The first 4 symptoms of binocular confusion concern primarily daytime vision and the fifth symptom concerns nighttime vision. Even with a conventional contact lens correction, symptoms of binocular confusion occur, especially with high optical aniseikonia for near or far distance. In addition, diplopia occurs in the presence of strabismus. The important result of our research is the fact that unilateral aphakes are severely disturbed by the 5 symptoms of binocular confusion described above. An attempt is made to depolarize current positions regarding intraocular implant lens surgery by presenting a reasonable list of advantages and disadvantages of the procedure. A current position on this controversial subject is presented and is based on currently available knowledge. Although this is a personal view, it is hoped that a middle ground can be created which would be acceptable to advocates and opponents of lens implant surgery.

Pars plana vitrectomy has provided an approach to those situations in which treatment of a retinal detachment becomes impossible due to vitreous pathology. The records of the first 207 pars plana vitrectomies performed by one of us (M.S.) were reviewed and attention was focused on the combined vitrectomy-scleral buckle procedures, particularly with regard to anatomical reattachment and visual improvement. PURPOSE: To report a sutureless method of amniotic membrane fixation using fibrin glue in two cases of symptomatic bullous keratopathy with poor visual potential.
METHODS: Under topical anesthesia, the loose epithelium was debrided up to 1.0 mm from the limbus and fibrin glue was applied uniformly and thinly onto the corneal surface. Cryopreserved amniotic membrane was evenly placed over the cornea and once a firm and uniform adhesion was achieved, excess membrane was trimmed off. A bandage contact lens was applied.

Residual vitreous base after vitreoretinal surgery was evaluated by ultrasound biomicroscopy (UBM).
