Relationship between subtypes of irritable bowel syndrome and severity of symptoms associated with panic disorder.

OBJECTIVE: To investigate the relationship between subtypes of irritable bowel syndrome (IBS) and severity of symptoms associated with panic disorder (PD). MATERIAL AND METHODS: The study comprised 178 consecutive new PD outpatients. Sixty-four patients met the Rome-II criteria for IBS (IBS[+]; 29 diarrhea-predominant IBS (IBSD), 14 constipation-predominant IBS (IBSC), 21 other types of IBS). RESULTS: IBSD patients with agoraphobia avoided a greater number of scenes owing to fear of panic attack than did PD patients without IBS (IBS[-]) and with agoraphobia. IBS[+] patients with avoidant beh…

Panic disorder and perceived parental rearing behavior investigated by the Japanese version of the EMBU scale.

Although recent studies have found dysfunctional parental rearing behaviour is associated with certain aspects of psychopathology of panic disorder (PD), the results are not in complete agreement. By using a translated Japanese version of the EMBU (Egna Minnen Beträffande Uppfostran), we investigated the parental rearing behavior perceived by 103 normal subjects, 71 PD patients with agoraphobia, and 32 PD patients without agoraphobia. The PD patients scored both parents as more rejecting and overprotective than did the controls. However, subgroup analysis showed that the patients with agorapho…

Perceived parental characteristics and reports of early parental deprivation in agoraphobic patients.

It is commonly believed that patients with agoraphobia have suffered more frequently from aberrant family experiences in childhood. Some authors have described an excess of parental deprivation and others have incriminated parental overprotectiveness. Recent studies have failed to confirm these impressions but, instead, find that agoraphobic patients recall deficits in parental warmth and care during their early years. This report of a Sydney case-control study adds support to the view that parental overprotectiveness in isolation is unlikely to place the child at greater risk of developing ag…

[The role of psychotherapy in managing panic disorder].

This general presentation reviews briefly the various psychotherapies proposed for panic attacks and agoraphobia treatment. Cognitive-behaviour therapy (CBT) is the only validated therapy. Behavioural model of agoraphobia and cognitive model of panic attacks are detailed. The main techniques are described. The results of controlled studies and meta-analyses demonstrate that CBT represents an alternative to anxiolytics and antidepressants. CBT is a brief form of therapy (10-20 sessions) which has been compared with waiting list, placebo, relaxation, antidepressants, anxiolytics or supportive th…

Solving panic disorder problems. When your patients' fears thwart their lives.

All in all, panic disorder with or without agoraphobia is a very satisfying illness to treat. Most patients enter treatment with considerable pain and disability and within a few weeks to months are free of panic attacks, no longer fearful of them, and no longer phobic. Panic disorder must be distinguished from generalized anxiety, and signs of agoraphobia must be sought during the initial diagnosis. Drug therapy is effective for panic disorder, while agoraphobia may respond to drug therapy or to a range of cognitive therapies.

Briquet's syndrome in association with depression and panic: a reconceptualization of Briquet's syndrome.

In a sample of 188 consecutive female psychiatric patients, Briquet's syndrome was found to be associated more closely with the aggregation of both major depression and either panic disorder or agoraphobia than with major depression or panic disorder-agoraphobia alone. These results are consistent with the hypothesis of a shared diathesis underlying some cases of major depression, panic disorder, agoraphobia, and Briquet's syndrome. The author proposes that Briquet's syndrome may represent the most extreme expression of a tendency for a number of physical and psychological syndromes to aggrega…

Is the course of panic disorder the same in women and men?

OBJECTIVE: Panic disorder with or without agoraphobia has a chronic relapsing course. Factors associated with poor outcome include early onset of illness and phobic avoidance. Several, but not all, authors have found a worse clinical course for women. Using observational, longitudinal data from the Harvard/Brown Anxiety Disorders Research Program, the authors analyzed remission and symptom recurrence rates in panic patients with respect to sex. METHOD: Male and female patients (N = 412) in an episode of panic with or without agoraphobia were assessed by structured interview and prospectively …

The symptom structure of panic disorder: a trial using factor and cluster analysis.

Using cluster analysis of 207 patients with panic disorder (PD), we investigated the relationships between several panic symptoms at the time of panic attacks, which included anticipatory anxiety, agoraphobia, and 13 clinical symptoms based on the Diagnostic and Statistics Manual-III-Revised. Cluster analysis revealed three panic symptom clusters: cluster A (dyspnea, choking, sweating, nausea, flushes/chills); cluster B (dizziness, palpitations, trembling or shaking, depersonalization, agoraphobia, and anticipatory anxiety); and cluster C (fear of dying, fear of going crazy, paresthesias, and …

Behavioural and combined therapy in panic states.

This review will focus only on those paradigms for treatment of panic states with agoraphobia. Until recently the behaviour therapy literature has completely ignored panic disorder and has focused exclusively on the agoraphobic aspect of this syndrome. To summarize the behavioural therapy of panic disorder and agoraphobia, it appears that exposure is in the short run the most effective behavioural paradigm in agoraphobia. However, when contrasted with cognitive approaches it does not appear to be as effective in the treatment of panic disorder. In conclusion, it is unclear whether we can speak…

Anxiety disorders and figural fluency: A measure of executive function.

BACKGROUND: Anxiety possibly interferes with executive functioning, although most studies rely on anxiety symptoms or lack control for comorbid depression. The objective of the present study is to examine the association between executive functioning and (individual) anxiety disorders with ak,ld without controlling for depression. METHOD: Generalized anxiety disorder (GAD), panic disorder with and without agoraphobia, agoraphobia, social phobia, as well as depressive disorder according to DSM-IV criteria were assessed with the Mini International Neuropsychiatric Interview in 82,360 community-…

Differential drug response of panic and agoraphobic avoidance in a case of panic disorder.

A case is presented of a patient with severe panic disorder and agoraphobia in whom initial treatment with clomipramine resulted in complete elimination of panic attacks, with no improvement in agoraphobic avoidance. The addition of phenelzine to the pre-existing clomipramine treatment resulted in rapid and complete disappearance of the agoraphobic avoidance. The possible implications of this case for our understanding of the neurobiological relationship between panic attacks and agoraphobia are discussed.

Some biological aspects of panic disorder.

A review is made of the aetiology of panic disorders and of their relationship with agoraphobia. The paper confines itself to the review of evidence of the lactate induction of panic attacks and the response of panic disorder and agoraphobia to some antidepressants. It does not deal with genetic evidence or the dexamethasone suppression test. It is concluded that biological disturbance is probably a major causal factor of panic disorder, producing a physiological response but associated with some other factor.

The structure of phobias in panic disorder.

Fear and avoidance of individual phobic situations were measured in 1168 patients with panic disorder suffering current attacks. Correlation and principal-components analyses give components of agoraphobia, illness phobia, and social phobia in panic disorder. Agoraphobia does not stand out so clearly as illness phobia and social phobia as a separate factor; 'fear of open spaces' shows some separation from other agoraphobic situations. Frequency of panic attacks and changes associated with their remission have no special relationship to agoraphobia. Closer attention to social and illness phobia…

Childhood separation anxiety and panic disorder: a comparative study.

1. The relationship between childhood separation anxiety and panic disorder in adults is analyzed using data from a multicenter trial in 107 patients. 2. The patients included in this study presented anxiety disorder with or without agoraphobia, diagnosed according to DSM-III criteria. 3. The percentage of patients with antecedents of separation anxiety was 17.8% in patients without agoraphobia and 21.7% in patients with agoraphobia. These rates are significantly higher than those encountered in a group of normal controls (4%). 4. The existence of separation anxiety in childhood does not seem …

The epidemiology of anxiety.

In a literature review the best estimates for prevalence of anxiety disorders were determined to be 3% for panic, 6% for agoraphobia, 3% for generalized anxiety, 2.5% for simple phobia, and 1.5% for social phobia. Anxiety disorders tended to have a 2:1 female preponderance. Age of onset was 17 years for social and simple phobia, 22 years for generalized anxiety, and 26 years for panic and agoraphobia. Simple phobia was found to respond best to behavioral treatment and to have a good prognosis. Panic and agoraphobia were found to be familial, to have a chronic course, and to respond best to a c…

Alprazolam in the treatment of panic attack patients with and without major depression.

Sixteen patients with panic attacks were treated with alprazolam at an anxiety clinic between March 1982 and April 1983. For all patients charts were reviewed for baseline data and treatment results at 1 and 6 months. Quantitated self-rating scales and the Clinical Global Impressions scale were used to assess progress. Alprazolam appeared effective for panic, agoraphobia, and depressive symptoms in 7 of 11 patients with either panic disorder or agoraphobia with panic attacks (DSM-III-defined diagnoses); side effects occurred in 4 of the 11 patients, were limited to oversedation, and resulted i…

Relationship between balance system function and agoraphobic avoidance.

There is a striking similarity between the situations avoided by people with agoraphobia and the environments which provoke disorientation in people with organic balance disorders. This study investigated the possibility that agoraphobia might be linked to balance system dysfunction by comparing the results of balance system tests in 36 people with symptoms of panic and agoraphobia and 20 normal controls. A traditional battery of audiovestibular tests was supplemented with moving platform posturography, which assesses the postural instability induced by disorienting perceptual conditions. Subj…
