Single-agent Bevacizumab in the Treatment of Symptomatic Newly Diagnosed and Recurrent/Refractory Pediatric Cervicomedullary Brainstem Low-grade Gliomas: A Single Institutional Experience.

Bevacizumab-based therapies have been utilized as single or combination therapy of refractory/recurrent pediatric low-grade gliomas. Its efficacy for symptomatic cervicomedullary low-grade gliomas (cmLGGs) in the upfront and the recurrent setting is less known. We report our retrospective single institutional experience from 2015 to 2021 with single-agent bevacizumab for symptomatic cmLGG. Six consecutive patients (4 female, ages 2 to 12 y) with newly diagnosed (n=3) and recurrent/refractory (n=3) symptomatic nondisseminated cmLGG (5/6 biopsy-proven, 2 BRAFV600E, 2 BRAF-KIAA1549) were treated

Radiotherapy for diffuse brainstem glioma in children and young adults.

BACKGROUND: Diffuse brainstem glioma is a devastating disease with very poor prognosis. The most commonly used radiological treatment is conventional fractionated radiation. So far, there is no meta-analysis or systematic review available that assesses the benefits or harms of radiation in people with diffuse brainstem glioma. OBJECTIVES: To assess the effects of conventional fractionated radiotherapy (with or without chemotherapy) versus other therapies (including different radiotherapy techniques) for newly diagnosed diffuse brainstem gliomas in children and young adults aged 0 to 21 years.

A phase II study of the farnesyl transferase inhibitor, tipifarnib, in children with recurrent or progressive high-grade glioma, medulloblastoma/primitive neuroectodermal tumor, or brainstem glioma: a Children's Oncology Group study.

BACKGROUND: An open-label Phase II study of tipifarnib was conducted to evaluate its safety and efficacy in children with recurrent or refractory medulloblastoma (MB)/primitive neuroectodermal tumor (PNET), high-grade glioma (HGG), and diffuse intrinsic brainstem glioma (BSG). METHODS: Between January 2004 and July 2005, patients were enrolled and stratified as follows: Stratum 1, recurrent or refractory MB/PNET; Stratum 2, recurrent or refractory HGG; and Stratum 3, recurrent or refractory BSG. Patients received tipifarnib 200 mg/m2 per dose twice daily for 21 days repeated every 28 days. Pa

Patterns of recurrence in brain stem gliomas: evidence for craniospinal dissemination.

PURPOSE: The 3-year survival rate of pediatric patients with infiltrating brain stem gliomas (BSG) is < 10%. Treatment involves local field radiation, and local failure has been the hallmark of recurrence. With therapeutic advances and improved radiographic monitoring, perceived and actual patterns of failure may change. We report patterns of recurrence in a group of patients with close follow-up, treated on an institutional protocol incorporating hyperfractionated involved-field radiation therapy and concomitant carboplatin, who have been uniformly staged and treated and have undergone MRI su

Primary brain stem tumours.

The material consisted of 31 patients with primary brain stem tumours who were investigated retrospectively. Twenty patients were younger than 16 years of age (range: 1 1/2-65 years). The mean age was 11 years. The female/male sex ratio was 19/12. Histological diagnoses were available for 18 patients. All of the verified tumours were gliomas. The diagnoses in 12 cases were made by section or biopsy; in 15 by standard clinical and radiological examinations and in 4 by exploratory operations. Headaches, mental changes, speech and gait disturbances were the most common symptoms at the onset of th

Update of ocular reticulum cell sarcoma.

Reticulum cell sarcoma involving the vitreous and the brainstem occurred in a 45-year-old man. He initially was seen with diplopia from a partial left-sided third cranial nerve palsy, which is rare. Later, a typical uveitis developed in the right eye. An initial diagnosis of brainstem glioma, based primarily on the computed tomographic scan findings and clinical history, was ultimately proved erroneous when the correct diagnosis was shown by the results of a cytologic examination of vitreous aspirate. Excellent visual response to a moderately high oral dose of steroids occurred, which has not

Magnetic resonance imaging of pediatric posterior fossa tumors.

MR detected abnormality in all 115 pediatric patients who subsequently had pathologically proven posterior fossa tumors. In 114, the initial magnetic resonance (MR) diagnosis was that of brain tumor. In 1, with less than 1-cm2 area of gadolinium enhancement, the significance of the initial finding was uncertain. Common posterior fossa tumor subgroups (brainstem gliomas, cerebellar astrocytomas, primitive neuroectodermal tumors, and ependymomas) have relatively consistent presentations on imaging studies. However, less common tumors mimic the MR appearance of more common ones, while common tumo

[Clinicophysiological studies of orbicularis oculi reflex in brain stem diseases].

Orbicularis oculi reflex is consisted of two components; one is early reflex response (R1) which is 14.1 +/- 2.2 msec in latency, and the other is late reflex response (R2) that is 35.0 +/- 8.6 msec in latency (mean +/- 2SD). R1 is observed on the ipsilateral side of stimulation and R2 is visible bilaterally on ipsiand contralateral sides of stimulation. Clinically orbicularis oculi reflex can be recorded easily, however, its clinical and physiological significance has not been clearly solved. Orbicularis oculi reflex was tested on patients with the brain stem lesions of various kinds, and the

Generation of diffuse intrinsic pontine glioma mouse models by brainstem-targeted in utero electroporation.

BACKGROUND: Diffuse intrinsic pontine gliomas (DIPGs) are highly lethal childhood brain tumors. Their unique genetic makeup, pathological heterogeneity, and brainstem location all present challenges to treatment. Developing mouse models that accurately reflect each of these distinct features will be critical to advance our understanding of DIPG development, progression, and therapeutic resistance. The aims of this study were to generate new mouse models of DIPG and characterize the role of specific oncogenic combinations in DIPG pathogenesis. METHODS: We used in utero electroporation (IUE) to

Multiple enchondromatosis Ollier's disease with two primary brain tumors.

A rare case of multiple enchondromatosis associated with 2 primary brain tumors is reported. A 21-year-old female who has diagnosed as Ollier's disease at the age of 9-years was admitted to the hospital with complaint of diplopia and progressive right upper and lower limb weakness. Magnetic resonance imaging of the brain depicted 2 intra-axial mass lesions. Stereotactic biopsy showed low grade fibrillary astrocytoma in both lesions. This report emphasizes that patients with Ollier's disease are at a higher risk for primary brain tumors than has been previously recognized.

The anterior inferior cerebellar artery in mass lesions. Preliminary findings with emphasis on the lateral projection.

Analysis of abnormalities of the configuration of the anterior inferior cerebellar artery in lateral projection facilitates diagnosis of posterior fossa masses. Cerebellopontomedullary angle lesions frequently cause arcuate posterior-inferior displacements of the rostrolateral artery or reversal of curvature of the ascending segment of the meatal loop. Vermian, hemispheric, intra-fourth ventricular, and suitably situated extra-axial masses alter the plane of the lateral loop of the caudomedial artery. Tonsillar herniation may be detected in patients with hypoplastic or absent posterior inferio

Dual perspective on autophagy in glioma: Detangling the dichotomous mechanisms of signaling pathways for therapeutic insights.

Autophagy is a normal physiological process that aids the recycling of cellular nutrients, assisting the cells to cope with stressed conditions. However, autophagy's effect on cancer, including glioma, is uncertain and involves complicated molecular mechanisms. Several contradictory reports indicate that autophagy may promote or suppress glioma growth and progression. Autophagy inhibitors potentiate the efficacy of chemotherapy or radiation therapy in glioma. Numerous compounds stimulate autophagy to cause glioma cell death. Autophagy is also involved in the therapeutic resistance of glioma. T

[A combination of a meningioma of the base of the skull and an arteriovenous micromalformation of the brain stem].

The paper describes a rare combination of atypical meningioma of the base of the skull, which was greatly extended, with a life-time unrecognized small vascular malformation of the brain stem at the level of the pons. Intraoperative hemorrhage into the stem from the micromalformation was a cause of the female patient's death. A detailed account of the operation and the postoperative period and a review of literature on the main aspects of the problem in the diagnosis of cerebral micromalformations are given.
