Late-Stage Diagnosis of Biliary Atresia in Rural America.

Biliary atresia (BA) is a rare neonatal disease characterized by progressive obstruction of the extrahepatic biliary system, with an etiology that remains poorly understood. This case describes the presentation of BA in a female neonate and underscores the consequences of delayed communication in a rural healthcare setting. The patient was born at a rural hospital, where laboratory tests were obtained but not processed and communicated to her pediatrician in a timely manner. As a result, her first notable clinical finding occurred at the two-week wellness visit, when mild hepatomegaly was susp

Implementing a nicotine cap to make America Healthy Again.

The US Department of Health and Human Services' (HHS) "Make America Healthy Again" initiative cannot eradicate the root causes of the nation's chronic disease epidemic without tackling cigarette smoking, which kills over 480 000 Americans and costs $600 billion annually. This commentary reinvigorates a pivotal 2017 FDA framework by urging the current administration to finalize the proposed rule establishing a maximum nicotine cap in all cigarettes and selected combusted tobacco. Evidence demonstrates that capping nicotine at nonaddictive levels (∼95% reduction) rapidly decreases dependence and

Clinical practice guideline for psoriasis management in Latin America.

BACKGROUND: Psoriasis care in Latin America is heterogeneous, and access to systemic agents, biologics, phototherapy, and monitoring resources varies widely across countries. OBJECTIVE: To develop a regional, evidence-based clinical practice guideline for psoriasis management using GRADE methodology. METHODS: The Latin American Psoriasis Society (SOLAPSO) convened a guideline development group, an extended expert panel, patients, and an independent methodological team. We framed clinical questions using PICO, conducted systematic literature reviews, rated certainty of evidence with GRADE, an

"Hepatocellular carcinoma surveillance: Current challenges in Latin America".

Hepatocellular carcinoma (HCC) is the most common primary liver cancer, predominantly linked with cirrhosis and chronic hepatitis B. In Latin America, disparities in healthcare access complicate HCC management. A recent expert meeting emphasized the importance of identifying at-risk populations for effective screening and surveillance, underlining the need for structured routine programs. Early detection of HCC improves outcomes and increases survival rates. Surveillance programs are essential, yet access to healthcare and treatment varies significantly across Latin America, making timely diag

Strategies for harm reduction in Latin America: the example of tobacco.

Tobacco consumption continues to impose a profound public health and economic burden across Latin America, disproportionately affecting men, adolescents, and low-income populations. Despite progress in some countries through implementation of the World Health Organization (WHO) Framework Convention on Tobacco Control (FCTC), significant gaps remain due to weak regulatory frameworks, limited enforcement capacity, and persistent interference from the tobacco industry. Against this backdrop, tobacco harm reduction (THR), the substitution of combustible products with non-combustible or lower-expos

Vaginal Microbiota and HPV in Latin America: A Narrative Review.

With the expansion of human microbiome studies in the last 15 years, we have realized the immense implications of microbes in human health. The human holobiont is now accepted, given the commensal relationships with bacteria, fungi, parasites, viruses, and human cells. The cervicovaginal microbiota is a specific case within the human microbiome where diversity is lower to maintain a chemical barrier of protection against infections. This narrative review focuses on the vaginal microbiome. It summarizes key findings on how native bacteria protect women from disease or predispose them to damagin

The 5T's of health disparities in multiple myeloma in Latin America.

PURPOSE OF REVIEW: Health disparities or inequities, which are defined as differences in the quality of medical and healthcare between populations among racial, ethnic, and socioeconomic groups, have been validated in numerous studies as determinants of survival and quality of life in different diseases, including cancer.Compared to the improvement in overall survival in developed countries in relation to better diagnostic opportunity and novel therapeutic approaches, low and middle-income countries still have significant barriers in accessing these therapies. The potential impact of overcoming

Infarctive encephalopathy in zoo-housed gibbons in North America.

Progressive neurologic signs without a known underlying etiology have been observed in managed gibbon populations housed at institutions in North America. In 2018, the Gibbon Species Survival Plan initiated a veterinary survey to evaluate clinical histories among gibbons displaying neurologic signs. The clinical results of this survey as well as the results of a centralized histologic review of brain samples from 5 species of managed gibbons displaying neurologic signs are outlined here. Reported neurologic deficits in gibbons surveyed included tremors (12/12, 100%), ataxia/incoordination (11/

Size Matters? Penis Dissatisfaction and Gun Ownership in America.

In this study, we formally examine the association between penis size dissatisfaction and gun ownership in America. The primary hypothesis, derived from the psychosexual theory of gun ownership, asserts that men who are more dissatisfied with the size of their penises will be more likely to personally own guns. To test this hypothesis, we used data collected from the 2023 Masculinity, Sexual Health, and Politics (MSHAP) survey, a national probability sample of 1,840 men, and regression analyses to model personal gun ownership as a function of penis size dissatisfaction, experiences with penis

Society for Healthcare Epidemiology of America Compendium updates 2022.

PURPOSE OF REVIEW: Healthcare-associated infections (HAIs) are a leading cause of preventable harm in US hospitals. Hospitals are required to conduct surveillance and report selected HAIs, including central line-associated bloodstream infections, catheter-associated urinary tract infections, colon and abdominal hysterectomy surgical-site infections, methicillin-resistant Staphylococcus aureus bacteremia, and Clostridioides difficile infections, to the CDC's National Healthcare Safety Network. RECENT FINDINGS: Up until the COVID-19 pandemic, there was significant progress in reducing HAIs. How

Survey on initial management of acute pancreatitis in Latin America.

OBJECTIVE: The population of Latin America harbors the highest incidence of gallstones and acute biliary pancreatitis, yet little is known about the initial management of acute pancreatitis in this large geographic region. PARTICIPANTS AND METHODS: We performed a post hoc analysis of responses from physicians based in Latin America to the international multidisciplinary survey on the initial management of acute pancreatitis. The questionnaire asked about management of patients during the first 72h after admission, related to fluid therapy, prescription of prophylactic antibiotics, feeding and

Liver resection for hepatolithiasis: A multicenter experience in Latin America.

BACKGROUND: Hepatolithiasis is a prevalent disease in Asia but rare in Western countries. An increasing number of cases have been reported in Latin America. Liver resection has been proposed as a definitive treatment for complete stone clearance. The aim of this study was to evaluate the postoperative outcomes of liver resection for the treatment of hepatolithiasis in 2 large hepatobiliary reference centers from South America. METHODS: We conducted a retrospective descriptive analysis from patients with hepatolithiasis who underwent liver resection between November 1986 and December 2018, in

Equity and Prevention of Cardiovascular Diseases in Latin America and the Caribbean.

Non-communicable diseases, particularly cardiovascular diseases, are the leading cause of decreased life expectancy and death in Latin America and the Caribbean. Although a lifestyle, which includes no tobacco use, good nutrition, and regular physical activity is touted as key to health, the environmental, racial, social and economic conditions, which underpin lifestyle are often ignored or considered only secondarily. Placing the main responsibility on a patient to change their lifestyle or to simply comply with pharmacological treatment ignores the specific conditions in which the individual

Femitags for feminist connected crowds in Latin America and Spain.

In recent years, women are acting all over the world against gender violence and femicide. This new wave of feminist claims is characterized by the intensive use of social media to spread consciousness and amplify influence. For this research, we analyse three femitags (i.e., feminist hashtags) from Twitter that have been relevant in different crucial mobilizations in Argentina, Spain, and Mexico. These are three hashtags with different functions for activism that have shown special relevance due to their continuity or their intensity in the Spanish-speaking area between 2015 and 2020 (before

Health Equity in a Post 'Roe Versus Wade' America.

For almost five decades, the US Supreme Court had protected the right to abortion care in the United States. However, the Court's decision in 'Dobbs versus Jackson Women's Health Organization' in 2022 established that the US Constitution does not confer a right to abortion, effectively overturning the Court's own previous judgment in 'Roe versus Wade' from 1973. How could a decision that overturns the precedent for nationwide access to abortion affect women, children, and physicians? How will state laws impact healthcare equity regarding reproductive rights going forward? How will geography af
