Ultrastructure of chondrocytes in osteoarthritic femoral articular cartilage.

BACKGROUND: Osteoarthritis (OA) is a common problem in elderly, but it is not an inevitable feature of ageing. About 80-90% of individuals of both sexes have radiographic evidence of OA by the time they reach an age of 65. But not all of them have the symptoms like pain and decreased joint motion. OBJECTIVE: The objective of the present study was conducted to find out whether the osteoarthritic changes in human articular cartilage are similar to the ageing process or not. METHODS: Femoral articular cartilage specimens obtained from 13 osteoarthritic patients (52-80 years) undergoing total kn…

Cartilage oligomeric matrix protein specific antibodies are pathogenic.

INTRODUCTION: Cartilage oligomeric matrix protein (COMP) is a major non-collagenous component of cartilage. Earlier, we developed a new mouse model for rheumatoid arthritis using COMP. This study was undertaken to investigate the epitope specificity and immunopathogenicity of COMP-specific monoclonal antibodies (mAbs). METHODS: B cell immunodominant regions on the COMP molecule were measured with a novel enzyme-linked immunosorbent assay using mammalian expressed full-length mouse COMP as well as a panel of recombinant mouse COMP fragments. 18 mAbs specific to COMP were generated and the path…

Novel mutation in boy with cartilage-hair hypoplasia.

BACKGROUND: Cartilage-hair hypoplasia (MIM 250250) is an autosomal recessive disease with diverse clinical manifestations. The clinical phenotypes include variable degrees of bone and hair dysplasia, deficient cellular and/or humoral immunity, and a predisposition to malignancy. METHODS: We performed genetic studies of a patient with disproportionate short stature and brittle scalp hair. Genetic studies were also carried out in the patient's parents. RESULTS: A novel maternal mutation that consisted of a duplication of 14 nucleotides at position -13 of the RNA component of the RNA component …

Alcama mediates Edn1 signaling during zebrafish cartilage morphogenesis.

The zebrafish pharyngeal cartilage is derived from the pharyngeal apparatus, a vertebrate-specific structure derived from all three germ layers. Developmental aberrations of the pharyngeal apparatus lead to birth defects such as Treacher-Collins and DiGeorge syndromes. While interactions between endoderm and neural crest (NC) are known to be important for cartilage formation, the full complement of molecular players involved and their roles remain to be elucidated. Activated leukocyte cell adhesion molecule a (alcama), a member of the immunoglobulin (Ig) superfamily, is among the prominent mar…

Autologous cartilage injection for the patulous eustachian tube.

The eustachian tube (ET) is normally closed, but it opens temporarily during swallowing. Patients with a patulous ET (PET) have various aural symptoms such as aural fullness, autophony, and hearing their own breathing. These symptoms are caused by abnormal transmission of sound from the pharynx to the middle ear via an open ET with little attenuation. We introduce a novel injection technique for the treatment of PET using autologous cartilage. This procedure is minimally invasive and has been successfully used to treat PET in 2 patients.

Cartilage cell proliferation in degenerative TFCC wrist lesions.

INTRODUCTION: The central zone of the triangular fibrocartilage complex (TFCC) of the wrist is thought to be avascular and is generally considered to lack any healing potential. AIM: The purpose of this study was to investigate, if cartilage cells of degenerative disc lesions possess any healing or proliferation potential and whether ulna length plays a significant role in the proliferation process. RESULTS: Cells positive for proliferating cell nuclear antigen (PCNA) were found in all specimens. Specimens of patients with ulna positive variance showed a decreased number of PCNA positive cel…

Repair of articular cartilage defects: review and perspectives.

Articular cartilage defects heal poorly and lead to catastrophic degenerative arthritis. Clinical experience has indicated that no existing medication substantially promotes the healing process and the cartilage defect requires surgical replacement, preferably with an autograft. However, there is a shortage of articular cartilage that can be donated for autografting. A review of previous unsuccessful experiences reveals the reason for the current strategy to graft cartilage defects with regenerated cartilage. Autologous cartilage regeneration is a cell-based therapy in which autogenous chondro…

Lateral compartment cartilage changes and lateral elbow pain.

The aim of our study is to document the arthroscopic findings in resistant lateral elbow pain. We have reviewed the findings in a consecutive series of 117 elbow arthroscopies performed on patients with lateral elbow pain resistant to conservative treatment. We found established degenerative changes involving articular cartilage in 68 patients (59%). In 60 of these 68 patients (88%) the degenerative changes were confined to the lateral compartment and contrasted with a normal appearance of the articular cartilage of the medial compartment. Primary lateral compartment arthritis is more common t…

Capitellar cartilage injuries concomitant with radial head fractures.

PURPOSE: To evaluate the incidence and types of capitellar cartilage injuries associated with higher-grade radial head fractures. METHODS: Fifty-one consecutive patients with operatively treated, unstable, displaced Mason type II to III radial head fractures were identified. Ten of 51 patients had capitellar cartilage injuries concomitant with these fractures. All cartilage injuries were identified at the time of surgery except in 1 patient whose injury was determined via computed tomography. There were 8 men and 2 women with an average age of 33 years (range, 24-39 years). Lesions were seen …

[Fracture of thyroid cartilage after a sneezing episode].

We present a case report of a thyroid fracture after a sneezing episode, with odynophagia, dysphonia, and neck pain. The examination showed oedema at the right vocal cord and haematoma at the right false vocal cord. An anterior thyroid fracture without displacement, and a subcutaneous emphysema could be seen on the CT. Thyroid fracture because of this aetiology is most exceptional; only 1 similar case report has been described in the literature.

[Influence of thyroid cartilage ossification in laryngeal ultrasound].

BACKGROUND: Topological features of the larynx and hypopharynx may be depicted during sonographic examination of the neck, although ultrasound is seen of minor diagnostic importance. The varying extend of thyroid cartilage ossification appears to be the major limiting factor. We intended to assess the sonographic accessability of laryngeal structures with high end resolution ultrasound devices. METHODS AND PATIENTS: We examined 101 patients endoscopically and by ultrasound. Normal laryngeal findings and pathologies were examined and analysed considering limitations by thyroid cartilage ossifi…

[Correction of ear malformations with autologous rib cartilage].

INTRODUCTION: Ear malformations are mainly isolated deformations but exist also in combination with various syndromes. A visible malformation in cases of microtia is a problem for the entire family. Auricular reconstruction with autologous cartilage has been well established in the last decade. Optimization of the technique has led to improved and reliable results. OPERATION AND RESULTS: Based on the experience of 120 cases we developed a concept that allows total ear reconstruction in two operative steps. In the first operation autologous rib cartilage is harvested and a natural framework is…

Ex vivo gene therapy approaches to cartilage repair.

Degeneration of articular cartilage is one of the great clinical challenges that still lack a convincing therapeutic solution. Traumatically induced lesions and final stages of osteoarthritis with substantial loss of cartilage tissue require the generation of new hyaline cartilage, because significant endogenous repair does not occur. Current joint-preserving surgical approaches, however, mostly lead to fibrous repair tissue that is rapidly degraded. In experimental studies, several differentiation factors have been shown to stimulate chondrogenesis, promoting the formation of functionally acc…

The heritable determinants of cartilage oligomeric matrix protein.

OBJECTIVE: Cartilage oligomeric matrix protein (COMP) is a cartilage matrix macromolecule. The protein is detectable in serum and has been investigated as a biomarker of osteoarthritis (OA). An association between COMP and OA has been shown, yet the precise factors governing serum levels of COMP remain unclear. The aim of this study was to determine whether genetic factors influence serum levels of COMP. METHODS: A classic twin study was conducted using COMP levels in serum obtained from healthy female twin volunteers. COMP levels were determined by an inhibition enzyme-linked immunosorbent a…

Rolled auricular cartilage grafts for dorsal augmentation rhinoplasty.

We present a technique, which has not been previously described in the literature, of using rolled autogenous conchal cartilage for dorsal augmentation via an endonasal or external rhinoplasty approach. This technique gives greater dorsal height compared with the more common layering techniques. It is most appropriate in the minimally or moderately saddled nose.

Brown tumor of the thyroid cartilage: CT findings.

Brown tumor of the larynx is extremely rare. We describe a patient with long-standing primary hyperparathyroidism and severe skeletal involvement associated with brown tumors of the axial and appendicular skeleton and of the thyroid cartilage. Ossification of the laryngeal skeleton may explain the presence of this process in this unusual location.
