Anatomical and clinical relevance of Kager's triangle: diagnostic approaches and therapeutic implications for related pathologies.

BACKGROUND: Kager's triangle (KT) is an anatomically defined space in the posterior ankle, bordered by the Achilles tendon (AT), flexor hallucis longus (FHL), and superior margin of the calcaneus. It contains Kager's fat pad (KFP), which plays a crucial role in ankle biomechanics and is implicated in several pathologies, including Achilles tendinopathy, retrocalcaneal bursitis, and posterior ankle impingement syndrome (PAIS). This review aimed to evaluate the anatomical, radiological, and clinical characteristics of the KT and assess its diagnostic and therapeutic relevance in posterior ankle …

Infection of tendon sheaths, joints, bursae, soft tissue, and tendon rapture by brucella: A case report.

KEY CLINICAL MESSAGE: Arthritis is one of the main presentations of chronic brucellosis, but bursitis and tendon rupture are also caused by brucellosis. Therefore, brucellosis should be considered in the differential diagnosis of arthritis, bursitis, and tendon rupture by physicians. In addition, early diagnosis and treatment are very important in the prevention of disability. ABSTRACT: Brucellosis is a zoonotic disease common in the Middle East. Manifestations of acute disease are fever, sweating, myalgia, and arthralgia. However, bone joint involvement occurs in 10%-85% of patients, and sac…

[Insertional Achilles tendinopathy : Differentiated diagnostics and therapy].

Achilles tendinopathy at the calcaneal insertion is classified into insertional tendinopathy, retrocalcaneal and superficial bursitis. The aim of this study was to present the current evidence on conservative and surgical treatment of insertional tendinopathy of the Achilles tendon. Conservative first-line therapy includes reduction of activity levels, administration of non-steroidal anti-inflammatory drugs (NSAID), adaptation of footwear, heel wedges and orthoses or immobilization. In addition, further conservative therapy options are also available. Eccentric stretching exercises should be i…

Magnetic resonance imaging features for diagnosing adhesive capsulitis of the shoulder: a systematic review and meta-analysis.

BACKGROUND: Various magnetic resonance imaging (MRI) characteristics are frequently employed to aid diagnose adhesive capsulitis of the shoulder (ACS) and offer valuable therapeutic insights. To identify and summarize the diagnostic accuracy of these features, a systematic review and meta-analysis were performed. METHODS: Four databases, including PubMed, EMBASE, Web of Science, and Cochrane Library, were searched. Overlapping descriptions used to represent the same imaging in different studies are grouped into one MRI feature. Pooled diagnostic accuracy, including sensitivity and specificity

Musculoskeletal Embolotherapy.

Painful articular musculoskeletal (MSK) conditions are common, debilitating and sometimes difficult to treat. Transcatheter embolisation may offer an alternative way to manage the pain associated with these conditions, but the field is still in its infancy. To date, the most commonly studied indication is knee osteoarthritis, but the technique has been trailed in multiple other tissue beds, including for adhesive capsulitis and tendinopathies. Whilst early results appear promising, the existing studies are mostly open-labelled and non-randomised; there is a need for high-quality evidence to ro

Incidence of greater trochanteric pain syndrome in patients suspected for femoroacetabular impingement evaluated using magnetic resonance arthrography of the hip.

OBJECTIVES: We evaluated the incidence of greater trochanter pain syndrome (GTPS) in patients who underwent magnetic resonance arthrography (MRA) of the hip for a suspected femoroacetabular impingement (FAI) syndrome. METHODS: Hip MRA performed at our institution (3/2012-1/2014) were reviewed. The absence/presence of FAI (cam, pincer, and mixed) was noted. GTPS diagnosis was based on gluteus medius/minimus tendinopathy/tears, trochanteric bursitis, fascia lata thickening, and trochanter bone oedema/erosion. Subgroup analysis for age (under/over 40 years) and FAI type (cam, pincer, and mixed) …

Narrative: Review of Anterior Knee Pain Differential Diagnosis (Other than Patellofemoral Pain).

PURPOSE OF REVIEW: This review presents a framework for constructing a differential diagnosis for chronic anterior knee pain associated with overuse other than patellofemoral pain. Traumatic, systemic, and pediatric injuries will not be covered. RECENT FINDINGS: From superficial to deep, the anterior knee can be conceptually organized into four layers: (1) soft tissue, (2) extensor mechanism, (3) intracapsular/extrasynovial, and (4) intra-articular. From superficial to deep, diagnoses to consider include bursitis, patellar and quadriceps tendinosis, fat pad impingement, and plica syndromes.

Minimally Invasive and Endoscopic Treatment of Haglund Syndrome.

Haglund syndrome is a triad of posterosuperior calcaneal prominence (Haglund deformity), retrocalcaneal bursitis, and insertional Achilles tendinopathy. The sources of pain include the posterior calcaneal wall cartilage, retrocalcaneal and subcutaneous adventitial bursa, and the Achilles tendon. Resection of the posterosuperior calcaneal tubercle, bursectomy, excision of the Achilles tendon pathology, reattachment of the Achilles tendon, gastrocnemius aponeurotic recession, and flexor hallucis longus transfer have been proposed as surgical treatment options. All of them can be performed endosc…

[Greater trochanteric pain syndrome].

Trochanteric bursitis, also known as "greater trochanter pain syndrome", is a frequent and often under-diagnosed cause of pain in the lateral hip region. The diagnosis is essentially based on the clinical examination; however various forms of imaging may be useful to confirm the diagnosis and particularly to ex- clude other aetiologies. The different therapeutic options include non-steroidal anti-inflammatories, physiotherapy, local injections of cortisone and local anaesthetic, and extra-corporeal shock wave therapy. Surgical intervention is only indicated in rare cases.

An Unusual Presentation of a Left Extra-acetabular Trochanteric Bursa Abscess.

Trochanteric bursitis is a common cause of lateral hip pain, particularly among elderly and postmenopausal females, but abscess formation within the trochanteric bursa is exceedingly rare. We report an unusual extra-acetabular trochanteric abscess identified during cadaveric dissection of an elderly female, characterized by irregular fibrotic and proteinaceous tissue with multiple inclusion bodies but no evidence of malignancy or granulomatous inflammation. Although limited by an absent clinical history, the gross and histopathologic findings suggest a chronic inflammatory or degenerative proc…

Abductor Reconstruction with Gluteus Maximus Transfer in Primary Abductor Deficiency during Total Hip Arthroplasty.

Abductor deficiency in native hip joint may cause severe limping and pain. It is more serious situation in case of arthroplasty due to instability and recurrent dislocation. Well-known causes of abductor deficiency are repeated surgery, chronic trochanteric bursitis, superior gluteal nerve injury, failure of repair of abductor tendon insertion to the greater trochanter. Author had experienced primary abductor deficiency during total hip replacement and treated successfully with the transfer of gluteus maximus. We'd like to introduce the operation technique with the review of literature.
