Corticotroph deficiency usually results from exogenous glucocorticoid therapy or, more rarely, from hypothalamopituitary damages. Its diagnostic may be difficult, especially when the deficit in ACTH secretion is partial. Stimulation tests are then necessary to investigate the integrity of the hypothalamopituitary-adrenal axis. The insulin tolerance test has become the gold standard, but its utilization is limited by several contra-indications. The metyrapone and cosyntropin tests represent currently used alternatives but their sensitivity is not optimal. Measurement of basal plasma dehydroepia

Blood eosinophilia generally indicates an underlying allergic, infectious or hematologic disease. Corticoadrenal insufficiency is known to be another cause of blood eosinophilia. Eosinophilic cystitis is a rare disease in which the bladder wall is infiltrated by eosinophils; however, the etiology of eosinophilic cystitis remains unclear. We report a case of corticoadrenal insufficiency with blood eosinophilia developing gross hematuria and eosinophilic cystitis. The patient was treated with medical therapy, including oral corticosteroids, obtaining excellent results.

Organ specific autoimmune diseases are predominantly diseases of the endocrine glands and involve amongst others the thyroid (Hashimoto's disease, primary myxoedema, Graves' disease), the islets of Langerhans (type I diabetes mellitus) and the adrenals (Addison's disease). Over the past fifty years the knowledge on the pathogenesis of these diseases has considerably increased, leading to a large number of newly developed diagnostic tools, particularly determination of autoantibodies. Most of these autoimmune diseases have a (long) subclinical latency period. During this latency period a reliab

Sarcoidosis meets the criteria for multi-organ disease, which differs from systemic disease in the following points: The preferred primary organ (lungs) is known, immunological processes take place only in the organs involved (T helper lymphocyte accumulation), the clinical picture depends upon the organ involvement, and granulomas are typical of the disorder. The clinical significance of the various organ manifestations covers a broad spectrum, which can also include a threat to life (myocardium), blindness, chronic invalidism (pulmonary fibrosis, hepatopathy, nephropathy), and cosmetic impai

Laugier-Hunziker syndrome is a rare, acquired disorder characterized by lenticular hyperpigmentation of the oral mucosa and longitudinal melanonychia. We present the case of a 63-year-old female with progressive, asymptomatic hyperpigmentation of buccal mucosa and a 7-year history of hyperpigmentation in several fingernails. Laugier-Hunziker syndrome was diagnosed based on the clinical features presented, dermoscopic findings and exclusion of underlying systemic diseases. Laugier-Hunziker syndrome is regarded as a diagnosis of exclusion. By identifying Laugier-Hunziker syndrome, other, more se

Autoimmune oophoritis is a rare cause of premature ovarian failure. Previous histological descriptions are sparse, with even fewer reports of cystic ovaries associated with this condition. Two cases of autoimmune oophoritis presenting as cystic ovaries with menstrual abnormalities are described, with immunocytochemical analysis of the inflammatory cell infiltrate. Serum autoantibodies to ovary and adrenal were present in both cases. Although rare, recognition of the condition by histopathologists is important because of the strong association with other 'organ-specific' autoimmune diseases, es

To differentiate between thyrotoxic myopathy and myodystrophy (of the limb-girdle type), severe myasthenia, polymyositis, Addison's disease, proximal spinal muscular atrophy, steroid myopathy and neurosis special diagnostic indices have been developed based on specific muscular weakness (and atrophy) pattern early in the disease, the sequence of separate muscles involvement as the pathological process progresses, disproportion between muscular weakness and atrophy, excessive folds of the skin above the affected muscles and the presence of deep reflexes, purposeful analysis of the anamnesis. The regulation of plasma beta-melanocyte-stimulating hormone (beta-MSH) in man has been studied utilizing a radioimmunoassay previously described (1). In normal subjects plasma beta-MSH values ranged from 20 to 110 pg/ml. Metyrapone increased and dexamethasone decreased plasma beta-MSH levels. Surgical stress stimulated beta-MSH secretion. Plasma beta-MSH levels were elevated in patients with untreated Addison's disease and untreated congenital adrenal hyperplasia, and these levels fell to normal during glucocorticoid therapy. In patients with Cushing's syndrome due to pituitary adrenocorticot

OBJECTIVE: Autoimmune-polyendocrinopathy-candidiasis-ectodermal-dystrophy (APECED) is a rare syndrome characterized by chronic candidiasis, chronic hypoparathyroidism and Addison's disease. APECED has been associated with mutations in autoimmune regulator (AIRE) gene. Our aim is to perform a genetic analysis of the AIRE gene in Italian APECED patients and in their relatives. Design AIRE mutations were determined by DNA sequencing in all subjects. Patients were tested for clinical autoimmune or non-autoimmune diseases, or for organ and non-organ specific autoantibodies.
PATIENTS: A total of 24

Jane Austen died 200 years ago at the age of 41 and authors have attributed her premature death to a wide variety of causes, which include Addison's disease and lymphoma. We have reviewed all of her available letters and extricated relevant medical information which reveal rheumatism, facial skin lesions, fever and marked fluctuation of these symptoms. The severity of these symptoms increased, leading to her death within a year. This range of clinical features fulfils the most recent classification criteria for systemic lupus erythematosus.

A 46 years old female, presented with severe fatigue, nypotension ana hyperpigmentation. Her basal serum cortisol level at 8 a.m. was < 1.0 μg/dl which suggested a diagnosis of Addison's disease. An association with latent autoimmune diabetes of adult and autoimmune hypothyroidism led to a diagnosis of Polyglandular Autoimmune Syndrome type II (PAS II). She also had alopecia universalis and hypoparathyroidism which are very rare in PAS type II syndrome. On treatment with hydrocortisone and fludrocortisone there was drastic improvement in the clinical features.

Adrenal function in twenty-three patients with paracoccidioidomycosis (South American Blastomycosis) has been assessed by measuring the response to adrenocortical stimulation with 1-24 ACTH. Two patients with overt Addison's disease showed very low basal levels and the complete absence of an increase in either cortisol or aldosterone secretion. Six patients showed probable diminished adrenal reserve in terms of cortisol and three patients showed diminished reserve in terms of aldosterone function. These findings indicate an incidence of significant hypoadrenalism in 44% of hospitalized patient

The interrelationship between prolactin (PRL) and the immune system have been elucitaded in the last decade, opening new important horizons in the field of the immunoendocrinology. PRL is secreted not only by anterior pituitary gland but also by many extrapituitary sites including the immune cells. The endocrine/paracrine PRL has been shown to stimulate the immune cells by binding to PRL receptors. Increased PRL levels, frequently described in autoimmune diseases, could depend on the enhancement of coordinated bi-directional communications between PRL and the immune system observed in these di

Infection by viral or bacterial pathogens has been suspected in playing a role in the development of autoimmune thyroid disease. Because Helicobacter pylori might be involved in the development of nongastrointestinal conditions such as rosacea, ischemic heart disease, and diabetes mellitus, we evaluated the prevalence of H. pylori infection in patients with autoimmune thyroid disease. Fifty-nine patients with autoimmune thyroid disease were included: autoimmune atrophic thyroiditis (n=21), Hashimoto's thyroiditis (n=18), and Graves' disease (n=20). Twenty patients with nontoxic multinodular go

Fludrocortisone acetate (Florinef) is a synthetic steroid with a potent mineralocorticoid action, and used for treatment of Addison's disease and salt losing form of congenital adrenal hyperplasia. It is also used for severe hypotension to restore adequate plasma volume. Caution is necessary for possible side effects of hypernatremia, hypokalemia, hyperglycemia and congestive heart failure when used chronically. A synthetic steroid which selectively binds and activates renal MR-LBD but not GR, or non-epithelial MR, although not found at present, may be a promising mineralocorticoids to be used

Deviation of cortisol metabolism in favour of its 6 beta-hydroxylated derivative was demonstrated in two patients with adrenal failure receiving substitution corticosteroid therapy and rifampicin. The existence of a frank increase in the metabolic clearance antipyrin was in favour of an hepatic enzyme induction. After rifampicin treatment was stopped, the 24 hour urinary excretion of 6 beta-OH-F returned to normal, demonstrating the responsability of the drug. This enzyme induction results in a need to increase the dose of hydrocortisone substitution therapy in patients with Addison's disease
