertension remains a significant cause of morbidity and mortality. Although conventionally classified as group 1 pulmonary arterial hypertension, systemic sclerosis-related pulmonary hypertension (SSc-PH) is a heterogeneous disease. The contribution of left-sided cardiac disease in SSc-PH remains poorly understood. RESEARCH QUESTION: How often does left ventricular (LV) dysfunction occur in SSc among patients undergoing right heart catheterization and how does coexistent LV dysfunction with SSc-PH affect all-cause mortality in this patient

Isolated right ventricular myocardial infarction: a case report.

BACKGROUND: Isolated right ventricular myocardial infarction (RVMI) due to a recessive right coronary artery (RCA) occlusion is a rare presentation. It is typically caused by right ventricle (RV) branch occlusion complicating percutaneous coronary intervention. We report a case of an isolated RVMI due to flush RCA occlusion presenting via our primary percutaneous coronary intervention ST-elevation myocardial infarction pathway. CASE SUMMARY: A 61-year-old female smoker with a history of hypercholesterolaemia presented via the primary percutaneous coronary intervention pathway with sudden onse

Clinical significance of spleen stiffness in patients with acute decompensated heart failure.

AIMS: Congestive splenomegaly is a classic sign of organ congestion in acute decompensated heart failure (ADHF). Shear wave elastography (SWE) allows the measurement of spleen stiffness (SS). We hypothesized that SS could quantify the severity of splenic congestion and predict adverse events in ADHF. METHODS AND RESULTS: This study included two cohorts: a haemodynamic cohort (62 HF patients) and an outcome cohort (115 ADHF patients). SS was measured by two-dimensional SWE on the same day of right heart catheterization in the haemodynamic cohort. Right atrial pressure (RAP) independently corre

How to repair an acquired Gerbode defect using an aortic root xenograft.

A 74-year old female patient presented with a 3-week history of fever, general weakness, and later developed complete heart block. On admission, the patient was febrile (temperature of 39°C) and tachypnoeic (respiratory rate of 29/min) with oxygen saturation of 95% on 2 L of oxygen. Her heart rate was 60 beats/min (VVI pacing), arterial blood pressure was 135/60 mm Hg and the Glasgow Coma Scale was 15. On chest auscultation, she had a harsh systolic murmur over her left precordium and bilateral rales. Blood tests demonstrated moderate anemia (hemoglobin of 95 g/L), leucocytosis (white blood ce

A case report: intravalvular regurgitation during percutaneous valve-in-ring implantation due to eccentric bulging of a balloon-expandable valve in a patient with severe right heart failure.

BACKGROUND: Severely reduced right heart function and high operative risk are major challenges in the treatment of tricuspid regurgitation (TR) as both can lead to low cardiac output heart failure (LCO-Hf). Alternative methods and criteria for patient selection are actively being sought. CASE SUMMARY: We report on a 66-year-old patient with severe right heart failure (rHF) with recurrent TR after prior surgical valve repair with a 32-mm-Edwards-MC3 annuloplasty ring (AR). Surgical revision was discarded due to extreme high surgical risk. A right ventricular assist device was discussed but dec
