Potocki-Shaffer syndrome in a child without intellectual disability-The role of PHF21A in cognitive function.

Potocki-Shaffer syndrome is a contiguous gene deletion syndrome involving 11p11.2p12 and characterized by multiple exostoses, biparietal foramina, genitourinary anomalies in males, central nervous system abnormalities, intellectual disability, and craniofacial abnormalities. Current literature implicates haploinsufficiency of three genes (ALX4, EXT2, and PHF21A) in causing some of the cardinal features of PSS. We report a patient with multiple exostoses, biparietal foramina, and history of mild developmental delay. Cognitive and behavioral testing supported formal diagnoses of anxiety, verbal

[Worster-Drought syndrome--a specific cerebral palsy syndrome--why is the diagnosis frequently overlooked?].

Worster-Drought syndrome (WDS) is a developmental disorder presenting as mild tetraplegic cerebral palsy (CP) with severe pseudobulbar palsy, manifested as articulation problems or anarthria, chewing, and swallowing difficulties and severe drooling. Epilepsy, learning disabilities, behavioral disturbances, and other congenital anomalies are common. WDS accounts for about 1% of CP cases. On brain imaging, migrational abnormalities are frequently found, mainly in the opercular area, however normal brain imaging does not exclude the diagnosis. Although children with WDS have serious, early-onset

Advances in EPG for treatment and research: an illustrative case study.

Electropalatography (EPG), a technique which reveals tongue-palate contact patterns over time, is a highly effective tool for speech research. We report here on recent developments by Articulate Instruments Ltd. These include hardware for Windows-based computers, backwardly compatible (with Reading EPG3) software systems for clinical intervention and laboratory-based analysis for EPG and acoustic data, and an enhanced clinical interface with client and file management tools. We focus here on a single case study of a child aged 10+/-years who had been diagnosed with an intractable speech disord

"I'm Destined to Ace This": Work Experience Placement During High School for Individuals with Autism Spectrum Disorder.

As postsecondary outcomes of adolescents with Autism Spectrum Disorder (ASD) are poor, there is a need for programs that aim to improve employment and education outcomes. This study employed a grounded theory approach to explore the key factors contributing to successful work placement experience and the perceived benefits of these placements from the perspective of adolescents with ASD (n = 5), their parents (n = 6) and employers (n = 6). Key factors contributing to success include preparing for the workplace, harnessing strengths and interests and developing work related skills, while the be

[Elbow dysplasia in the dog: finite element analysis].

For young active dogs of large, fast-growing breeds, diseases of the elbow represent an increasing important disorder. Genetic predisposition, overweight and joint overload have been proposed as possible causes of elbow dysplasia. In this study, the influence of various biomechanical parameters on load transfer in healthy and pathological dog elbows has been analysed by means of a two-dimensional finite element model. Pathological changes in the elbow structure, such as altered material properties or asynchronous bone growth, have a distinct influence on the contact pressure in the joint artic

CT evaluation of malformed external and middle ear and its surgical correlation.

This study evaluated the diagnostic value of high resolution CT (HRCT) for atresia of the external auditory meatus (EAM) and isolated middle ear malformation. CT scan of the temporal bone was done in 33 patients with such disorders and 5 patients with otosclerosis etc. were studied in the same way for comparison. Coronal HRCT clearly showed conditions of the atresia plate and malformation of the malleus, incus, abnormal course of facial canal and changes in the vestibular window. The axial HRCT is useful to demonstrate the articulations in between the malleus, incus, stapes and the cochlear wi

An Explanatory Model of Speech Communication Centered on Multiscale Rhythmic Modulation: Implications for Motor Speech Assessment and Intervention for Individuals With Amyotrophic Lateral Sclerosis.

PURPOSE: This study proposed an explanatory model of speech communication centered on multiscale rhythmic modulation to inform motor speech assessment and management. To these ends, a fit-for-purpose, automated measurement tool was used to evaluate and/or cross-validate (a) the previously reported effect of a neuromotor disorder-amyotrophic lateral sclerosis (ALS)-and (b) the effects of two cueing strategies, commonly used in managing motor speech disorders, on rhythmic modulation of speech. METHOD: A secondary analysis was carried out on the X-ray Microbeam database. The analyzed data includ

Investigating intervention dose frequency for children with speech sound disorders and motor speech involvement.

BACKGROUND: Treatment outcome data for children with severe speech sound disorders with motor speech involvement (SSD-MSI) are derived from Phase I clinical research studies. These studies have demonstrated positive improvements in speech production. Currently there is no research examining the optimal treatment dose frequency for this population. The results of this study, which is the first of its kind, will inform the delivery of effective services for this population. AIMS: To investigate optimal treatment dose frequency for the Motor Speech Treatment Protocol (MSTP) for children with SSD

Temporal variability in sung productions of adolescents who stutter.

UNLABELLED: Singing has long been used as a technique to enhance and reeducate temporal aspects of articulation in speech disorders. In the present study, differences in temporal structure of sung versus spoken speech were investigated in stuttering. In particular, the question was examined if singing helps to reduce VOT variability of voiceless plosives, which would indicate enhanced temporal coordination of oral and laryngeal processes. Eight German adolescents who stutter and eight typically fluent peers repeatedly spoke and sang a simple German congratulation formula in which a disyllabic

Cerebellar motor function in spina bifida meningomyelocele.

Spina bifida meningomyelocele (SBM), a congenital neurodevelopmental disorder, involves dysmorphology of the cerebellum, and its most obvious manifestations are motor deficits. This paper reviews cerebellar neuropathology and motor function across several motor systems well studied in SBM in relation to current models of cerebellar motor and timing function. Children and adults with SBM have widespread motor deficits in trunk, upper limbs, eyes, and speech articulators that are broadly congruent with those observed in adults with cerebellar lesions. The structure and function of the cerebellum

Degenerative disorders of the temporomandibular joint: etiology, diagnosis, and treatment.

Temporomandibular joint (TMJ) disorders have complex and sometimes controversial etiologies. Also, under similar circumstances, one person's TMJ may appear to deteriorate, while another's does not. However, once degenerative changes start in the TMJ, this pathology can be crippling, leading to a variety of morphological and functional deformities. Primarily, TMJ disorders have a non-inflammatory origin. The pathological process is characterized by deterioration and abrasion of articular cartilage and local thickening. These changes are accompanied by the superimposition of secondary inflammato

Childhood apraxia of speech: children at risk for persistent reading and spelling disorder.

This article discusses written language development in children with childhood apraxia of speech (CAS). Children with CAS are at risk for persistent reading and spelling disorder in addition to their spoken communication difficulties. The article highlights four factors that increase the risk of written language disorder in this population: (1) the nature of the speech disorder, (2) the presence of phonological awareness difficulties, (3) genetic risk factors, and (4) the negative impact of early reading difficulty on later written language development. The article suggests that traditional ap

[Open rhinophonia in adults: a rare manifestation of cranial polyneuritis].

A 46-year-old male patient reported difficulties in speech and swallowing following gastroenteritis. Marked open nasality (open rhinophonia) and swallowing difficulties with occasional passing of food into the nasopharynx was observed during speaking with the head held in an upright position. The patient was able to articulate clearly with the head reclined or in a lying position. Endoscopy identified complete bilateral soft palate paresis consistent with bilateral glossopharyngeal nerve palsy. Additional symptoms of cranial nerve palsy appeared in the course of the disease. [Language development disorders--studies of preschool children of the Leipzig special language school].

Children with severe speech disorders were examined to determine the causes of their abnormal speech development. Thirty-seven preschool and school-age children were followed up and results compared. All children had low intelligence quotients, with most of the children coming from low social backgrounds. However, both of these factors (intelligence and low social background) had no influence on the improvement of speech levels after therapy. Two-thirds of the children with moderate conductive hearing losses showed improvement after therapy, so that the effect of this hearing loss was believed

Pearls: diplopia.

Double vision may arise from ocular, neurologic, or extraocular muscle disorders. The approach to patients with diplopia requires a systematic approach to the history and the physical examination. There are many challenges in the evaluation of diplopia, ranging from the patient's mental status to the fine points of the examination. This article provides a process for interviewing and examining these patients, explaining the rationale and differential diagnosis of various clinical presentations. Common causes of monocular and binocular diplopia are addressed by the pattern of diplopia described

Transsexualism - attitudes in general practice.

Transsexualism was long regarded by the medical profession as a mental disorder. Historically, denial, aversion therapies, hormone 'reinforcement' and even electroconvulsive shock treatments were the lot of those compelled to articulate their overwhelming need to identify as members of the gender opposite that assigned to them at birth. We now know and understand that, just as the gonads, genitals and chromosomes are differentiated as to gender, so too is the brain. While the relationship between brain morphology and 'gender identity' is yet to be finally proven beyond scientific doubt, the ex

Chromosome 22q11.2 microdeletion in a patient with hemophilia A.

We report a 6-year-old patient with hemophilia A, who also exhibited clinical features typical of 22q11.2 deletion syndrome (22qDS). The specific traits were mild mental retardation, speech delay, hypernasal speech, deficits in voice quality and articulation, narrow palpebral fissures, broad and depressed nasal root, high-arched palate, microstomia, and overfolded ears. The patient had no associated congenital cardiac or palatal malformations. It can be particularly difficult to identify this syndrome in newborns and infants without congenital heart defects. This case underlines that microdele
