Relationship between the frequency of paroxysmal episodes of atrial fibrillation and pulmonary venous flow pattern.

AIMS: Chronic atrial fibrillation causes mechanical remodelling of the atria, but it is uncertain whether self-terminating episodes of paroxysmal atrial fibrillation (PAF) alter atrial mechanical function during normal sinus rhythm. This study was designed to assess the relationship between the frequency of symptomatic arrhythmic episodes and pulmonary venous flow (PVF) pattern among patients with PAF. METHODS AND RESULTS: The effect of symptomatic arrhythmic episodes on PVF was studied in 85 patients with lone PAF (age 48+/-8 years, 66 men). PVF was measured with transthoracic echocardiograp

The effects of daily dose of intense exercise on cardiac responses and atrial fibrillation.

Atrial fibrillation (AF) is a supraventricular tachyarrhythmia that is strongly associated with cardiovascular (CV) disease and sedentary lifestyles. Despite the benefits of exercise on overall health, AF incidence in high-level endurance athletes rivals that of CV disease patients, suggesting a J-shaped relationship with AF. To investigate the dependence of AF vulnerability on exercise, we varied daily swim durations (120, 180 or 240 min day-1 ) in 7-week-old male CD1 mice. We assessed mice after performing equivalent amounts of cumulative work during swimming (i.e. ∼700 L O2  kg-1 ), as dete

Overexpression of junctate induces cardiac hypertrophy and arrhythmia via altered calcium handling.

Junctate-1 is a newly identified integral endoplasmic/sarcoplasmic reticulum Ca2+ binding protein. However, its functional role in the heart is unknown. In the present study, the consequences of constitutively overexpressed junctate in cardiomyocytes were investigated using transgenic (TG) mice overexpressing junctate-1. TG mice (8 weeks old) showed cardiac remodeling such as marked bi-atrial enlargement with intra-atrial thrombus and biventricular hypertrophy. The TG mice also showed bradycardia with atrial fibrillation, reduced amplitude and elongated decay time of Ca2+ transients, increased

Transcriptional changes associated with advancing stages of heart failure underlie atrial and ventricular arrhythmogenesis.

BACKGROUND: Heart failure (HF) is a leading cause of mortality and is associated with cardiac remodeling. Vulnerability to atrial fibrillation (AF) has been shown to be greater in the early stages of HF, whereas ventricular tachycardia/fibrillation develop during late stages. Here, we explore changes in gene expression that underlie the differential development of fibrosis and structural alterations that predispose to atrial and ventricular arrhythmias. OBJECTIVE: To study transcriptomic changes associated with the development of cardiac arrhythmias in early and late stages of heart failure.

Ventricular electrical delay as a predictor of arrhythmias in patients with cardiac resynchronization implantable cardioverter defibrillator.

BACKGROUND: Left ventricular (LV) remodeling and clinical response to cardiac resynchronization therapy (CRT) is inversely related to electrical dyssynchrony, measured as LV lead electrical delay (QLV). Presence of atrial or ventricular arrhythmia is correlated with worsening heart failure and LV remodeling. OBJECTIVE: We sought to assess the association of QLV with arrhythmic events in CRT recipients. METHODS: We identified patients implanted with a CRT device at our center. QLV interval was measured and corrected for baseline QRS (cQLV). We performed multivariable Logistic regression to as

Characteristics of Low-Voltage Zones in Patients With Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Atrial fibrillation (AF) frequently coexists with heart failure with preserved ejection fraction (HFpEF), and low-voltage zones (LVZs) in the atria represent atrial remodeling. However, the impact of HFpEF on the extent and distribution of LVZs and ablation outcomes remains unknown. OBJECTIVE: To characterize LVZs in patients with HFpEF undergoing AF ablation and evaluate their association with arrhythmia recurrence. METHODS: A total of 128 consecutive patients (66±12 years) who underwent AF ablation for the first time were retrospectively analyzed. HFpEF was defined as a left ve…

Low-density lipoprotein cholesterol is inversely associated with left atrial low-voltage areas in patients with persistent atrial fibrillation: a retrospective cohort study.

BACKGROUND: Left atrial low-voltage areas (LVAs) reflect advanced atrial remodeling and are associated with postablation recurrence in patients with persistent atrial fibrillation (PerAF). However, the relationship between routinely measured metabolic parameters and LVAs remains insufficiently characterized. This study aimed to identify clinical factors associated with LVAs in patients with PerAF, with particular emphasis on metabolic parameters. METHODS: This retrospective single-center cohort included consecutive PerAF patients who underwent initial catheter ablation between January 2021 an…

Breast tumour-secreted ADAM10 mediates atrial fibrogenesis and fibrillation.

BACKGROUND AND AIMS: Breast cancer (BC)-related atrial fibrillation (AF) contributes to an overall poor prognosis and raises great concerns. However, beyond cancer treatment-related AF, it is unknown whether BC itself is sufficient to directly induce AF. This study aimed to explore the role of BC in the development of AF, focusing on the tumour itself rather than therapy-related cardiotoxicity. METHODS: A retrospective analysis was conducted on 1224 female BC patients before therapy and 18 159 healthy female participants. The prevalence of AF was assessed, and markers of atrial remodelling, i

Rivaroxaban Treatment Prevents Atrial Myocyte Hypertrophy in Goats with Persistent Atrial Fibrillation by Inhibition of Protease-Activated Receptor-1.

Atrial fibrillation (AF) is associated with an increased risk of stroke and hypercoagulability. Coagulation factors mediate remodeling processes via protease-activated receptors (PARs) in various organs. We evaluated whether inhibition of factor Xa (FXa) via rivaroxaban protects against atrial structural remodeling in goats with persistent AF and explored FXa and thrombin hypertrophic effect on human iPSC-derived cardiomyocytes (hiPSC-CMs).Three groups of goats were tested: CTRL AF (control AF, n = 10), RIVA AF (rivaroxaban treatment during AF, n = 11), and SHAM (no AF, n = 10). Pacing-induced

Recurrence of atrial fibrillation after internal cardioversion of persistent atrial fibrillation: prognostic importance of electrophysiologic parameters.

BACKGROUND: The purpose of this study was to determine whether the extent of atrial electrical remodeling affects the recurrence of atrial fibrillation (AF) after cardioversion of persistent AF (PAF). METHODS AND RESULTS: Internal atrial cardioversion was performed in 47 patients with PAF. The right atrial monophasic action potential duration (RA-MAPD) at pacing cycle lengths (PCLs) of 800-300 ms and P wave signal-averaged electrocardiogram were recorded after cardioversion. Bepridil (150-200 mg/day) and carvedilol (10 mg/day) were administered to all patients after cardioversion. Of the 47 p

Increased dispersion and shortened refractoriness caused by verapamil in chronic atrial fibrillation.

OBJECTIVES: The objective was to assess the effect ofverapamil on atrial fibrillation (AF) cycle length and spatial dispersion of refractoriness in patients with chronic AF. BACKGROUND: Previous studies have suggested that verapamil prevents acute remodeling by AF. The effects of verapamil in chronic AF are unknown. METHODS: During electrophysiologic study in 15 patients with chronic AF (duration >1 year), 12 unipolar electrograms were recorded from right atrial free wall, right atrial appendage and coronary sinus, along with monophasic action potential recordings from the right atrial appen

Erosion of the Right Atrial Wall After Implantation of a Patch-Like Septal Occluder.

CASE SUMMARY: Percutaneous closure of secundum-type atrial septal defects (ASDs) using patch-like Gore devices is anticipated to be safe, even in deficient rims. Here, however, we report the first case of atrial wall erosion following a technically straightforward and successful Gore cardioform ASD occluder implantation since its Food and Drug Administration and European Conformity approval. We conclude that, in principle, any device can cause mechanical erosion of the atrial wall, especially since the unpredictable extent of right atrial remodeling after unloading or the varying integrity of …

Left ventricular remodeling and dysfunction in primary aldosteronism.

Primary aldosteronism (PA) is a common cause of secondary hypertension and is associated with worse cardiovascular outcomes. The elevated aldosterone in PA leads to left ventricular (LV) remodeling and dysfunction. In recent decades, clinical studies have demonstrated worse LV remodeling including increased LV mass and cardiac fibrosis in patients with PA compared to patients with essential hypertension. Several mechanisms may explain the process of aldosterone-induced LV remodeling, including directly profibrotic and hypertrophic effects of aldosterone on myocardium, increased reactive oxygen

Updated pathophysiological overview of functional MR (ventricular and atrial).

Basic mechanism of ventricular functional mitral regurgitation (FMR) is subvalvular tethering. Left ventricular (LV) dilatation, in association with mitral valve (MV) annular dilatation, causes outward displacement of papillary muscles (PMs), which abnormally pulls or tethers MV leaflets, resulting in MV tenting, reduction in leaflets coaptation and MR. Because surgical annuloplasty does shorten distance between anterior and posterior MV annuli to improve coaptation but does not address this subvalvular tethering, ventricular FMR frequently persists or recurs in the chronic stage after surgica

Chimerism analysis within 6 months of allogeneic stem cell transplantation predicts relapse in acute myeloid leukemia.

BACKGROUND: Atrial septal defects (ASD) are associated with an increased incidence of atrial arrhythmias, but their electrophysiological consequences are poorly defined. We hypothesised that conduction and repolarisation would be preferentially altered in the right atrium of ASD patients. OBJECTIVE: To quantify atrial conduction and repolarisation in ASD patients and determine the impact of structural remodelling on restitution properties. METHODS: Patients with an ASD (n = 22) underwent bi-atrial electroanatomic mapping and quantification of effective refractory periods, longitudinal and tr

From moderate to strenuous training: unravelling mechanistic contributors and biomarkers for atrial fibrillation in exercise.

AIMS: The impact of the transition from moderate to strenuous exercise on atrial fibrillation (AF) risk and its underlying mechanisms remain poorly understood. We aimed to analyse biatrial remodelling after moderate and strenuous exercise, compare it with pathological atrial damage, and non-invasively identify strenuous exercise insults. METHODS AND RESULTS: Young male Wistar rats were trained at a moderate (MOD) or high-intensity (INT) load; sedentary rats served as controls. After 16 weeks, electrophysiological and echocardiographic studies were obtained, and atrial samples were used for fi

Favorable cardiac remodeling in response to treatment in obstructive hypertrophic cardiomyopathy: a current appraisal.

Hypertrophic cardiomyopathy is a heterogenous genetic condition caused by myocyte disarray with varying degrees of interstitial fibrosis. Traditional management strategies focused on symptom mitigation and sudden cardiac death prevention among high-risk patients. The only disease-modifying treatments have historically been septal reduction therapy or heart transplantation. However, cardiac myosin inhibitors have emerged as promising novel pharmacotherapies. Emerging evidence indicates that cardiac myosin inhibitors as well as surgical myectomy result in not only symptomatic benefit and reducti
