Self-expanding metal stents (EMS) have gained widespread popularity for the palliative treatment of various stenoses in blood vessels, biliary ducts, bronchi, and the gastrointestinal tract. Regarding stenoses of the upper gastrointestinal tract, EMS insertion for gastric outlet and duodenal stenoses is considerably more difficult than for esophageal stenoses. We describe a reliable EMS insertion technique for malignant gastric outlet and duodenal stenoses.

Mounier-Kuhn syndrome, or tracheobronchomegaly, is a rare clinical and radiologic condition characterized by marked tracheobronchial dilation and recurrent lower respiratory tract infections. Diagnosis is typically accomplished with the use of computed tomography and bronchoscopy, as well as pulmonary function testing. Patients may be asymptomatic; however, symptoms can range from minimal with preserved lung function to severe respiratory failure. Therapy, if any, is supportive but minimal. Marked aneurysmal dilation of the central and branch pulmonary arteries in utero in patients with tetralogy of Fallot with absent pulmonary valve can often exhibit extrinsic compression of the trachea and bronchi. The major morbidity in these patients remains postoperative ventilation issues. This case report highlights the role of intraoperative bronchoscopy in providing guidance for obtaining optimal bronchial decompression that was achieved by an initial pulmonary arteriopexy followed by an aortopexy.

Pneumothorax secondary to nontuberculous mycobacterial lung disease is often intractable and associated with high mortality. An endobronchial Watanabe spigot (EWS) is a silicon plug developed for bronchoscopic bronchial occlusion. Here we report a case of intractable pneumothorax secondary to nontuberculous mycobacterial lung disease successfully treated using EWSs. Occlusion of the multiple ectatic bronchi connected to the bronchopleural fistula was essential for optimal effect of the EWSs.

A variety of pathologic processes can involve the central airways. Abnormalities may either diffusely or focally involve the tracheal or mainstem bronchial walls. Diseases that diffusely involve the tracheal wall can be subclassified as sparing the membranous trachea or circumferentially involving the tracheal wall. Focal diseases of the trachea and mainstem bronchi include benign and malignant causes. Additionally, congenital and acquired morphologic abnormalities of the trachea will be reviewed.

A 50-year-old man presented with respiratory distress from central airway compression secondary to malignant mediastinal adenopathy. The stenosis involved the carinal triangle and created residual luminal diameters of 6 mm, 6 mm, and pinhole in the distal trachea and right and left mainstem bronchi, respectively. Carcinoid is a slowly developing neuroendocrine tumour. It appears with frequency of 1.5/100,000 persons. Usually it is localized in appendix, small intestine, rectum and bronchi. Clinical sings. of carcinoid syndrome develop in only 10% cases of tumour. We present three cases of carcinoid: the first one with evidences of heart insufficiency, the second one with evidences of colon cancer, the third one coexisting with Graves-Basedow disease.

Two babies with asplenia syndrome are described. Both presented with the severe cardiovascular anomalies found in this syndrome and had Howell-Jolly bodies in peripheral blood smears. The chest X-rays showed dextrocardia, decreased pulmonary circulation and a horizontal symmetrical liver, and were helpful in the clinical diagnosis. Additional findings at autopsy included a mobile cecum and bilateral trilobed lungs with bilateral eparterial bronchi.

Report on a thanatophoric dwarfism, which is a variety of achondroplasia. New born children with this failure are not able to survive the first few days after birth. This early death is due to a cardiorespiratory deficiency as a consequence of the extremely short ribs but also of the abnormal cartilaginous growth in the wall of bronchi and bronchioles. In this present case of the thanatophoric dwarfism the question is discussed in how far the disorder of the bronchial cartilage is the basis of the respiratory distress.
