Standard Model Imaging in the Brain and Spinal Cord of MS Patients: Initial Assessment and Comparison to Diffusion Tensor Imaging.

Multiple sclerosis (MS) pathology manifests across the central nervous system (CNS), yet magnetic resonance imaging (MRI) studies frequently assess the brain and spinal cord (SC) in isolation. It remains unclear whether microstructural injury in normal-appearing white matter (NAWM) and lesions manifests identically across these distinct anatomical environments or if advanced biophysical models offer diagnostic advantages over standard diffusion techniques. We performed a comprehensive 3T MRI protocol with same-session brain and cervical SC imaging on 34 persons with relapsing-remitting MS (pwR

Radiofrequency electrical conductivity reveals a distinct dimension of early human brain development.

Quantitative MRI has characterised early human brain development primarily through measures of water mobility, leaving other biophysical properties of maturing tissue unexplored. Here we show that radiofrequency electrical conductivity, reflecting ionic composition, membrane density and extracellular geometry in addition to water content, can be extracted retrospectively and at scale from the phase of conventional MRI acquisitions, revealing a distinct dimension of early brain tissue maturation and state. Applying electrical property tomography to 888 neonatal MRI sessions (784 subjects, 26-45

The utility of serial echocardiograms for organ procurement in brain death.

Heart transplantation rates are limited by a shortage of donor hearts, and left ventricular dysfunction is an important cause. We hypothesized that an early echocardiographic evaluation in severe brain injury (performed within 12 hours from ICU admission) could allow the detection of potentially reversible left ventricular (LV) abnormalities and thus the initiation of tailored treatment whose effects could be assessed at a second echocardiogram performed when brain death develops. We assessed this hypothesis in 49 patients with severe brain injury who were potential heart donors. A reduction i

Retinal nerve fiber layer thickness and thalamus pathology in multiple sclerosis patients.

BACKGROUND AND PURPOSE: Visual impairments are frequent in multiple sclerosis (MS). Optic neuritis can directly reduce retinal nerve fiber layer (RNFL) thickness. Our objectives were to evaluate associations of the RNFL thickness (RNFLT) of MS patients with magnetic resonance imaging (MRI) measures of regional brain atrophy and tissue injury in the post-chiasmatic deep gray matter (GM) section of the visual pathway. METHODS: Retinal nerve fiber layer thickness was measured using optical coherence tomography (OCT) in 96 relapsing-remitting MS (RR-MS) patients and 46 controls. MRI was obtained

Combat Helmet Suspension System Stiffness Influences Linear Head Acceleration and White Matter Tissue Strains: Implications for Future Helmet Design.

Combat helmets are expected to protect the warfighter from a variety of blunt, blast, and ballistic threats. Their blunt impact performance is evaluated by measuring linear headform acceleration in drop tower tests, which may be indicative of skull fracture, but not necessarily brain injury. The current study leverages a blunt impact biomechanics model consisting of a head, neck, and helmet with a suspension system to predict how pad stiffness affects both (1) linear acceleration alone and (2) brain tissue response induced by both linear and rotational acceleration. The approach leverages diff

Immunohistochemical optimization of a panel of antibodies against amyloid precursor protein for immunodetection of axonal injury in neurologic diseases of sheep.

Amyloid precursor protein (APP) is the most sensitive early immunomarker of axonal injury in the brain. Currently, there are several commercially available antibodies to detect axonal spheroids expressing APP. However, immunolabeling of a given molecule can be variable when antibodies are directed to different epitopes. In this study, we optimized a panel of antibodies (22C11, Y188, NAB228, C1/6.1) for labeling APP-immunopositive axonal spheroids in ovine brains and compared the patterns of injury in brains containing spheroids from neurotoxic (clostridial focal symmetrical encephalomalacia [F

Cognitive and neural effects of exercise following traumatic brain injury: A systematic review of randomized and controlled clinical trials.

Objective: Sub-maximal aerobic exercise can alleviate brain injury-related symptom burden. There is substantial data from animal studies and a growing clinical evidence base to suggest that exercise may also improve cognitive and neural outcomes following brain injury. We performed this systematic review to consolidate evidence from randomized and controlled clinical trials on the effects of exercise on cognitive and neuroimaging outcomes following brain injury in humans.Design: Systematic review.Data sources: MEDLINE, EMBASE, PsychINFO, CINAHL, SPORTDiscus, and Cochrane Central Database.Eligi

Numerical Reconstruction of Cyclist Impact Accidents: Can Helmets Protect the Head-Neck of Cyclists?

Cyclists are vulnerable road users and often suffer head-neck injuries in car-cyclist accidents. Wearing a helmet is currently the most prevalent protection method against such injuries. Today, there is an ongoing debate about the ability of helmets to protect the cyclists' head-neck from injury. In the current study, we numerically reconstructed five real-world car-cyclist impact accidents, incorporating previously developed finite element models of four cyclist helmets to evaluate their protective performances. We made comparative head-neck injury predictions for unhelmeted and helmeted cycl

Characterization of axonal injury produced by controlled cortical impact.

Axonal injury and behavioral changes were evaluated 3-7 days after traumatic brain injury. Previous research from this laboratory demonstrated that clinical central nervous pathology is produced by dynamic brain compression using a stroke-constrained impactor. We wanted to determine if the technique also would produce diffuse axonal injury after recovery from the procedure. The experiments were performed at Wayne State University School of Medicine using aseptic techniques while assuring analgesic care. Impacts were performed at 4.3 m/sec or 8.0 m/sec, with congruent to 10% compression (2.5 mm

A prospective microstructure imaging study in mixed-martial artists using geometric measures and diffusion tensor imaging: methods and findings.

Although diffusion magnetic resonance imaging (dMRI) has been widely used to characterize the effects of repetitive mild traumatic brain injury (rmTBI), to date no studies have investigated how novel geometric models of microstructure relate to more typical diffusion tensor imaging (DTI) sequences. Moreover, few studies have evaluated the sensitivity of different registration pipelines (non-linear, linear and tract-based spatial statistics) for detecting dMRI abnormalities in clinical populations. Results from single-subject analyses in healthy controls (HC) indicated a strong negative relatio

Topological Aberrance of Structural Brain Network Provides Quantitative Substrates of Post-Traumatic Brain Injury Attention Deficits in Children.

Background: Traumatic brain injury (TBI)-induced attention deficits are among the most common long-term cognitive consequences in children. Most of the existing studies attempting to understand the neuropathological underpinnings of cognitive and behavioral impairments in TBI have utilized heterogeneous samples and resulted in inconsistent findings. The current research proposed to investigate topological properties of the structural brain network in children with TBI and their relationship with post-TBI attention problems in a more homogeneous subgroup of children who had severe post-TBI atte

Traumatic brain injury: neuropathological, neurocognitive and neurobehavioral sequelae.

Traumatic brain injury (TBI) causes substantial neurological disabilities and mental distress. Annual TBI incidence is in magnitude of millions, making it a global health challenge. Categorization of TBI into severe, moderate and mild by scores on the Glasgow coma scale (GCS) is based on clinical grounds and standard brain imaging (CT). Recent research focused on repeated mild TBI (sport and non-sport concussions) suggests that a considerable number of patients have long-term disabling neurocognitive and neurobehavioral sequelae. These relate to subtle neuronal injury (diffuse axonal injury) v

Multistage investigation of predictive factors for tracheostomy in brain injury: a bibliometric, descriptive, and retrospective analysis.

BACKGROUND: Brain injury, particularly traumatic brain injury (TBI), stands as a prominent global cause of mortality and disability. Tracheostomy in TBI patients may lead to added complications. However, the current literature lacks consistency regarding predictive factors for tracheostomy in this patient population. This study seeks to investigate and validate specific predictive factors associated with the need for tracheostomy in TBI patients through a multi-faceted approach involving bibliometric analysis, descriptive examination, and retrospective research. METHODS: This study employs a

Development of brain injury criteria (BrIC).

Rotational motion of the head as a mechanism for brain injury was proposed back in the 1940s. Since then a multitude of research studies by various institutions were conducted to confirm/reject this hypothesis. Most of the studies were conducted on animals and concluded that rotational kinematics experienced by the animal's head may cause axonal deformations large enough to induce their functional deficit. Other studies utilized physical and mathematical models of human and animal heads to derive brain injury criteria based on deformation/pressure histories computed from their models. This stu

Magnesium sulfate (MG) prevents maternal inflammation induced offspring cerebral injury evident on MRI but not via IL-1β.

OBJECTIVE: As maternal treatment with magnesium sulfate (MG) may protect the fetal brain, we sought to assess the inflammation associated neuroprotective potential of MG and its association to interleukin 1β (IL-1β). METHODS: Pregnant Sprague-Dawley rats at 18-day gestation received i.p. lipopolysaccharide (LPS) or saline. Dams were randomized to treatment with s.c. saline (control), or MG prior to or following the i.p. injection, resulting in three groups. At the end of the treatment, fetal brain IL-1β was quantified for 18 pregnant rats (six of each group). Another 18 pregnant rats delivere

Neuropathology of explosive blast traumatic brain injury.

During the conflicts of the Global War on Terror, which are Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF), there have been over a quarter of a million diagnosed cases of traumatic brain injury (TBI). The vast majority are due to explosive blast. Although explosive blast TBI (bTBI) shares many clinical features with closed head TBI (cTBI) and penetrating TBI (pTBI), it has unique features, such as early cerebral edema and prolonged cerebral vasospasm. Evolving work suggests that diffuse axonal injury (DAI) seen following explosive blast exposure is different

[Head injuries: What the team physician needs to know].

A concussion is the mildest form of a mild traumatic brain injury (tbi) and resembles the most prevalent type of sports associated tbi. Diffuse axonal injuries, the main pathophysiological mechanism of concussion, leads to disruption of communication between different brain areas. The resulting clinical symptoms may relate to several clinical domains (cognition, fatigue, anxiety disorders, headaches/migraines or vestibulo-ocular problems), all of which need to be assessed in a clinical screening during an evaluation for possible concussion. Appropriate and consensus-based protocols to conduct

Diffusion tensor imaging: a review for pediatric researchers and clinicians.

Diffusion tensor imaging (DTI) is a magnetic resonance imaging technique that allows for the visualization and characterization of the white matter tracts of the brain in vivo. DTI does not assess white matter directly. Rather, it capitalizes on the fact that diffusion is isotropic (equal in all directions) in cerebral spinal fluid and cell bodies but anisotropic (greater in one direction than the other directions) in axons that comprise white matter. It provides quantitative information about the degree and direction of water diffusion within individual units of volume within the magnetic res
