Omental transfer for the treatment of mediastinitis in an infant.

Mediastinitis following congenital heart surgery is relatively uncommon but is usually seen in the setting of postoperative low cardiac output. Conservative treatment utilizing debridement and irrigation is associated with significant morbidity and mortality. We report the successful application of the omental transfer technique in the treatment of mediastinitis in a 6 month old.

Unusual presentation of left atrial myxoma in a child mimicking acute poststreptococcal glomerulonephritis.

Cardiac myxomas are rare in children and can mimic other conditions. We report a 14-year-old boy initially diagnosed with acute poststreptococcal glomerulonephritis based on hematuria, mild proteinuria, and elevated anti-streptolysin-O titers. However, worsening dyspnea, edema, and signs of heart failure prompted further evaluation. Echocardiography revealed a large left atrial mass with impaired biventricular function and tricuspid regurgitation. Surgical excision confirmed a myxoma. The patient's renal and systemic symptoms were attributed to low cardiac output and venous congestion, not tru

Emergency surgical closure of postinfarction ventricular septal defect on the beating heart.

We present a 77-year-old female with recent myocardial infarction complicated by antero-apical ventricular septal defect (VSD) and quick development of low cardiac output. Using interrupted horizontal mattress sutures VSD closure was successfully performed on the beating heart. Transthoracic echocardiography at the four-month follow-up did not reveal residual VSD.

Update on surgical sepsis syndrome.

BACKGROUND: Sepsis is a serious complication in surgical patients, and is associated with prolonged hospital stay and high mortality rates. The definitions of sepsis have been revisited recently. This article reviews how definitions have changed over the years, and provides an update on basic pathobiology and essential aspects of treatment. METHODS: PubMed was searched for reports published in English before October 2016, using the search terms 'surgical sepsis' AND 'surgical ICU'. The reference lists of articles identified in the search were also checked. Other relevant literature was select

Changes in sarcomeric and non-sarcomeric cytoskeletal proteins and focal adhesion molecules during clinical myocardial recovery after left ventricular assist device support.

BACKGROUND: Reverse remodeling can occur after left ventricular assist device (LVAD) support, which is sufficient in some cases to allow explantation of the device without cardiac transplantation. The molecular mechanisms involved remain unknown. A specific pattern of expression of sarcomeric and non-sarcomeric proteins in the myocardium is thought to be essential for normal myocardial function. However, a detailed protein analysis of their role in recovery has not been performed previously. METHODS: Myocardial samples were collected at implantation and explantation in 7 patients with dilated

Outcome of percutaneous coronary intervention versus coronary bypass grafting for patients with low left ventricular ejection fractions, unstable angina pectoris, and risk factors for adverse outcomes with bypass (the AWESOME Randomized Trial and Registry).

The recently concluded Angina With Extremely Serious Operative Mortality Evaluation (AWESOME) was a randomized clinical trial of percutaneous coronary intervention versus coronary bypass graft surgery among patients with medically refractory ischemia who were at high risk for coronary bypass graft surgery because of > or =1 risk factors that included severely reduced left ventricular (LV) function, defined as LV ejection fraction <35%. This study reports the outcome of patients with LV ejection fraction <35% in the randomized clinical trial and the physician-directed and patient choice registr

Management of heart failure: evidence versus practice. Does current prescribing provide optimal treatment for heart failure patients?

Heart failure is an increasingly common and costly chronic disorder, with a rising prevalence of at least 2% in populations over the age of 45 years, mortality rates that are as poor as common solid cancers, and very high health care utilisation costs. Despite increased evidence supporting a range of effective interventions, predominantly therapeutic, there remain significant degrees of physician underperformance in terms of heart failure diagnosis and management. Until the early 1990s, the management of heart failure was largely confined to the symptomatic relief of patients with well establi

Skeletal muscle dysfunction in chronic obstructive pulmonary disease and chronic heart failure: underlying mechanisms and therapy perspectives.

Low exercise tolerance has a large influence on health status in chronic obstructive pulmonary disease and chronic heart failure. In addition to primary organ dysfunction, impaired skeletal muscle performance is a strong predictor of low exercise capacity. There are striking similarities between both disorders with respect to the muscular alterations underlying the impairment. However, different alterations occur in different muscle types. Histologic and metabolic data show that peripheral muscles undergo a shift from oxidative to glycolytic energy metabolism, whereas the opposite is observed

[Blunt chest trauma with severe pulmonary contusion and traumatic myocardial infarction].

In the literature the incidence of cardiac involvement in blunt chest trauma varies considerably. This reflects the diagnostic problems encountered in polytraumatised patients. We report the case of an 18 year old man who suffered bilateral pulmonary contusion and traumatic myocardial infarction following a motorbike accident. The myocardial infarct was diagnosed by means of ECG, cardiac enzymes and echocardiography. When the diagnosis was made the time for successful interventional treatment had lapsed. A coronary angiography was performed after stabilisation which revealed a proximal dilatat

[Measures for decreasing the early mortality after valvular replacement cardiovascular surgery].

To probe the effective measure for decreasing the early mortality after artificial valvular replacement. We analyzed the cause of death among 1215 patients receiving artificial valvular replacement who were admitted in our intensive care unit during 1990-1995. All 44 deaths were serious-illed patients with preoperative cardiac functions of 3-4 degree. The factors affecting surgical survival rate include: enhancement of patient's cardiac function before operation and prevention of cardiac arrhythmia; a clear operational view and myocardial protection, especially for reoperative patients; applic

Safety of high energy internal cardioversion for atrial fibrillation.

High energy internal cardioversion has been proposed as an alternative method to cardiovert drug refractory or external cardioversion refractory atrial fibrillation. However, the safety of this technique has not been clearly evaluated. We reviewed findings in 53 patients who underwent 55 sessions of high energy internal cardioversion (2 patients underwent 2 sessions) for termination of longstanding atrial fibrillation. Shocks energy varied from 70-270 J. Three patients had 3 shocks during the same session, 5 had 2, and 47 only 1. Success rate was 89% (success was defined as immediate conversio

Effects of aging on neuroendocrine activation in subjects and patients in the presence and absence of heart failure with left ventricular systolic dysfunction.

The neuroendocrine profile and echocardiographic features of 40 patients (81 +/- 1 years, means +/- standard error) with heart failure and impaired left ventricular systolic function were compared with those of an age-matched group of healthy subjects, 20 younger patients with heart failure (aged 58 +/- 1 years) and 15 younger healthy subjects. Normal elderly subjects had a neuroendocrine profile similar to that of healthy younger subjects apart from elevated plasma norepinephrine (958 +/- 84 vs 302 +/- 118 pg/ml; p< 0.001) and atrial natriuretic peptide ( 40 +/- 6 vs 28 +/- 5 pg/ml; p<0.05).

Hemopump fails as bridge to transplantation in postinfarction ventricular septal defect.

From 1990, six patients were bridged to transplantation with a catheter-mounted axial flow pump (Hemopump). Indications were graft failure (two patients), postinfarction ventricular septal defect (two patients), myocarditis (1 patient), and myocardial infarction (1 patient). The 21F cannula, inserted via the groin, was used as a partial assist in four patients, and the 31F cannula, inserted via the ascending aorta, was used to assist the other two patients completely. Hemodynamic recovery was achieved in all patients (mean cardiac index rose from 2.1 L/gm/m2 to 3.85 L/gm/m2 after 1 hour assist
