The Role of Cyclosporine in the Treatment of Severe Ulcerative Colitis in the Pediatric Population: A Narrative Review Exploring Known Strategies and New Possibilities.

Cyclosporine is a selective calcineurin inhibitor used, among other areas, in pediatric gastroenterology as rescue therapy in the treatment of severe ulcerative colitis (ASUC-acute severe ulcerative colitis) in children. The introduction of systemic glucocorticosteroids (GCSs) has significantly reduced mortality in ASUC. However, it should be emphasized that long-term use of these drugs is limited due to the high risk of adverse effects observed during therapy and their limited efficacy. The role of cyclosporine as a second-line therapy is referred to as a 'rescue therapy,' which aims to allev

Bridging with a single glucocorticoid injection in psoriatic arthritis: Real-world evidence of short-term effectiveness, safety, and enhanced response in high-inflammatory burden patients.

BACKGROUND: The use of adjunctive systemic glucocorticoids (GC) as bridging therapy in psoriatic arthritis (PsA) remains controversial due to limited disease-specific evidence and safety concerns. Although clinical experience suggests potential benefits in selected patients, clear criteria for appropriate use are lacking. The unique pharmacokinetic profile of intramuscular compound betamethasone offers a bridging strategy but has been insufficiently studied. OBJECTIVES: To investigate the real-world effectiveness and safety of single intramuscular GC injection as a bridging therapy for active

Lisocabtagene maraleucel for relapsed/refractory large B-cell lymphoma: a cell therapy consortium real-world analysis.

Lisocabtagene maraleucel (liso-cel) is an autologous CD19-directed chimeric antigen receptor T-cell therapy approved for the treatment of relapsed/refractory large B-cell lymphoma. We present a multicenter retrospective study evaluating safety, efficacy, and resource use of liso-cel in the standard-of-care setting. Patients received commercial liso-cel at 7 US medical centers, and patient selection, toxicity management, and disease assessment followed institutional practices. Among 101 patients who received infusion, the median age was 71 years (35% aged ≥75 years), 68% had a Charlson comorbid

Percutaneous coronary intervention as a bridge to renal transplantation in a patient with end-stage renal disease--a case report.

Renal transplantation is one of the preferred modes of replacement therapy in patients with end-stage renal disease. Cardiovascular disease remains the leading cause of morbidity and mortality in patients with end-stage renal disease and renal transplant recipients. The authors describe a patient with end-stage renal disease who developed unstable angina before renal transplantation. Emergent cardiac catheterization and percutaneous coronary intervention served as a bridge to his successful renal transplantation without complications.

The automated implantable cardiac defibrillator. Prophylaxis in cardiac sarcoidosis.

A patient with cardiac sarcoidosis proved by biopsy specimen and no history of sudden death or clinical sustained ventricular tachycardia prophylactically received an implantable cardioverter defibrillator (ICD) that later reversed an episode of near syncope. The patient was supported with the ICD until heart transplantation. The physiology and treatment of arrhythmias associated with cardiac sarcoidosis is described. Consideration for use of the ICD in asymptomatic patients and as bridge therapy until heart transplantation is discussed.

Extracorporeal Membrane Oxygenation Ethics: A Call for Action.

TOPIC IMPORTANCE: Extracorporeal membrane oxygenation (ECMO) is a relatively novel, intensive medical intervention. It is known to raise important yet unsettled ethical concerns. OVERVIEW: We established the ECMO Ethics Workgroup in 2021 to improve our understanding of these ethical issues and their possible resolutions. This paper explains the methods used to produce our insights and recommendations, which additional papers from the project elaborate on in detail. Initially, over 6 months, the 37 group members described the full range of ethical issues in ECMO care they were familiar with fr

Effect of a Conservative Enoxaparin Bridging Protocol for HeartMate 3 Left Ventricular Assist Devices.

Bridging subtherapeutic international normalized ratios (INRs) in patients with left ventricular assist devices is not well studied and may cause harm. This retrospective, single-center, pre- and post-cohort study assessed whether a more conservative enoxaparin bridging protocol for patients with subtherapeutic INRs correlated with a reduction in major bleeding and thrombotic events in patients with HeartMate 3 devices. Eighty-two patients and 237 subtherapeutic episodes were included. The rate of the primary composite outcome was numerically lower in the post-group (5.7% vs . 1.5%, p = 0.075)

Tracking symptom dynamics in people living with HIV on different treatment: a longitudinal cross-lagged panel network analysis.

OBJECTIVE: To characterize dynamic interrelationships among physical, cognitive, and psychological symptoms in people living with HIV and identify bridge symptoms between subgroups receiving distinct antiretroviral therapy regimens. METHODS: A longitudinal design was employed in this study. A total of 676 individuals diagnosed with HIV filled out the Self-Report Symptom Checklist (HIV-SRSC) at baseline, 3-month, and 6-month follow-ups in the clinic. We evaluated bridge symptoms-those that connect various communities-within longitudinal networks utilizing cross-lagged panel network analyses (C

Venoarterial extracorporeal membrane oxygenation is a viable option as a bridge to heart transplant.

OBJECTIVE: Venoarterial extracorporeal membrane oxygenation is a rescue therapy for patients in cardiogenic shock. We hypothesize that patients bridged to heart transplant with extracorporeal membrane oxygenation have decreased survival. METHODS: The United Network of Organ Sharing database was retrospectively reviewed from January 1, 1999, to March 31, 2018, for heart transplant recipients. Recipients bridged with any form of mechanical support and those without support were compared with recipients bridged with extracorporeal membrane oxygenation. The primary end point was restricted mean s

Radiation and Chimeric Antigen Receptor T-cell Therapy in B-cell Non-Hodgkin Lymphomas.

Chimeric antigen receptor T-cell therapy (CAR-T) is a revolutionary advancement in the management of chemotherapy refractory B-cell non-Hodgkin lymphomas representing a potentially curative therapy in scenarios that were previously only palliative. CAR-T cell therapy is associated with unique toxicities as well as practical challenges. One of those challenges is how to manage active lymphoma during the weeks-long CAR-T manufacturing process. Radiation therapy, steroids, and systemic therapy have all been used for what would be considered "bridging therapy" during this time frame. Radiation the

Risk factors for early death in patients bridged to transplant with continuous-flow left ventricular assist devices.

BACKGROUND: Recent evidence suggests patients bridged to heart transplant (BTT) have equivalent outcomes as those undergoing conventional orthotopic heart transplantation (OHT). However, data on risk factors for early death in BTT patients are limited. METHODS: We retrospectively reviewed the United Network for Organ Sharing database of all patients bridged to OHT with a HeartMate II from January 2005 to December 2010. The primary outcome was all-cause 90-day mortality. Additional postoperative outcomes were cerebrovascular accident and need for renal replacement therapy. Kaplan-Meier analysi

LVAD as a Bridge to Transplantation-Current Status and Future Perspectives.

Heart failure (HF) is a common disease associated with high morbidity and mortality rates despite advanced pharmacological therapies. Heart transplantation remains the gold standard therapy for end-stage heart failure; however, its application is curtailed by the persistent shortage of donor organs. Over the past two decades, mechanical circulatory support, notably Left Ventricular Assist Devices (LVADs), have been established as an option for patients waiting for a donor organ. This comprehensive review focuses on elucidating the benefits and barriers associated with this application. We prov

The HAS-BLED score predicts bleedings during bridging of chronic oral anticoagulation. Results from the national multicentre BNK Online bRiDging REgistRy (BORDER).

Patients who receive long-term oral anticoagulant (OAC) therapy often require interruption of OAC for an elective invasive procedure. Current guidelines allow bridging therapy with either unfractionated heparin (UFH) or low-molecular-weight heparin (LMWH). Apart from the risk of embolism, bleeding is an important complication in this setting and the optimal perioperative management of such patients is still under discussion. The aims of this prospective, observational, multicentre registry of patients treated by cardiologists were: 1) to evaluate current practice of perioperative management of

Repair of Sinus Venosus Atrial Septal Defect in 2 Patients With Severe Pulmonary Arterial Hypertension.

Pretreatment with targeted pulmonary arterial hypertension (PAH) therapy in adults with unrepaired atrial septal defects (ASDs) has been increasingly reported as a means to improve pulmonary hemodynamics and expand candidacy for defect closure. However, most published reports describe heterogeneous ASD cohorts, predominantly composed of secundum ASDs, with limited data on sinus venosus atrial septal defects (SVASDs). We describe two cases of adult superior SVASD with severe PAH, initially deemed unsuitable for closure, who underwent prolonged pretreatment with PAH-targeted therapy and subseque

A Case Report of HIV Transmission Within a Household Involving a Married Man Who Has Sex with Men in China.

BACKGROUND: Married Men who have Sex with Men (MSM) constitute a bridge population for HIV diffusion into low-risk groups, with spousal non-disclosure rates exceeding 60% in China. It will increase the risk of secondary transmission within families. Despite the timely implementation of evidence-based interventions, reducing MTCT rates to below 2% in China, residual pediatric infections persist. CASE REPORT: In 2025, a man in Jinan City, Shandong Province, was diagnosed with HIV infection likely acquired through sexual contact with men and may have subsequently transmitted the virus to his bre

Impella 5.0: An intermediate strategy for bridging a patient from infected durable LVAD to cardiac transplant.

Cardiac transplant remains the gold-standard treatment for end-stage heart failure more than 50 years after the procedure was first pioneered. The number of transplant operations performed annually has been limited by the global imbalance of donor organs to recipients. This imbalance helped fuel the development of left ventricular assist devices (LVADs) first as a bridge to cardiac transplant and subsequently as destination therapy. Driveline infection is one of several problems that continue to plague LVADs through several generations of improvement. We present a patient with an infected LVAD

[Extracorporeal life support and heart-lung transplant in children].

Extracorporeal life support and heart and/or lung transplant are the last resort in children with end-stage cardiac and/or pulmonary failure and short-term life threaten. Currently, circulatory support is used as a bridge to recovery or as a bridge to transplant but not as a destination therapy. The Excor Berlin Heart is the long-lasting external pneumatic ventricular assist system that is currently available from infancy to adulthood. Long-term prognosis after pediatric cardiac and/or pulmonary transplant is conditioned by the occurrence of graft failure, coronary disease of the cardiac graft

HeartWare ventricular assist device as a bridge to heart transplantation in a patient with congenitally corrected transposition of the great arteries and dextrocardia.

Congenitally corrected transposition of the great arteries (ccTGA) is a rare condition with prevalence of <0.5%. Dextrocardia is reported among 20% of them. Among patients with ccTGA, heart failure is a common presentation, especially in the fourth or fifth decade of life and survival is dismal without heart transplantation. A left ventricular assist device (LVAD) is considered for bridge to transplantation if early heart transplantation is not available or as destination therapy for patients ineligible for heart transplant. Our patient had ccTGA and dextrocardia, after which he developed fail
