A spinal cord infarction that occurred after laparoscopic gastrectomy performed under general anesthesia and epidural analgesia.

BACKGROUND: Spinal cord infarction (SCI) after epidural anesthesia is quite rare. Although most cases of perioperative SCI are associated with aortic, cardiac, or spinal surgery, and/or abnormal preoperative conditions, such as spinal stenosis or hypercoagulopathy, intraoperative events, such as severe hypotension or epidural puncture and catheterization, can be contributory factors. CASE PRESENTATION: A 52-year-old male was underwent laparoscopic gastrectomy. Before induction of general anesthesia, an epidural catheter was placed without any problems. The patient had no pain and no complaint

Transient left ventricular dysfunction due to coronary spasm after spinal anesthesia with bupivacaine - a case report.

Bupivacaine is a long-acting local anesthetic (LA) used for cutaneous infiltration, peripheral nerve blocks, epidural and spinal anesthesia. However, its application may result in cardiovascular complications such as: hypotension, bradycardia, cardiac arrest and toxic myocardial injury. The authors describe a 53-year-old male with a history of cigarette smoking, admitted for an elective inguinal hernia surgery. Before surgery, the patient received subarachnoid injection of bupivacaine (20 mg). After the operation, he developed transient hypotension. Blood pressure returned to normal after gelo

Evaluation of antihypotensive techniques for cesarean section under spinal anesthesia: Rapid crystalloid hydration versus intravenous ephedrine.

BACKGROUND: Spinal anesthesia is a preferred technique over general anesthesia for cesarean delivery. It avoids maternal airway related complications, aspiration and neonatal depression. However hypotension following spinal anesthesia can lead to decrease in uterine blood flow and neonatal hypoxia. AIMS: We aimed to evaluate the efficacy of 15 mL.kg- 1of crystalloid preloading versus prophylactic intravenous bolus of 10 mg ephedrine as an antihypotensive measure for cesarean section. METHODS: A prospective randomized double blind study was conducted in hundred ASA grade I/II parturient under

Repair of duodenal atresia under spinal anesthesia in a low-birth-weight preterm neonate: case report.

Duodenal atresia is a well-recognized cause of neonatal bowel obstruction. General anesthesia with tracheal intubation is the traditional anesthetic technique for surgical correction of this condition. Metabolic abnormalities and fluid deficits coupled with residual anesthetics are known to increase the risk of postoperative apnea, prolonging the operating room time and delaying extubation. Spinal anesthesia (SA) is an accepted alternative to general anesthesia in formerly preterm infants. In the current literature, there are reports of successful use of SA for simple infraumbilical surgery an

Evaluation of reliability of post-tetanic motor-evoked potential monitoring during spinal surgery under general anesthesia.

STUDY DESIGN: A prospective research. OBJECTIVE: Compare the reliability of post-tetanic motor-evoked potential (p-MEP) monitoring in the detection of motor injury during spinal surgery with that of conventional MEP (c-MEP). SUMMARY OF BACKGROUND DATA: Myogenic MEPs are sensitive to suppression by anesthetics and neuromuscular blockade. Recently, we reported a new technique for MEP recording, called "p-MEP" in which MEP amplitude can be enlarged by tetanic stimulation of peripheral nerve before transcranial stimulation in comparison with that of c-MEP. The purpose of this study is to compare

Delayed hypoxemia after bone cement insertion during total hip replacement under spinal anesthesia--a case report.

We report a case of delayed hypoxemia in an aged healthy male patient, which developed 2 hours after cementation of the prosthesis in total hip replacement (THR) under spinal anesthesia. The patient was doing well throughout the operation but unfortunately, progressive tachypnea was noted 1 h after he was transferred to the recovery room (i.e. 2 h after the application of bone cement into the femur). An hour further, distinct wheeze was heard bilaterally on auscultation, which signified bronchospasm. Arterial blood gases analysis revealed a low PaO2 of 71 mmHg and a decrease of oxygen saturati

Prevention of hypotension after spinal anesthesia for cesarean section: six percent hetastarch versus lactated Ringer's solution.

This study was designed to determine whether preoperative administration of 6% hetastarch decreases the incidence and severity of hypotension after spinal anesthesia for cesarean section. Forty nonlaboring ASA class I and II women having nonurgent cesarean sections were randomized to receive either 500 mL of 6% hetastarch plus 1 L lactated Ringer's solution (LR) (n = 20), or 2 L of LR (n = 20) prior to induction of spinal anesthesia. Hypotension occurred in 45% of patients who received hetastarch vs 85% of those who received only LR (P < 0.05), and minimum systolic blood pressure was lower in

Different local anesthetic volumes for unilateral spinal anesthesia in varicose vein surgery prospective randomized clinical trial.

BACKGROUND: Varicose vein surgery is one of the most frequent indications for ambulatory vascular interventions. Although spinal anesthesia provides reliable surgical conditions, conventional intrathecal doses may delay mobilization and discharge due to prolonged block duration and hemodynamic instability. Unilateral spinal anesthesia with reduced local anesthetic doses has been proposed to facilitate early recovery. This study aimed to compare the perioperative block characteristics and recovery profiles of two different intrathecal bupivacaine doses in patients undergoing ambulatory varicose

Serious adverse effect of low-dose dexmedetomidine under spinal anesthesia: A case report and literature review.

BACKGROUND: Dexmedetomidine is a highly selective α2-adrenergic agonist with sedo-analgesic properties. It has been approved by the United States Food and Drug Administration (FDA) for sedation of critically ill patients, facilitation of awake procedures, and management of patient agitation. However, its use has been occasionally associated with adverse effects like bradycardia, hypotension, and asystole. The risk of these effects, particularly cardiac arrest, is increased in the elderly, patients with multiple comorbidities, and administration of higher doses of the drug (especially loading d

Can subclavian/infraclavicular axillary vein collapsibility index predict spinal anesthesia-induced hypotension in cesarean-section operations?

OBJECTIVE: Spinal anesthesia-induced hypotension (SAIH) is relatively common in pregnant women and has serious maternal and fetal side effects. In patients who are hypovolemic during spinal anesthesia, there may be a significant decrease in blood pressure caused by the decrease in preload. Subclavian vein sonography is a useful method for evaluating preoperative intravascular volume status. This study aimed to evaluate the efficacy of the pre-operative subclavian vein or infraclavicular axillary vein (SCV-AV) collapsibility index for predicting SAIH in cesarean-section (C-section). PATIENTS A
