Emerging drugs for psoriatic arthritis.

INTRODUCTION: The majority of Psoriatic Arthritis patients experience a good clinical response to anti-Tumor Necrosis Factor (TNF)-α therapies. However, treatment failure with anti-TNF-α can represent a relevant clinical problem. AREAS COVERED: We review the efficacy and safety profile of biological therapies that have been reported from randomized, controlled trials in phase II and phase III available in Pubmed Database for agents targeting IL-12/23p40 antibody (ustekinumab) and IL-17 (secukinumab), inhibitor of phosphodiesterase 4, (apremilast), and of JAK/STAT pathways (tofacitinib) and CT

Biologic therapy in Crohn's disease: state of the art.

PURPOSE OF REVIEW: This review is intended to appraise the evolution and latest developments in treatment strategies and management of patients with Crohn's disease. RECENT FINDINGS: The last 24 months has further established the role of anti-tumor necrosis factor therapy in Crohn's disease with two new agents demonstrating efficacy in well designed randomized controlled trials. Furthermore, other important strategies have been identified including inhibition of leukocyte trafficking and blocking of interleukin-12 and 23. Along with the evolution in drug therapies there has been more extensiv

[Yersinia pestis pathogenesis and diagnostics].

Plague is an acute bacterial infection caused by Gram negative organism Yersinia pestis. This bacteria is subdivided into three classical biotypes: Orientalis, Medievalis and Antiqua. Plague is transmitted via flea vectors from rodents to humans and by respiratory droplets from animals to humans or humans to humans. This agent is on the top of the CDC list of "Critical Biological Agents"--category A. It appears to be a good candidate agent for a bioterrorist attack. Type III secretion (TTS) is a mechanism by which Y. pestis communicates with eukaryotic cells by injecting bacterial proteins acr

Enhanced liver cell mutations in trematode-infected Big Blue transgenic mice.

Parasite infections in humans have long been associated with specific types of cancers. Schistosoma hematobium is a known inducer of urinary bladder cancer, Helicobacter pylori is a gastric carcinogen, and hepatitis B virus and Opisthorchis viverrini are causative agents of liver cell cancers. Another liver fluke, Fasciola hepatica, has also been identified as a neoplastic risk agent, primarily in animals. We used F. hepatica as a model agent to determine if the presence of an aggressive liver fluke could induce mutagenic events in mammalian tissue. Using the Big Blue(R) transgenic mouse assay

Switching to tezepelumab from other biologics in patients with severe asthma: a retrospective study.

BACKGROUND: Tezepelumab, an anti-thymic stromal lymphopoietin antibody, has shown efficacy in reducing exacerbations and improving control in patients with severe asthma (SA), including those with low type 2 (T2) inflammation. We aimed to evaluate the efficacy and safety of switching to tezepelumab in patients with SA that was inadequately controlled by previous biologics. METHODS: In this retrospective study, we included patients with SA previously treated with at least one biologic and subsequently switched to tezepelumab (210 mg every 4 weeks) for ≥24 weeks between December 2022 and Decemb

Enhancing schistosomiasis control strategy for zimbabwe: building on past experiences.

Schistosoma haematobium and Schistosoma mansoni are prevalent in Zimbabwe to levels that make schistosomiasis a public health problem. Following three national surveys to map the disease prevalence, a national policy on control of schistosomiasis and soil transmitted helminths is being developed. This paper reviews the experiences that Zimbabwe has in the area of schistosomiasis control with a view to influence policy. A case study approach to highlight key experiences and outcomes was adopted. The benefits derived from intersectoral collaboration that led to the development of a model irrigat

Performance assessment of an equine metabolomics model for screening a range of anabolic agents.

INTRODUCTION: Despite their ban, Anabolic Androgenic Steroids (AAS) are considered as the most important threat for equine doping purposes. In the context of controlling such practices in horse racing, metabolomics has emerged as a promising alternative strategy to study the effect of a substance on metabolism and to discover new relevant biomarkers of effect. Based on the monitoring of 4 metabolomics derived candidate biomarkers in urine, a prediction model to screen for testosterone esters abuse was previously developed. The present work focuses on assessing the robustness of the associated

Pharmacological management of osteogenesis.

Osteogenesis and bone remodeling are complex biological processes that are essential for the formation of new bone tissue and its correct functioning. When the balance between bone resorption and formation is disrupted, bone diseases and disorders such as Paget's disease, fibrous dysplasia, osteoporosis and fragility fractures may result. Recent advances in bone cell biology have revealed new specific targets for the treatment of bone loss that are based on the inhibition of bone resorption by osteoclasts or the stimulation of bone formation by osteoblasts. Bisphosphonates, antiresorptive agen

A review of hydrogen sulfide (H2S) donors: Chemistry and potential therapeutic applications.

Hydrogen sulfide (H2S) is a ubiquitous small gaseous signaling molecule, playing an important role in many physiological processes and joining nitric oxide and carbon monoxide in the group of signaling agents termed gasotransmitters. Endogenous concentrations of H2S are generally low, making it difficult to discern precise biological functions. As such, probing the physiological roles of H2S is aided by exogenous delivery of the gas in cell and animal studies. This need for an exogenous source of H2S provides a unique challenge for chemists to develop chemical tools that facilitate the study o

Detection and management of latent tuberculosis infections before biologic therapy for psoriasis.

The biologic agents can be highly efficacious in the treatment of psoriasis and psoriatic arthritis; however, their use is associated with an increased risk of developing active TB. In particular, TNF-α plays critical role in preventing TB infection and reactivation of latent TB infection (LTBI). Therefore, it is critical that all patients be screened for LTBI prior to initiating therapy. An expert panel of Italian dermatologists met recently with the goal of producing a consensus paper on screening and chemoprophylaxis for LTBI in Italian psoriasis patients treated with biologics. Toxicological evaluation of dry ice expanded tobacco.

A tiered testing strategy has been developed to evaluate the potential of tobacco processes, ingredients, or technological developments to change the biological activity resulting from burning tobacco. The strategy is based on comparative chemical and biological testing. Dry ice expanded tobacco (DIET) is an example of a common tobacco expansion process currently used in the manufacture of cigarettes to increase tobacco filling capacity. As part of the toxicological evaluation of DIET, test cigarettes containing DIET were compared with control cigarettes containing tobacco expanded with a trad

Cognitive impairment in elderly patients with rheumatic disease and the effect of disease-modifying anti-rheumatic drugs.

Recent development of biologic disease-modifying anti-rheumatic drugs (DMARDs) has led to better control of disease activity among patients with chronic rheumatological diseases. Many patients with rheumatic disease are living longer, adding to the growing elderly population. Rheumatic diseases, most notably rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE), are known to increase the risk of cognitive impairment. Systemic inflammation associated with chronic rheumatological diseases has been postulated to be key driver of cognitive decline. Recent development of classic and biol

Methotrexate in inflammatory bowel disease.

Over the past decade methotrexate has emerged as a new treatment for chronically active Crohn's disease. Although controlled trials to compare the relative efficacy and safety of azathioprine and methotrexate in therapy-resistant patients are desirable, these studies will be difficult, if not impossible, to conduct because of the relatively small differences in potency and tolerability between these agents. A more productive area for future investigations is to explore the use of these drugs in combination with infliximab and other biologic treatments.
