Release of markedly increased quantities of prostaglandin D2 from the skin in vivo in humans following the application of sorbic acid.

BACKGROUND AND DESIGN: Sorbic acid is a preservative that can induce cutaneous vasodilation characterized by erythema, urticaria, and stinging when applied topically to humans. Previous studies have suggested that prostaglandins may mediate the vasodilation, but the prostaglandin responsible has not been established. Recently, we have shown that cutaneous erythema similar to that associated with the application of sorbic acid is induced by topical administration of methylnicotinate and is mediated by the release of prostaglandin D2 (PGD2) from the skin. Therefore, we examined whether the cutan

Optimal Route of Tranexamic Acid Administration in Total Knee Arthroplasty: A Bayesian Network Meta-Analysis with Decision-Analytic Cost Evaluation.

BACKGROUND: Tranexamic acid (TXA) is widely used in total knee arthroplasty (TKA) to reduce blood loss and transfusion; however, the optimal route of administration remains uncertain, particularly when considering both clinical outcomes and cost. METHODS: A Bayesian network meta-analysis of randomized controlled trials was performed comparing intravenous (IV), topical, oral, combined, and no TXA in TKA. The primary outcome was total blood loss. The secondary outcomes included hemoglobin (Hb) decrease, allogeneic transfusion, and venous thromboembolism (VTE). There were 40 randomized controlle

Transnasal spread of bupivacaine into the pterygopalatine fossa following endoscopically assisted cotton swab placement: a cadaveric study.

BACKGROUND: There are conflicting data on the efficacy of transnasal topical anesthetic approaches intended to achieve a pterygopalatine ganglion block, specifically regarding the extent to which local anesthetics reach the pterygopalatine fossa. This cadaveric study aims to determine whether bupivacaine can reach the pterygopalatine fossa following topical administration near the sphenopalatine foramen using endoscopically assisted cotton ball placement. METHODS: Nine fresh cadavers underwent topical nasal administration of a solution containing bupivacaine, methylene blue, and iodine contra

Instant-gelling electrospun polyvinyl alcohol nanofiber mats for enhanced retinal drug delivery via topical ocular application.

Topical application is the preferred route for drugs to treat ocular diseases, but most current eye drop preparations deliver active ingredients only transiently to surface structures. To enhance ocular drug delivery following topical application, we developed a preparation utilizing the unique property of polyvinyl alcohol (PVA) nanofibers to gel rapidly upon contact with minimal tear fluid on the ocular surface, thereby enhancing precorneal residence time. Coumarin 6 (C6), a poorly water-soluble fluorescent compound, was used as a tracer to evaluate the intraocular drug delivery by several P

Nebulized Lidocaine Through a Channeled Endoscope for Office Laryngology Procedures.

OBJECTIVE: This study sought to compare topical lidocaine delivered through a channeled endoscope versus traditional anesthetic methods for office laryngology procedures. METHODS: A single-institution prospective, crossover study of adult patients undergoing office laryngology procedures from February 2023 to February 2024 was conducted. Patient-reported outcomes for procedural tolerance and anesthetic preference were compared between the investigational method of channel-nebulized lidocaine versus control anesthetic delivery (trans-tracheal or drip catheter), with each patient serving as his

Acne in transgender patients: disease management and treatment recommendations from a group of experts on acne.

In transmen (women to men), the testosterone (T) therapy used to support the masculinizing process may cause acne. Conversely, in transwomen (men to women), feminising hormones may reduce acne but cause skin dryness and irritation which may be further exacerbated if mild acne persists, requiring topical treatments. Gonadotropin-releasing hormone analogues suppress endogenous sex hormones, thus avoiding the onset of acne in prepubertal individuals. To identify the specific issues of acne in transgender patients and propose suitable medical care for dermatologists. A group of five experts on acn

Neurosensory abnormalities and stability of a mouse model of dry eye disease.

This study aimed to use a mouse model of dry eye disease (DED) induced by topical administration of benzalkonium chloride (BAK) and assess its stability and the presence of neurosensory abnormalities, including ocular pain. Eight-week-old C57BL6/6 N male mice were used in this study. Mice were treated with 10 μL of 0.2% BAK dissolved in artificial tears (AT), administered twice daily for 7 days. After one week, animals were randomized into two groups: one was administered with 0.2% BAK in AT once per day for 7 days, while the other was not further treated. Corneal epitheliopathy was quantified

Effect of topical berberine in murine cutaneous leishmaniasis lesions.

OBJECTIVES: More effective topical treatments remain an unmet need for the localized forms of cutaneous leishmaniasis (CL). The aim of this study was to evaluate the efficacy and safety of a topical berberine cream in BALB/c mice infected with Leishmania major parasites. METHODS: A cream containing 0.5% berberine-β-glycerophosphate salt and 2.5% menthol was prepared. Its physicochemical and stability properties were determined. The cream was evaluated for its capacity to reduce lesion size and parasitic load as well as to promote wound healing after twice-a-day administration for 35 days. Cli

The efficacy of nitroglycerin to prevent radial artery spasm and occlusion during and after transradial catheterization: A systematic review and meta-analysis of randomized controlled trials.

Radial artery spasm (RAS) is the most common cause of transradial access site crossover and is a common intra-procedural complication. RAS incidence can lead to radial artery occlusion (RAO) postprocedure, preventing the radial artery as a future access site. We evaluated the efficacy of nitroglycerin preventing RAS and RAO during transradial catheterization discussing the different routes of administration, including topical, subcutaneous, and intra-arterial. A systematic review and meta-analysis included all relevant articles until April 23, 2022. We searched six databases Google Scholar, We

Assessment of In-Situ Gelling Microemulsion Systems upon Temperature and Dilution Condition for Corneal Delivery of Bevacizumab.

Bevacizumab (BVZ), a recombinant humanized monoclonal antibody, has recently been proposed as a topical application in the treatment of anterior segment neovascularization; however, as there are some disadvantages in the administration of common eye-drops, ophthalmic topical drug delivery systems are under study to improve the precorneal residence time, reducing the frequency of administration. In this work, oil-in-water and water-in-oil BVZ-loaded microemulsions are developed, able to increase their viscosity, either by the formation of a liquid-crystalline structure upon aqueous dilution, th

Design of a Topically Applied Gel Spray Formulation with Ivermectin Using a Novel Low Molecular Weight Gelling Agent, Palmitoyl-Glycine-Histidine, to Treat Scabies.

Palmitoyl-glycine-histidine (Pal-GH) is a new low molecular weight gelling agent. It exhibits thixotropic behavior, low viscosity, and high dissolving properties for a wide range of hydrophilic to lipophilic drugs. Orally administered ivermectin (IVM) is used to treat scabies. However, this treatment is associated with well-known side effects, thus a study is awaited to search for alternative routes of administration. Although a topical formulation of IVM could be a candidate, it requires whole body application except the head and face for several hours on a daily basis. Therefore, in this stu

Participation of nitric oxide in the mustard oil-induced neurogenic inflammation of the rat paw skin.

The possibility of nitric oxide (NO) participating in the neurogenic inflammatory reaction of the rat hindpaw skin to topical application of mustard oil was examined in anesthetized rats. Vasodilation was measured by contactless infrared emission thermography, plasma protein exudation was measured by the Evans blue leakage technique, and formation of oedema was determined by measurement of the paw volume. Topical administration of mustard oil increased the temperature of the paw skin by 2-3 degrees C, a response that was accompanied by plasma protein extravasation and followed by development o

Topical application of low molecular weight heparin in a rabbit traumatic anastomosis model.

Low molecular weight heparins (LMWHs) are antithrombotic drugs composed of lower molecular weight components of heparin with an apparent molecular weight in the range of 4.0-8.0 KDA. These agents have been used clinically for several years. They have different mechanisms of action compared to heparin, a longer half-life and much higher bioavailability. Anticoagulant drugs such as heparin have been used topically in our previous studies to avoid bleeding complications observed with systemic administration. In this study, low molecular weight heparin (Certoparin, Sandoz) was topically administer

Topical Spironolactone in the Treatment of Evaporative Dry Eye Disease.

Meibomian gland dysfunction (MGD) is associated with evaporative dry eye syndrome, which is characterized by a reduction in meibum secretion and tear film instability. Present treatments provide only temporary relief, thereby necessitating the exploration of novel therapeutic strategies for chronic treatment. This study aims to evaluate topical spironolactone, a medication with anti-mineralocorticoid, anti-androgenic, and anti-inflammatory properties, in treating dry eye. A retrospective observational study was performed on the medical records of 102 patients diagnosed with dry eye disease. Th

Gene therapy for psoriasis in the K14-VEGF transgenic mouse model by topical transdermal delivery of interleukin-4 using ultradeformable cationic liposome.

BACKGROUND: Topical transdermal gene delivery to the skin shows great potential for painless, non-invasive administration of vaccines and therapeutic agents. Interleukin (IL)-4 strategies have shown a good antipsoriatic effect in clinic trials. To date, no information has been acquired on the effectiveness of gene therapy for psoriasis in the K14-VEGF transgenic mouse model by topical transdermal penetration of murine IL-4 (mIL-4) using ultradeformable cationic liposome (UCL). METHODS: In the present study, we synthesized an UCL and determined a suitable formula for transdermally delivering p
