Prophylaxis for pulmonary acid aspiration.

Pulmonary acid aspiration in the perioperative period was first noted as a medical concern more than 40 years ago. Despite this awareness, identification of the patient at risk is by no means certain, and the true incidence of this condition is not known. Various therapeutic interventions have been proposed and tried. Physical therapies, such as pressure on the cricoid, have been successful, as has the administration of clear antacids prior to surgery. The histamine (H2)-receptor antagonists cimetidine and ranitidine have also been used successfully as prophylactic therapy when enough time is …

Observations on management of choledochoduodenal fistula due to penetrating peptic ulcer.

Perforation into the biliary tract is a rare complication of benign peptic ulcer disease. This report describes our experience with 4 patien … disease or malignant ulcer. Medical management of the underlying ulcer disease with cimetidine and antacids usually suffices to relieve symptoms and to allow healing of the fistula. Surgery becomes necessary if biliary stenosis or cholangitis occurs, or if other aspects of the ulcer disease mandate operative treatment.

Surgical Management of Gastroesophageal Reflux Disease in Patients with Severe Esophageal Dysmotility.

BACKGROUND: Gastroesophageal reflux disease (GERD) and esophageal dysmotility are often disqualifying criteria for fundoplication due to dysphagia complications. A tailored partial fundoplication may improve GERD in patients with severe esophageal motility disorders. We evaluate this approach on GERD improvement in non-achalasia esophageal dysmotility patients. METHODS: A single-institution prospective database was reviewed (2007-2016), with inclusion criteria of GERD, previous diagnosis of non-achalasia esophageal motility disorder, and laparoscopic partial fundoplication. Diagnosis of previ

Efficacy of lansoprazole in the treatment of gastroesophageal reflux disease in children.

OBJECTIVES: To assess the efficacy of lansoprazole for the relief of symptoms due to gastroesophageal reflux disease (GERD) in children 1 to 11 years of age. In addition, the efficacy in healing of erosive esophagitis (EE) was determined in those children with EE who were enrolled in the study. METHODS: In this phase I/II, open-label, multicenter (11 sites) U.S. study, children with symptomatic GERD, EE by endoscopy, and/or intraesophageal pH < 4 for greater than 4.2% of the time based on 24-hour pH testing were assigned, on the basis of body weight, to lansoprazole 15 mg (< or = 30 kg) or 30

Randomised clinical trial: the use of alginates during preinvestigation proton pump inhibitor wash-out and their impact on compliance and symptom burden.

BACKGROUND/AIMS: Investigation of gastro-oesophageal reflux disease is usually performed off proton pump inhibitors (PPIs). This can exacerb … g. Preinvestigation instructions involved stopping PPIs and H2 receptor antagonists for 1 week, but antacids and alginates were allowed until the night before. Participants were randomised to follow these standard instructions (control group), or the same instructions with the provision of Gaviscon Advance to be taken four times daily (treatment group). Gastroesophageal reflux disease. Control of symptoms, prevention of complications.

GERD is a common disease that has a significant impact on patients' quality of life. Many surveys have shown that up to 60% of the population suffers from GERD at some time. Heartburn and acid regurgitation are the classic symptoms, but manifestations are variable. Of the several extraesophageal manifestations of GERD, reflux-induced asthma may be the most important. Complications of GERD include peptic strictures of the esophagus and Barrett's metaplasia. Clinical diagnosis of GERD can be confirmed by response to antisecretory therapy. A high-dose trial of a PPI is becoming an accepted diagno

[Pharmacokinetics of colloidal bismuth subcitrate. Solution versus tablet].

The aim of the present study was the investigation of the pharmacokinetics of bismuth after application of different preparations of colloid bismuth subcitrate (CBS; CAS 57644-54-9). 6 healthy volunteers were recommended to take a solution containing 240 mg CBS b.d. before breakfast and before the evening meal for 2 weeks, whereas 6 other volunteers received tablets containing 120 mg CBS 2 b.d. In both groups resulting daily CBS dose was 480 mg. On day 7 and day 14, 24 h urine excretion of bismuth was found to be significantly lower after application of the solution as compared to the one afte

Gastroesophageal reflux. Simple measures often suffice.

Regurgitation of the gastric contents into the esophagus is common and often unnoticed. When symptoms such as heartburn, a sour or bitter taste in the mouth, or even chest pain mimicking angina pectoris or myocardial ischemia prompt a patient to seek help, the factor or factors responsible for reflux must be sought. The possible underlying causes are numerous, as Dr Bachman points out in this discussion of the pathophysiology, diagnosis, and treatment of gastroesophageal reflux. The desired end point of management was well stated by Seneca over 2,000 years ago as "a good-humored stomach."

Therapeutic implications of duodeno-gastric reflux.

Both medical and surgical treatment of gastroduodenal reflux have many inherent limitations. Some available drugs act on gastroduodenal reflux by enhancing gastric emptying. Other agents bind bile acids intragastrically or protect gastric epithelial cells from the cytotoxic effect of duodenal juice. Bioavailability of these latter substances is not fully investigated. One has to consider that in general treatment of gastroduodenal reflux must be administered over a considerable length of time. Greater importance has, therefore, to be attached to drug safety than to any impressive short-term ph

Functional dyspepsia: from human to dog, a retrospective study of 29 cases illustrating a complex entity.

BACKGROUND: Functional gastrointestinal disorders, including mostly functional dyspepsia (FD) and irritable bowel syndrome, are highly preva … so at 1.5 years. Various treatments were attempted with varying success, including dietary changes, antacids, prokinetics, and steroids. Follow-up data were available for 21 dogs, of which 76% demonstrated clinical improvement over a median follow-up period of 12 months. Interestingly, anxiety disorder of the owner was commonly reported. CONCLUSIONS: Func …

Pharmacokinetic-pharmacodynamic drug interactions with nonsteroidal anti-inflammatory drugs.

The nonsteroidal anti-inflammatory drugs (NSAIDs) are very commonly prescribed, especially in the elderly population. In many countries more than 10 different NSAIDs are available. As the older pyrazole compounds like phenylbutazone, oxyphenbutazone and azapropazone are most prone to pharmacokinetic interactions, the use of these compounds should be avoided where possible. Acidic NSAIDs interact with bile acid-binding resins, resulting in decreased concentrations of NSAIDs in the blood. In earlier reports it was suggested that the absorption of NSAIDs was affected by antacids and sucralfate. M…

[Research progress of epigenetic regulation in the pathogenesis of aluminum exposure].

Aluminum is one of the most abundant elements on earth. Aluminum compounds are widely used in food additives, antacids, cooking utensils and so on. Human exposure to aluminum is mainly through diet and drinking water, while excessive intake of aluminum can accumulate in tissues and cause toxic reactions. In the central nervous system, aluminum exposure is closely related to a series of nervous system diseases such as Alzheimer's disease, Parkinson's disease and amyotrophic lateral sclerosis. Epigenetic modification refers to the regulation of gene expression without changing the DNA sequence, …
