Bilateral monitoring of cerebral oxygen saturation results in recognition of aortic cannula malposition during pediatric congenital heart surgery.

Congenital heart surgery is associated with a 2-25% reported incidence of neurological complication. Near-infrared spectroscopy (NIRS) can detect changes in regional cerebral saturation index (rSO2i) during cardiac surgery. If rSO2i decreases significantly, treatment algorithms are used to restore baseline values, potentially avoiding neurological complications. The efficacy of bilateral NIRS monitoring in pediatric congenital heart surgery has been debated. We report a case in which bilateral NIRS monitoring detected an abrupt decrease in rSO2i (right greater than left) after initiation of by

Implanted cannula-mediated repetitive administration of Abeta25-35 into the mouse cerebral ventricle effectively impairs spatial working memory.

Amyloid beta (Abeta) is closely related to the onset of Alzheimer's disease (AD). To construct AD animal models, a bolus administration of a large dose of toxic Abeta into the cerebral ventricles of rodents has been performed in earlier studies. In parallel, a continuous infusion system via an osmotic pump into the cerebral ventricle has been developed to make a rat AD model. In this study, we developed a mouse AD model by repetitive administration of Abeta25-35 via a cannula implanted into the cerebral ventricle. Using this administration system, we reproducibly constructed a mouse with impai…

Assessment of perfusion toward the aortic valve using the new dispersion aortic cannula during coronary artery bypass surgery.

When there is an echocardiographic diagnosis of severe mobile atherosclerotic plaque in the aortic arch or descending aorta, perfusion toward the aortic arch during cardiopulmonary bypass may create a high risk of embolic neurologic injury. Other perfusion methods, such as cannulation of the femoral or axillary arteries, are not always possible, due to atherosclerosis. The ascending aorta may be an alternative site for perfusion, since it is less frequently diseased. We assessed a new technique of perfusion toward the aortic valve using a new cannula designed for this purpose (Dispersion aorti…

[Incidence of hemorrhage following suprapubic Cystofix puncture using a new trocar-cannula combination--a randomized, prospective comparative study].

In this study a new trocar-cannula-unit(10 French) is introduced, which was used in a prospective randomised comparative study in 74 patients for suprapubic bladder punction. Another 75 patients served as controls receiving a suprapubic punction with the standard Cystofix (French 10)-system. In the control-group 9 cases (12%) of macrohematuria occurred. On the other hand we found that the incidence of macrohematuria was reduced to 5 cases (6.8%), when using the new trocar-cannula-unit, which had been developed in our hospitals.

Cannula Pairing Determines Distal Limb Perfusion During Peripheral Veno-Arterial Extracorporeal Membrane Oxygenation: An Experimental Mock-Loop Study.

BACKGROUND: Limb ischaemia remains a common complication of peripheral venoarterial extracorporeal membrane oxygenation (VA-ECMO). Reinforced introducer sheaths are commonly used off-label as distal perfusion cannulas; however, their ability to provide adequate distal flow across clinically relevant ECMO configurations remains uncertain. We sought to evaluate the influence of cannula size configurations on distal flow fraction using a simple mock circulatory loop. METHODS: Distal flow fraction was evaluated across 24 configurations comprising 15-21Fr arterial return cannulas and 7-9Fr reinfor…

Clinical Predictors of High-Flow Nasal Cannula Failure in Children with Acute Respiratory Illness: A Single-Center Experience.

High-flow nasal cannula (HFNC) therapy is increasingly used for managing pediatric respiratory distress; however, early identification of children at risk of treatment failure remains challenging. This retrospective observational study evaluated clinical predictors of HFNC failure among children aged 1 mo to 12 y admitted with moderate-to-severe respiratory illness at a tertiary care center between April and June 2025. Fifty children received HFNC therapy, of whom 44 (88%) had successful outcomes, while six (12%) required invasive mechanical ventilation, among these two died. HFNC failure was …

Aberrant Right Subclavian Artery Causing Dampening of the Right Radial Arterial Cannula Tracing Following Transesophageal Echocardiography Probe Insertion.

An aberrant right subclavian artery (ARSA) is the most common congenital anomaly of a left aortic arch. Given the anatomical course of the ARSA, it generally remains asymptomatic throughout life without hemodynamic or respiratory effects. We report the intraoperative diagnosis of ASRA during surgery for congenital heart disease when there was dampening of the waveform from the right radial arterial cannula during transesophageal echocardiography (TEE) probe placement. The subsequent identification of ARSA highlights the importance of considering vascular anomalies in cases of unexplained intra…

Airway Secretion Volume and Physiological Effects of High-Flow Nasal Cannula in Acute Respiratory Failure Because of Bronchiectasis.

BACKGROUND: Acute respiratory failure (ARF) in bronchiectasis is frequently accompanied by excessive airway secretions. High-flow nasal cannula (HFNC) delivers heated, humidified gas and may improve respiratory mechanics, but quantitatively measured secretion burden during HFNC in bronchiectasis-related ARF is poorly described. METHODS: We conducted a prospective, single-center pilot cohort study in a respiratory intermediate care unit including adults with computed tomography-confirmed bronchiectasis admitted with ARF and treated with HFNC as first-line therapy. The primary outcome was cumul…

Airway pressures generated by high flow nasal cannula in patients with acute hypoxemic respiratory failure: a computational study.

INTRODUCTION AND OBJECTIVES: High flow nasal cannula (HFNC) therapy is an increasingly popular mode of non-invasive respiratory support for the treatment of patients with acute hypoxemic respiratory failure (AHRF). Previous experimental studies in healthy subjects have established that HFNC generates flow-dependent positive airway pressures, but no data is available on the levels of mean airway pressure (mPaw) or positive end-expiratory pressure (PEEP) generated by HFNC therapy in AHRF patients. We aimed to estimate the airway pressures generated by HFNC at different flow rates in patients wit…

Comparative Effectiveness of High-Flow Nasal Cannula and Noninvasive Ventilation in Acute Hypoxemic Respiratory Failure: A Scoping Review.

Acute hypoxemic respiratory failure (AHRF) is a leading cause of ICU admissions, and noninvasive respiratory support modalities such as high-flow nasal cannula (HFNC) and noninvasive ventilation (NIV) are frequently employed. However, the comparative effectiveness of these two interventions across different patient populations remains unclear. This scoping review aimed to synthesize current evidence comparing HFNC and NIV in the management of adult patients with AHRF, focusing on outcomes such as intubation rates, mortality, patient comfort, oxygenation, and complications. Studies were include…

Effect of High-Flow Nasal Cannula vs. Facemask on Arterial Oxygenation During Liver Radiofrequency Ablation: Randomized Controlled Trial.

Background and Objectives: Percutaneous liver radiofrequency ablation (RFA) under monitored anesthesia care (MAC) carries a risk of hypoxia due to respiratory depression. Ensuring adequate oxygenation during such procedures is essential for patient safety. This study aimed to evaluate whether a high-flow nasal cannula (HFNC) improves oxygenation compared to a simple facemask during RFA. Materials and Methods: In this prospective, randomized controlled trial, 51 patients undergoing ultrasound-guided RFA under MAC were allocated to receive oxygen via an HFNC (30 L/min) or a facemask (6 L/min). A…

Intravenous anesthesia with high-flow nasal cannula improves recovery in elderly undergoing ureteroscopic lithotripsy: a prospective controlled study.

BACKGROUND: Intravenous anesthesia with high-flow nasal cannula (HFNC) has been reported to benefit oxygen reserves and enhance postoperative recovery in surgeries requiring low neuromuscular blockade. This study investigated whether HFNC improves recovery quality in elderly undergoing ureteroscopic holmium laser lithotripsy (UHLL). METHODS: We enrolled 106 elderly patients undergoing UHLL, with 96 patients (48 per group) included in the final analysis. Patients were randomly assigned (1:1, stratified by sex) to receive either HFNC (HFNC group) or laryngeal mask airway (LMA) assisted general …

Derivation of a Procedural Performance Checklist for Bifemoral Veno-Venous Extracorporeal Membrane Oxygenation Cannula Placement in Operational Environments.

BACKGROUND: Veno-venous extracorporeal membrane oxygenation (VV ECMO) is a low-frequency, high-intensity procedure used for severe lung illness or injury to facilitate rapid correction of hypoxemia and respiratory acidosis. This technology is more portable and extracorporeal support is more frequently performed outside of the hospital. Future conflicts may require prolonged causality care and more specialized critical care capabilities including VV ECMO to improve patient outcomes. We used an expert consensus survey based on a developed bifemoral VV ECMO cannulation checklist with an operation…

High-Flow Nasal Cannula Versus Noninvasive Ventilation as Initial Treatment in Acute Hypoxia: A Propensity Score-Matched Study.

IMPORTANCE: Patients presenting to the emergency department (ED) with hypoxemia often have mixed or uncertain causes of respiratory failure. The optimal treatment for such patients is unclear. Both high-flow nasal cannula (HFNC) and noninvasive ventilation (NIV) are used. OBJECTIVES: We sought to compare the effectiveness of initial treatment with HFNC versus NIV for acute hypoxemic respiratory failure. DESIGN SETTING AND PARTICIPANTS: We conducted a retrospective cohort study of patients with acute hypoxemic respiratory failure treated with HFNC or NIV within 24 hours of arrival to the Univ…

Enhancing exercise tolerance in interstitial lung disease with high-flow nasal cannula oxygen therapy: A randomized crossover trial.

BACKGROUND AND OBJECTIVE: Interstitial lung disease (ILD) is characterized by dyspnoea on exertion and exercise-induced hypoxaemia. High-flow nasal cannula (HFNC) therapy reduces the respiratory workload through higher gas flow and oxygen supplementation, which may affect exercise tolerance. This study aimed to examine the effects of oxygen and gas flow rates through HFNC therapy on exercise tolerance in ILD patients. METHODS: We conducted three-treatment crossover study. All ILD patients performed the exercises on room air (ROOM AIR setting: flow, 0 L/min; fraction of inspired oxygen [FiO2],…
