The effects of alcohol on heart muscle have been explored in numerous studies of normal and diseased humans, intact animals, and isolated animal heart muscle preparations. The results vary according to dose, route, duration, and frequency of administration, parameters measured, and pathologic state of subjects. Most studies indicate that alcohol in sufficient doses decreases myocardial contractility. The dose required for this effect in humans may be lower if there is clinical evidence of heart muscle disease or if the subject has ingested substantial amounts of alcohol for a long time.

BACKGROUND: Thiamine deficiency is common in parts of Asia and causes beriberi. Pharmacokinetics of thiamine in deficient populations are unknown.
OBJECTIVE: We characterized thiamine pharmacokinetics in Cambodian mothers and their breastfed infants.
DESIGN: Total plasma thiamine, whole-blood thiamine diphosphate (TDP), and breast milk total thiamine were measured in 16 healthy Cambodian mothers and their infants before and after mothers received oral thiamine hydrochloride (100 mg for 5 d). Assays were also performed in 16 healthy American mothers.
RESULTS: On day 1, Cambodian mothers were…

This paper investigates a suspected outbreak of beri-beri which occurred in The Gambia in 1988. The outbreak was spatially specific and had a distinct age and gender bias, with young men aged 15-49 being the group mainly affected. The age and gender bias of the outbreak is particularly interesting as women and children in the developing world are usually more susceptable to conditions associated with malnourishment, than men. The authors investigate the peculiar nature of the outbreak and find that they cannot easily explain the apparent age and gender bias. The authors note that at the time o

Toxigenic fungi and mycotoxins entered human food supplies about the time when mankind first began to cultivate crops and to store them from … ds of people in Europe in the last millennium. Known in Japan since the 17th century, acute cardiac beriberi associated with the consumption of moldy rice was found to be due to citreoviridin produced by Penicillium citreonigrum. This toxin was believed to be only of historic importance until its reemergence in Brazil a few years ago. Other Penicillium tox …

Thirty-eight patients with beri-beri (13 dry, 14 wet and 11 mixed type) were seen at the Royal Victoria Hospital, Banjul, The Gambia, in 1990-1991, with a peak in September-October (25 cases) during the latter half of the rainy season. They were all non-confined residents of urban areas. Overt risk factors were pregnancy (6), alcohol (5), fevers (4), chronic disability (4), and exercise (2). Four patients (10.5%) died. The potential for large outbreaks of beri-beri exist in many urban areas of West Africa where polished rice is the staple diet.

Micronutrient deficiencies occur frequently in refugee and displaced populations. These deficiency diseases include, in addition to the most common Fe and vitamin A deficiencies, scurvy (vitamin C deficiency), pellagra (niacin and/or tryptophan deficiency) and beriberi (thiamin deficiency), which are not seen frequently in non-emergency-affected populations. The main causes of the outbreaks have been inadequate food rations given to populations dependent on food aid. There is no universal solution to the problem of micronutrient deficiencies, and not all interventions to prevent the deficiency…

The concept of risk in the field of avitaminoses is very important and useful for the practitioner, who should consider two aspects: a) risk … vitaminoses can be distinguished in deficiency with: a) a complete clinical symptomatology (scurvy, beriberi, pellagra, rickets, osteomalacia, xerophthalmia, hemeralopia); b) an incomplete clinical symptomatology (mono- or oligo-symptomatic or partial clinical picture); c) a biochemical symptomatology only (subclinic or clinically asymptomatic picture).

Evidence suggests disparities in breastfeeding … tritional practices in adolescents and young women focusing on ethnic groups (Hausa, Fulani, Kanuri/Beriberi) and geographic regions (South East) that are lagging behind.

BACKGROUND: Thiamine (vitamin B(1)) deficiency is associated with severe diseases such as beriberi and Wernicke encephalopathy. Although most Americans have sufficient dietary intake, thiamine deficiency is observed in the alcohol-dependent and elderly populations. Measurement of thiamine concentration in whole blood provides an assessment of vitamin B(1) status in at-risk individuals.
METHOD: We used TCA to precipitate proteins in whole blood. Thiamine and its phosphate esters were derivatized using potassium ferricyanide to thiochromes, which were separated by gradient elution on a reversed…

On the other hand, some breast-fed infants suffered from beriberi and lead encephalopathy. 5. Until 1974-75, when the recommendations of WHO and Japanese Government were announced, there had been a steady decline in breastfeeding throughout Japan. Thereafter, physicians supported breastfeeding and made efforts to …

Thiamine is an essential water-soluble vitamin that plays an important role in energy metabolism. Thiamine deficiency presents many challenges to clinicians, in part due to the broad clinical spectrum, referred to as thiamine deficiency disorders (TDDs), affecting the metabolic, neurologic, cardiovascular, respiratory, gastrointestinal, and musculoskeletal systems. Concurrent illnesses and overlapping signs and symptoms with other disorders can further complicate this. As such, TDDs are frequently misdiagnosed and treatment opportunities missed, with fatal consequences or permanent neurologic
