[Renovascular hypertension: diagnostic and therapeutic strategy].

Investigations for renal artery stenosis have been greatly facilitated by advances in imaging techniques. Intravenous digital subtraction angiography is now performed in all patients with progressive, drug-resistant hypertension associated with aorto-iliac lesions or with renal impairment induced by angiotensin-converting enzyme inhibitors. Yet the finding of hypertension with renal artery stenosis is not enough to make the diagnosis of renovascular hypertension, this term being reserved to hypertension reversible by revascularization. The selection of patients who may benefit from revasculari

When Is Diagnostic Subtraction Angiography Indicated Before Clipping of Unruptured and Ruptured Intracranial Aneurysms? An International Survey of Current Practice.

INTRODUCTION: The goal of this survey is to investigate the indications for preoperative digital subtraction angiography (DSA) before clipping of ruptured and unruptured intracranial aneurysms in an international panel of neurovascular specialists. METHODS: An anonymous survey of 23 multiple-choice questions relating to indications for DSA before clipping of an intracranial aneurysm was distributed to the international panel of attendees of the European-Japanese Cerebrovascular Congress (EJCVC), which took place in Milan, Italy on 7-9 June 2018. The survey was collected during the same confer

Diagnosing Vasospasm After Subarachnoid Hemorrhage: CTA and CTP.

Cerebral vasospasm is a potentially devastating complication in patients with aneurysmal subarachnoid hemorrhage. The purpose of this article is to review the use of computed tomogram (CT) angiography and CT perfusion in the diagnosis of cerebral vasospasm after aneurysmal subarachnoid hemorrhage and also assess their use in guiding treatment decisions. Both techniques are widely used for other indications but their use in cerebral vasospasm has not been well defined. Computed tomogram angiography can directly visualize arterial narrowing and CT perfusion is able to evaluate differences in per

Endovascular coil embolization of unruptured posterior communicating artery aneurysm.

The patient is a 60-year-old woman who presented to her primary care physician with new onset of headache. She was neurologically intact without cranial nerve deficit. An outpatient CT angiogram (CTA) revealed no subarachnoid hemorrhage, but showed a right-sided posterior communicating artery aneurysm measuring 11 mm by 10 mm. Digitally subtracted cerebral angiography confirmed these measurements and showed that the aneurysm was amenable to endovascular coil embolization. The patient underwent aneurysm coiling without complication and was discharged to home on postoperative Day 1. The video ca

[Spinal dural arteriovenous fistulas : Diagnostics and therapy].

Spinal dural arteriovenous fistulas are rare spinal vascular malformations which can cause progressive paraparesis and paraplegia if not treated. As symptoms are unspecific diagnosis is often delayed and clinical outcome is dependent on early therapy. While magnetic resonance imaging (MRI) is the first choice imaging procedure, selective spinal digital subtraction angiography is necessary to analyze the angioarchitecture and to plan the treatment. This article provides an overview on the epidemiology, etiology, clinical aspects and imaging features as well as therapeutic aspects of spinal dura

Automatic anatomical labeling method of cerebral arteries in MR-angiography data set.

To improve the accuracy and robustness of 2D/3D registration of digital subtraction angiography images and magnetic resonance angiography (MRA) data, we have developed an automatic method for anatomical labeling of the cerebral arteries in MRA data. The anatomical labeling method is a location-based method which segments an artery tree to branches and classifies the branches into labeled segments, i.e., internal carotid arteries (ICA), basilar artery (BA), middle cerebral arteries (MCA), A1 segments of the anterior cerebral artery (ACA(A1)), other segments of the anterior cerebral artery (ACA)

Pedal artery imaging using DSA, CE-MRA and duplex.

Diabetic macroangiopathy is associated with a typical disease pattern, mostly affecting the tibial but sparing the pedal arteries. Bypass grafts to these patent pedal vessels have evolved into a feasible technique. Restoring a palpable pulse in the ischemic foot they create the prerequisite to heal an ulcer or gangrene. Preoperative evaluation of pedal arteries demands a detailed depiction of the entire pedal vascular system. At present, the main options for preoperative pedal artery imaging are selective DSA contrast enhanced MRA and Duplex ultrasound. The aim of this review article is to giv

4D Flow MRI Reflects Physiological Hemodynamics for the Diagnosis and Management of Portosystemic Shunts.

A woman in her sixties with portosystemic shunt and hepatic encephalopathy underwent open mesenteric vein ligation, resulting in improved portal flow and blood ammonia. In this case, 4D flow MRI was a valuable diagnostic and follow-up tool, visualizing and quantifying physiological portal hemodynamics with features distinct from those of contrast-enhanced CT and digital subtraction angiography. Our case study highlights the value of 4D flow MRI for managing portosystemic shunts.

[Imaging of actinomycosis: CT scan, bronchial embolization and pathology].

Pulmonary actinomycosis is a rare infectious disease that can be difficult to diagnose due to nonspecific imaging abnormalities and to a need for repeated lung sampling by CT-guided biopsy or bronchoscopy. It may present with hemoptysis, which can occur with or without antibiotic therapy and bronchial artery embolization may be required. We report here a case of pulmonary actinomycosis with imaging by thoracic CT, digital subtraction angiography, and pathological specimens.

A rare occurrence of persistent hypoglossal artery and its clinical significance.

Persistent hypoglossal artery (PHA) is an embryological vascular variation mostly originating from the internal carotid artery. The presence of PHA has been associated with the incidence of some diseases such as cerebral ischaemia, atherosclerosis, and aneurysm. Here, a very rare case of PHA that was discovered incidentally by digital subtraction angiography in Turkey is reported. Endovascular stenting was not performed for this patient. Also, its clinical importance is discussed.

Clinical features and management of traumatic ophthalmic artery aneurysm: a case report and literature review.

We report a 52-year-old man with progressive disturbance of visual acuity following a head injury. Magnetic resonance imaging (MRI) showed an aneurysm of the internal carotid artery. Digital subtraction angiography (DSA) 57 days after onset found that the aneurysm originated in the proximal ophthalmic artery. We treated the patient with an intravascular coil embolization, partially filling the aneurysm. Follow-up DSA indicated that the aneurysm did not recur.

Atypical Feeding and Drainage of Dural AV Fistula.

Dural arteriovenous fistulas (DAVF) are frequent causes of pulsatile tinnitus. Color Doppler sonography may play a useful, complementary role to CTA/MRA and digital subtraction angiography (DSA) in the assessment of these anomalies' characteristics, such as an ipsilateral increased flow volume and a low resistive index. In this article, we report a case of DAVF first detected with Color Doppler sonography that displayed an uncharacteristic venous drainage pattern.

Percutaneous retrieval of a radiolucent anchoring sleeve embolized in pulmonary artery during pacemaker implantation.

An 85-year-old female presented to our institution with symptomatic sick sinus syndrome. During pacemaker implantation, an anchoring sleeve in the right ventricular lead was embolized in the left pulmonary artery. Although the anchoring sleeve was radiolucent, digital subtraction angiography revealed an angiographic filling defect in the lower branch of the left pulmonary artery, and a snare catheter enabled the anchoring sleeve to be grasped and extracted.

Successful interventional management for subclavian artery injury secondary to internal jugular catheterization: a report of two cases.

Central venous cannulation through the right internal jugular vein is a widely used technique, and complications due to this procedure are very rare. We report two cases of right massive hemothorax due to inadvertent puncture of a right subclavian artery. Digital subtraction angiography demonstrated a perforated subclavian artery in both cases. These were successfully treated with transcatheter arterial embolization for the bleeding points of the right subclavian arteries.

[Renal arteriovenous fistula associated with fibromuscular dysplasia of the renal arteries].

Three to 5% of hypertensive diseases have a renovascular origin. Atherosclerosis and fibromuscular dysplasia are the two major causes of renovascular hypertension. Association of renal arteriovenous fistula and fibromuscular dysplasia is uncommon. The authors propose to illustrate the usefulness of Doppler ultrasonography and digital subtraction angiography in the diagnostic and therapeutic management of a renal arteriovenous fistula associated with fibromuscular dysplasia discovered after severe preeclampsia in a 30 year-old woman.
