Valsartan for prevention of recurrent atrial fibrillation.

BACKGROUND: Atrial fibrillation is the most common cardiac arrhythmia, and no current therapy is ideal for control of this condition. Experimental studies suggest that angiotensin II-receptor blockers (ARBs) can influence atrial remodeling, and some clinical studies suggest that they may prevent atrial fibrillation. METHODS: We conducted a large, randomized, prospective, placebo-controlled, multicenter trial to test whether the ARB valsartan could reduce the recurrence of atrial fibrillation. We enrolled patients who were in sinus rhythm but had had either two or more documented episodes of a…

Developmental and genetic aspects of atrial fibrillation.

Atrial fibrillation (AF) is the most common cardiac arrhythmia encountered in clinical practice. The abnormal rhythm is associated not only with a variety of symptoms, such as palpitations, dizziness, or shortness of breath, but also with increased risk of stroke, heart failure, and mortality. A genetic predisposition is suggested by the fact that the relative risk for the development of AF is estimated at 85% in individuals with at least one parent with a history of AF. Current therapeutic strategies include control of rate or rhythm with medication and catheter ablation procedures. Especiall…

Patterns of activation in human atrial fibrillation.

The electrophysiological properties within the pulmonary vein (PV) and PV-left atrial (LA) junction were evaluated using a basket catheter in patients with paroxysmal atrial fibrillation. The effective refractory period of the distal PV was significantly shorter than that of the PV-LA junction. The conduction delay from the distal to proximal PV was significantly longer than that from the proximal to distal PV. During initiation of AF, a reentry involving exit and entrance breakthrough points at the PV-LA junction was observed. The presence of anisotropic conduction properties at the PV-LA jun…

[The role of permanent pacing in atrial fibrillation].

Nowadays permanent pacing of the heart helps patients not only with pathological bradycardia or asystole in the tract of the AV blocks (AVB) and sick sinus syndrome (SSS), but is also valuable in the prevention of atrial fibrillation (AF) as well as the stabilization of the rhythm in chronic AF. For this reason patients with AF are often candidates for implantation of a cardiac pacemaker. The basic problem for doctor qualifying patients for pacemaker implantation with AF is the choice of a suitable type of pacing. However, this is not the only problem. New possibilities in the tract of solid a…

Pacing therapy for prevention of atrial fibrillation.

The role of permanent pacing for the prevention of atrial fibrillation is reviewed. A moderate decrease in the incidence of acute and chronic atrial fibrillation has been seen with atrial pacing compared with ventricular pacing, especially in patients with sinus node dysfunction based on a meta-analysis of clinical trials. A variety of different pacing algorithms have been studied in small numbers of patients for only short durations of time and have shown an inconsistent effect on different measures of atrial fibrillation burden. The interatrial septum or Bachmann's bundle has been shown in s…

Anticoagulation in atrial fibrillation: a contemporary viewpoint.

Anticoagulation therapy in patients with atrial fibrillation is important. This review consists of three parts: chronic anticoagulation, anticoagulation for cardioversion, and a brief comment on anticoagulation around the time of left atrial radiofrequency ablation. The risk stratification scheme of the American College of Cardiology/American Heart Association/European Society of Cardiology (ACC/AHA/ESC) guidelines for chronic anticoagulation is briefly reviewed. Although there are several other similar schemes, they are not identical. The key point is the balance between benefit and risk. Som…

Prevention of stroke in patients with atrial fibrillation.

Nonvalvular atrial fibrillation (AF) is an independent risk factor for stroke that becomes increasingly prevalent as populations age. More than half a dozen clinical trials have demonstrated that anticoagulation with the vitamin K antagonist warfarin is the most effective therapy for stroke prophylaxis in AF. The narrow therapeutic index of warfarin requires that the intensity of anticoagulation be maintained within the international normalized ratio (INR) range of 2.0 to 3.0 to optimize efficacy while minimizing bleeding risk. The pharmacokinetics of warfarin are subject to variability due to…

Pharmacological approaches in the treatment of atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and is associated with substantial cardiovascular morbidity and mortality. The arrhythmia can be initiated and/or maintained by rapidly firing foci, single- and multiple-circuit reentry. Once initiated, AF alters atrial electrical and structural properties (atrial remodeling) in a way that promotes its own maintenance and recurrence and may alter the response to antiarrhythmic drugs. Thus, initial episodes of paroxysmal (self-terminating) AF lengthens to the point where the arrhythmia becomes persistent (requires cardiove…

[Atrial fibrillation as a complication of colorectal tumors].

We undertook a study to provide a quantitative basis to the clinical observation of an unexpected high frequency of atrial fibrillation in patients with colorectal cancer. In our study we considered a total of 1463 patients admitted during a period of 12 years (1987-1998) to the Department of Surgery of our Hospital for surgical treatment of colorectal cancer (case group: mean age 66 years) or non-neoplastic diseases including inguinal hernia, cholelithiasis, varicose veins and hemorrhoids (control group: mean age 65.6 years). We found a three times higher likelihood of having atrial fibrillat…

Atrial fibrillation in hypertension: predictors and outcome.

Incidence, determinants, and outcome of atrial fibrillation in hypertensive subjects are incompletely known. We followed for up to 16 years 2482 initially untreated subjects with essential hypertension. At entry, all subjects were in sinus rhythm. Subjects with valvular heart disease, coronary artery disease, preexcitation syndrome, thyroid disorders, or lung disease were excluded. During follow-up, a first episode of atrial fibrillation occurred in 61 subjects at a rate of 0.46 per 100 person-years. At entry, subjects with future atrial fibrillation differed (all P<0.05) from those without by…

Management of the older person with atrial fibrillation.

Atrial fibrillation (AF) is associated with a higher incidence of mortality, stroke, and coronary events than is sinus rhythm. AF with a rapid ventricular rate may cause a tachycardia-related cardiomyopathy. Immediate direct-current (DC) cardioversion should be performed in patients with AF and acute myocardial infarction, chest pain due to myocardial ischemia, hypotension, severe heart failure, or syncope. Intravenous beta blockers, verapamil, or diltiazem may be given to slow immediately a very rapid ventricular rate in AF. An oral beta blocker, verapamil, or diltiazem should be used in pers…

[Pathophysiology of pacing in patients with atrial fibrillation].

The limited efficacy of and side effects associated with antiarrhythmic drug therapy have led to renewed interest in non-pharmacologic treatment options for paroxysmal atrial fibrillation. In addition to catheter ablation of the initiating ectopic atrial beats, electrical stimulation of the atrium is a new and promising method to reduce the frequency of arrhythmia recurrences. Recent studies have confirmed the importance of both the initiating triggers and the electrophysiologic substrate for the recurrence and perpetuation, respectively, of atrial fibrillation. Bradycardia and pauses, atrial …

[Diagnostic approaches and complications of atrial fibrillation].

Atrial fibrillation is the most frequently occurring cardiac arrhythmia. Its mode of clinical presentation is varied. Frequently asymptomatic, it is nonetheless often responsible for tiredness, dyspnea or palpitations. More rarely, it presents with one of its two major complications: cardiac failure or a systemic thromboembolic event, often cerebral.

Use of beta-blockers in atrial fibrillation.

Severe community-acquired pneumonia (CAP) is a life-threatening condition that requires intensive care unit (ICU) admission. Clinical presentation is characterized by the presence of respiratory failure, severe sepsis, or septic shock. Severe CAP accounts for approximately 5-35% of hospital-treated cases of pneumonia with the majority of patients having underlying comorbidities. The most common pathogens associated with this disease are Streptococcus pneumoniae, Legionella spp., Haemophilus influenzae, and Gram-negative enteric rods. Microbial investigation is probably helpful in the individua

Role of transthoracic echocardiography in atrial fibrillation.

Atrial fibrillation is a major clinical problem that is predicted to be encountered more frequently as the population ages. The clinical management of atrial fibrillation has become increasingly complex as new therapies and strategies have become available for ventricular rate control, conversion to sinus rhythm, maintenance of sinus rhythm, and prevention of thromboembolism. Clinical and transthoracic echocardiographic features are important in determining etiology and directing therapy for atrial fibrillation. Left atrial size, left ventricular wall thickness, and left ventricular function h…

Localization of arrhythmogenic triggers of atrial fibrillation.

INTRODUCTION: Mapping procedures to identify triggers of atrial fibrillation from pulmonary veins (PVs) are not well established. We sought to determine the value of multipolar recordings from the coronary sinus (CS) and crista terminalis (CT) for identifying the origin of paced and atrial premature depolarizations (APDs) initiating atrial fibrillation from left versus right PVs. METHODS AND RESULTS: Fifteen patients with paroxysmal atrial fibrillation refractory to medications had decapolar catheters (5-mm electrode, 2-mm interelectrode spacing) placed in the CS and posterior medial to the C…
