Viral metagenomics in Brazilian multiply transfused patients with sickle cell disease as an indicator for blood transfusion safety.

BACKGROUND AND AIM: Patients with sickle cell disease (SCD) are submitted to multiple transfusions in order to increase the oxygen capacity of the blood, decrease blood viscosity, and suppress the sickling of the cells. Multiply transfused patients with SCD represent significant risk of acquiring parenterally transmitted infections. The analysis of the virome profile of high-risk multiply transfused patients with SCD can reveal the presence of parenterally transmitted viruses and therefore be used an indirect approach for evaluation of blood transfusion safety. MATERIALS AND METHODS: Blood sa…

Prediction of massive intraoperative blood transfusion requirement in lung transplantation.

BACKGROUND: Massive transfusion (MT) during lung transplantation (LTx) is associated with primary graft dysfunction and increased risk of post-LTx mortality and morbidity. To date, predictive factors for MT during LTx are unknown. This study aimed to establish a predictive model for intraoperative MT in patients undergoing LTx to provide personalized risk prediction. METHODS: This retrospective cohort study included adult patients who underwent LTx at the Second Affiliated Hospital of the Zhejiang University School of Medicine between 2020 and 2023. MT was defined as an intraoperative transfu

Whole-Blood Transfusion From Empiricism to Evidence: A Narrative Review.

IMPORTANCE: Hemorrhage remains the leading preventable cause of trauma death, and many fatalities occur before definitive hemorrhage control. Whole blood has reemerged as a promising strategy for early hemostatic resuscitation. However, its modern role remains incompletely defined because the available evidence is heterogeneous and the adoption of whole blood is shaped by safety considerations, implementation challenges, and inequities in access. OBSERVATION: Whole blood delivers red cells, plasma factors, and platelets in physiologic proportions through a single product, compressing the time

Prehospital whole blood transfusion versus component therapy-A scoping review.

BACKGROUND: Whole blood has gained a foothold over component therapy in civilian emergency care, especially in emergency departments. In a prehospital setting, whole blood might be the logistically easier alternative compared to component therapy. However, most previous studies have been conducted in the emergency room setting. We performed a scoping review to explore what is known about the outcomes, safety and benefits of whole blood given prehospitally compared to component therapy in a primarily adult civilian population. STUDY DESIGN AND METHODS: We formed a systematic search strategy an

A Rare Case of Yellow Urticaria Developing During Blood Transfusion.

Yellow urticaria is a rare clinical variant of urticaria characterized by yellowish wheals, typically associated with hyperbilirubinemia. Its occurrence as a transfusion-related reaction is uncommon and may mimic more serious transfusion complications. We report the case of a 15-year-old Saudi female patient who presented with prolonged vaginal bleeding and severe iron deficiency anemia requiring packed red blood cell transfusion. Approximately 15 minutes after transfusion initiation, she developed pruritic yellowish urticarial wheals over the trunk and extremities, without angioedema or hemod

Artificial elevation of phosphatidylethanol due to red blood cell transfusion.

INTRODUCTION: Phosphatidylethanol (PEth) is a marker of alcohol consumption used in clinical and forensic settings. PEth positivity in individuals expected to abstain from alcohol can have serious consequences. PEth is located on erythrocytes, thus packed red blood cell (pRBC) transfusion is a potential cause of false-positive results. This report is the first to demonstrate this phenomenon in an authentic patient who was negative for PEth immediately prior to transfusion. METHODS: Residual blood samples collected for clinical testing before and after pRBC transfusion and citrated pRBC segmen

Predictors of Blood Transfusion in Patients Undergoing Lumbar Spinal Fusion.

OBJECTIVE: To determine risk factors for perioperative blood transfusion after lumbar fusion surgery. METHODS: After institutional review board approval, a retrospective cohort study of adult patients who underwent lumbar fusion at a single, urban tertiary academic center was retrospectively retrieved. Our primary outcome, blood transfusion, was collected via chart query. A receiver operating characteristic curve was used to evaluate the regression model. A P-value < 0.05 was considered statistically significant. RESULTS: Of the 3,842 patients, 282 (7.3%) required a blood transfusion. For pa…

The Use of Whole Blood Transfusion During Non-Traumatic Resuscitation.

BACKGROUND: Evidence from military populations showed that resuscitation using whole blood (WB), as opposed to component therapies, may provide additional survival benefits to traumatically injured patients. However, there is a paucity of data available for the use of WB in uninjured patients requiring transfusion. We sought to describe the use of WB in non-trauma patients at Brooke Army Medical Center (BAMC). MATERIALS AND METHODS: Between January and December 2019, the BAMC ClinComp electronic medical record system was reviewed for all patients admitted to the hospital who received at least

West Nile virus and blood transfusion safety: A European perspective.

BACKGROUND AND OBJECTIVES: There is a growing concern for the transmission of arboviral infections by blood transfusion in Europe. However, no assessment of the risk of transmission through all European blood supplies has been reported. Risk regulations at a European level should take differences in local transmission risk and the risk of transmission by travelling donors into consideration. MATERIALS AND METHODS: A risk model and publicly available tool were developed to calculate the risk of transmission by all European blood supplies for arboviral outbreaks within Europe. Data on individua…

Factors associated with postcesarean blood transfusion: a case control study.

OBJECTIVE: Cesarean delivery (CD) is a known risk factor for postpartum hemorrhage. However, the characteristics associated with post-CD transfusion are not well-established. We aimed to assess blood transfusion rates and associated factors following CD. METHODS: A retrospective case-control study of women who underwent CD at a university hospital. The study group comprised all women who received blood transfusion following surgery. A control group of women who did not receive postoperative blood transfusion was assigned in a two-to-one ratio. RESULTS: During study period, the overall post-C…

Red blood cell transfusion for people undergoing hip fracture surgery.

BACKGROUND: The incidence of hip fracture is increasing and it is more common with increasing age. Surgery is used for almost all hip fractures. Blood loss occurs as a consequence of both the fracture and the surgery and thus red blood cell transfusion is frequently used. However, red blood cell transfusion is not without risks. Therefore, it is important to identify the evidence for the effective and safe use of red blood cell transfusion in people with hip fracture. OBJECTIVES: To assess the effects (benefits and harms) of red blood cell transfusion in people undergoing surgery for hip frac

Precautions surrounding blood transfusion in autoimmune haemolytic anaemias are overestimated.

BACKGROUND: It is very evident that many precautions are taken regarding transfusion of red blood cells in patients with autoimmune haemolytic anaemia. Frequently, considerable efforts are made to examine the indication and serological compatibility prior to transfusion in such patients. However, at times, this may unnecessarily jeopardize patients who urgently require a red blood cell transfusion. MATERIALS AND METHODS: Thirty-six patients with warm-type autoimmune haemolytic anaemia were included in this study. All patients had reactive serum autoantibodies and required blood transfusion. S…

Tranexamic Acid reducing blood transfusion in children undergoing craniosynostosis surgery.

BACKGROUND: Surgical correction of craniosynostosis in children is associated with substantial intraoperative bleeding. Intraoperatively administered tranexamic acid (TXA) can lessen blood loss during orthopedic and cardiovascular surgery, but its efficacy in craniosynostosis surgery is uncertain. Therefore, a meta-analysis performed with published comparative studies was to determine whether TXA could reduce packed red blood cells (or erythrocytes) (PRBCs) transfused and blood loss during pediatric craniosynostosis surgery. METHODS: Two PubMed and EMBASE electronic databases were searched un
