Bruising: when it is spontaneous and not idiopathic thrombocytopenia purpura.

Bruising is a presentation that often causes concern. There are many causes of bruising in children, including non-accidental injury, which must be excluded. We report a case of a 22-month-old boy where all the common diagnoses were excluded. We highlight the need to be aware of transient acquired inhibitors of coagulation that can cause spontaneous bleeding.

Homemade Steam Inhalation Therapy Devices: A Neglected Cause of Severe Pediatric Burns-An Urgent Call for Education and Safety Measures.

Steam inhalation therapy is common practice for coryzal symptoms, especially in children. However, it can cause major scald injuries due to accidental contact with the hot water. The authors used an electronic database to retrospectively identify all children admitted with burn injuries secondary to steam inhalation therapy during a 5-year period from September 2019 to August 2024 at Trousseau Hospital pediatric burn unit. Data collected included patient demographics, mechanism of burn, burn areas and extent, need of surgical treatment, type of hospitalization, length of stay, infections, and

Multidisciplinary Management of Low-velocity Nonmissile Penetrating Head Injuries.

Introduction Penetrating head injuries (PHIs) can have diverse presentations and mechanisms; therefore, treatment methods have not been clearly outlined. Vascular injury is common and foreign body removal is often required. We present three cases to illustrate low-velocity nonmissile penetrating head injuries (NPHIs) and discuss a multidisciplinary approach. Methods We present a case series from our institution that illustrates the importance of multidisciplinary treatment of these injuries. All injuries are low- velocity NPHIs with separate mechanisms and anatomical locations. Results Multidi

Influence of age and fall type on head injuries in infants and toddlers.

UNLABELLED: Age-based differences in fall type and neuroanatomy in infants and toddlers may affect clinical presentations and injury patterns. OBJECTIVE: Our goal is to understand the influence of fall type and age on injuries to help guide clinical evaluation. DESIGN/SETTING/PARTICIPANTS: Retrospectively, 285 children 0-48 months with accidental head injury from a fall and brain imaging between 2000 and 2006 were categorized by age (infant ≤1 year and toddler=1-4 years) and fall type: low (≤3 ft), intermediate (>3 and <10 ft), high height falls (≥10 ft) and stair falls. OUTCOME MEASURES: C

Bruising in Infants: An Approach to the Recognition of Child Physical Abuse.

Infants (children <12 months of age) are maltreated at more than twice the rate of any other child age group, and infants die because of maltreatment at 3 times the rate of any other age group in childhood. The incidence of hospitalization for serious physical abuse in children also is highest for infants. Successful recognition of medically mild signs of physical abuse in infants, such as certain bruising patterns, can, therefore, lead to lifesaving interventions. The importance of the recognition of medically mild injuries due to physical abuse is underscored by the finding that a high perce

Accidental vaginal incision during cesarean section. A report of four cases.

BACKGROUND: Transverse vaginal incision during cesarean section, also known as anterior vaginotomy, is a recognized entity, mostly made unintentionally. CASES: At our institution, four patients underwent a transverse vaginal incision during cesarean section over an eight-year period. In three of them, excessive bleeding required blood transfusion. In one case, only hypogastric artery ligation resulted in bleeding control. None of the patients had subsequent vaginal deliveries. CONCLUSION: The exact incidence of anterior vaginotomy is difficult to evaluate. Accidental vaginal incision occurs

Free flow associated with electronic infusion devices: an underestimated danger.

Accidental, uncontrolled free flow after removal of administration sets from electronic infusion devices (EIDs) has only occasionally been reported. Recent evidence indicates that this problem is far more widespread and serious than previously understood. Well-documented cases of accidental death and serious injury have now come to light and are a particular cause for concern. Many of the EIDs commonly used in hospitals incorporate sets that offer no protection if someone removes the set from the device without first closing the set's flow control clamp. Because the problem has infrequently be

Non-Fatal accidental strangulation in a stroke patient.

Non-fatal strangulation poses a significant concern in clinical forensic medicine. Indeed, surviving victims often present a diverse array of signs and symptoms. In rare instances, acute pathological events that manifest with symptoms similar to those secondary to neck compression (e.g., stroke with subsequent neurological symptoms, etc.) can lead to loss of consciousness, inadvertently resulting in strangulation. Therefore, in this context, forensic investigation plays a crucial role in interpreting the signs and symptoms presented by the victim and determining whether they are the cause or c

The epidemiology of hand injuries in The Netherlands and Denmark.

Little is known about the magnitude of hand injuries at national levels. This paper quantifies and characterises the incidence of hand injuries that require acute medical attention at Emergency Departments in The Netherlands and Denmark. Except for the incidence rate and the referral after treatment, the overall picture of hand injuries is similar for both countries: hand injuries show a peak for teenagers, result mainly from home and leisure accidents, are mainly caused by objects and falls, the majority affect fingers and result mainly in superficial injuries, open wounds and fractures, a sm

[Level of total injury severity as a possible parameter for evaluation of height in fatal falls].

The injuries caused by high falls from over the seventh floor are considered mostly fatal. It is possible to survive the high fall from over 30 metres at landing on the surface of high deformity (snow or water). The Injury Severity Score (ISS) due to a high fall is influenced by the height from which the body has fallen, then, the body and surface features, and finally, the manner of body impact on to the surface. The aim of the study was determining the possibility of the height estimation that caused the fatality of the impact, considering the severity of injury expressed by ISS. A retrospec

Voices from the Ground: Community Perspectives on Preventing Unintentional Child Injuries in Low-Income Settings.

Unintentional injuries significantly contribute to mortality and morbidity among children under five, with higher prevalence in low- and middle-income countries (LMICs). Deprived communities in these regions face increased injury risks, yet there is limited research on child safety tailored to their unique challenges. To address this gap, we conducted focus group discussions in rural Uganda, involving parents, village health workers, community leaders, teachers, and maids. The objective was to understand community perceptions around child safety and determine what culturally and age-appropriat

Naphthalene ball poisoning: a rare cause of acquired methaemoglobinaemia.

A 15-year-old boy presented to emergency services with accidental naphthalene ball ingestion. Following consumption he developed methaemoglobinaemia, massive intravascular haemolysis and acute kidney injury. He had no history suggestive of congenital haemoglobin M disease. Development of severe methaemoglobinaemia and intravascular haemolysis is quite unusual after consumption of a single ball of naphthalene. The patient was managed with ascorbic acid and intravenous N-acetyl cysteine. He also required haemodialysis for acute kidney injury that developed secondary to pigment nephropathy.

[Acute and subacute chemical pneumonitis].

Acute or subacute chemical-induced lung injury is rarely compound specific and is most often caused by an accidental occupational, domestic or environmental exposure to an inhaled chemical agent. The industrial disaster that happened in Bhopal in 1984, accidental poisoning with chlorine and petroleum hydrocarbons and also vesicant gases used during conflicts, are specific examples. Rarely, a chemical agent can cause lung damage by being ingested and reaching the lung through the systemic circulation (for example accidental or deliberate paraquat ingestion). Household accidents should not be un

Surgical gloves.

Surgeons and obstetricians, over the centuries, were only too aware that accidental open injuries during their work, especially in a septic case, could lead to an infected wound, a fulminating illness and often death. Even before the bacterial nature of infection had been established in the mid 19th century, it was still obvious that this dangerous and often fatal condition was caused by the transfer of some poisonous material or 'miasma' from the patient to his surgeon. As long ago as 1758, an obstetrician named Walbaum protected his hands by covering them with sheep's caecum. Others used glo

Transorbital intracranial impalement injuries by wooden foreign bodies: clinical, radiological and forensic aspects.

Facial impalement injuries involve the danger of diagnostic pitfalls. Even if the penetrating object extends into the cranial cavity, the clinical symptoms and the radiological signs may be uncharacteristic. In order to illustrate the diagnostic problems, two cases of accidental impalement are reported. In both of them, a wooden foreign body penetrated via the orbita into the cranium and remained undetected at first. Imaging by CT and MRI is presented and compared with regard to the respective diagnostic validity of these methods. Due to its specific anatomic configuration, the orbita constitu
