Cognitive Impairment and Dementia in Primary Care: Current Knowledge and Future Directions Based on Findings From a Large Cross-Sectional Study in Crete, Greece.

Introduction: Dementia severely affects the quality of life of patients and their caregivers; however, it is often not adequately addressed in the context of a primary care consultation, especially in patients with multi-morbidity. Study Population and Methods: A cross-sectional study was conducted between March-2013 and December-2014 among 3,140 consecutive patients aged >60 years visiting 14 primary health care practices in Crete, Greece. The Mini-Mental-State-Examination [MMSE] was used to measure cognitive status using the conventional 24-point cut-off. Participants who scored low on MMSE

Strategies and innovations in hypertension management for sickle cell patients: A narrative review.

Sickle cell disease (SCD) remains a challenging hematologic disorder, characterized by chronic hemolysis, vaso-occlusive events, and multi-organ complications. Hypertension, a prevalent comorbidity in SCD, poses significant clinical implications, exacerbating the complexities of disease management and impacting patient outcomes. Understanding the intricate interplay between SCD and hypertension is pivotal. Mechanistic insights uncover a landscape characterized by chronic hemolysis, endothelial dysfunction, altered nitric oxide bioavailability, and increased oxidative stress, contributing to el

Ayurveda in Contemporary Clinical Practice: Integrating Traditional Principles With Preventive, Metabolic, and Mind-Body Medicine.

The increasing burden of chronic, metabolic, and stress-related disorders has created a need for clinical models that emphasize prevention, lifestyle modification, individualized care, and long-term risk reduction. Ayurveda offers a structured framework for diet, behavior, metabolic regulation, constitutional assessment, and mind-body balance; however, its integration into contemporary clinical practice remains constrained by fragmented evidence, heterogeneous interventions, and variable methodological quality. This review examines the relevance of Ayurveda in preventive, metabolic, and mind-b

The model of care at a leading medical cannabis clinic in Canada.

Medical cannabis access has been legalized in more than 30 countries worldwide and popularity among patients is increasing rapidly. Cannabinoid-based treatments have been shown to be beneficial for several symptoms such as chemotherapy-induced nausea and vomiting, spasticity, chronic pain, intractable seizures and insomnia, yet high-quality clinical trials are still limited. As millions of patients now have legal access to medical cannabis, little information is available about the development of best clinical practices and an effective medical cannabis clinic model. A medical cannabis clinic

Evaluation of the "safe multidisciplinary app-assisted remote patient-self-testing (SMART) model" for warfarin home management during the COVID-19 pandemic: study protocol of a multi-center randomized controlled trial.

BACKGROUND: Warfarin treatment requires frequent monitoring of INR (international normalized ratio) to adjust dosage in a therapeutic range. In China, patients living in small towns usually go to tertiary hospitals to get warfarin monitoring and dosing, resulting in low frequencies of follow-ups and high incidence of complications. Influenced by the COVID-19 pandemic, patients on warfarin have further reduced their visits to healthcare institutions. While patient self-testing (PST) via using a point-of-care testing device for INR measuring at home has been widely used in developed countries an

Development of planning of the integrated care for older people in China: a theory of change approach.

BACKGROUND: Integrated Care for Older People (ICOPE), developed by the World Health Organization (WHO) with a global perspective, faces varying degrees of barriers to implementation, particularly in middle-and low-income countries. Therefore, as with other new public service interventions, we draw on established integrated care interventions to design a Theory of Change (ToC) model for ICOPE, and to propose methods and pathways for adapting community-based integrated care models for older people (OP) to specific contexts, thereby updating and enhancing the implementation of ICOPE. METHODS: An

Remote Patient Monitoring in Guam: A Pathway to Implementation.

This case study examines the feasibility of implementing Remote Patient Monitoring (RPM) in Guam to improve chronic disease management and expand access to care in a Medically Underserved Area. While challenges exist, Guam's strategic role as a communication hub for Micronesia, supportive telehealth policies, and an island-wide broadband expansion initiative create favorable conditions for RPM adoption. Drawing on an initial feasibility trial conducted between a Nurse Practitioner in the contiguous United States and a Registered Nurse in Guam, the authors propose a four-phase implementation pa

Individual Heterogeneity in the Probability of Hospitalization, Skilled Nursing Facility Admission, and Mortality.

BACKGROUND: Multimorbidity is common in adults aged 65 and older and is associated with health care utilization and mortality, but most methods ignore the interrelationship among concurrent outcome nor provide person-specific probabilities. METHOD: A longitudinal cohort of 5300 older Americans from the 2011-2015 rounds of the National Health and Aging Study was linked to Center for Medicare and Medicaid Services claims. Odds ratios for 15 chronic conditions adjusted for sociodemographic factors were estimated using a joint model of hospitalization, skilled nursing facility (SNF) admission, an

Self-care behaviors among Iranian adults with non-alcoholic fatty liver disease based on the health belief model and structural equation modeling.

Non-alcoholic fatty liver disease (NAFLD) is increasingly recognized as a widespread chronic liver condition globally, with a prevalence of 33% in Iran (35% in males and 37% in females). Understanding health-related behaviors in NAFLD patients is crucial for designing effective interventions. This study is novel in applying the Health Belief Model (HBM) and Structural Equation Modeling (SEM) to examine psychological determinants correlated with self-care behaviors among Iranian adults with NAFLD. A cross-sectional study was conducted with 513 NAFLD patients at the Internal Medicine Clinic of S

Association of Opioid Use Disorder Diagnosis With Use of Physical Therapy and Chiropractic Care Among Chronic Low Back Pain Patients: A Group-Based Trajectory Analysis.

Nonpharmacologic approaches are recommended as first-line treatment for chronic pain, and their importance is heightened among individuals with co-occurring opioid use disorder (OUD), in whom opioid therapies may be particularly detrimental. Our objectives were to assess the receipt and trajectories of nonpharmacologic pain treatment and determine the association of OUD diagnosis with these trajectories. This retrospective cohort study used Medicare claims data from 2016 to 2018 and applied group-based trajectory models to identify distinct patterns of physical therapy (PT) or chiropractic car

Costs associated with erythropoiesis-stimulating agent administration to hemodialysis patients.

BACKGROUND: Treatment of anemia in hemodialysis patients usually requires the use of expensive erythropoietic proteins. Cost analyses usually focus on drug acquisition costs. Other costs associated with anemia therapy include resources for anemia monitoring as well as preparation and administration of an erythropoiesis-stimulating agent. METHODS: The nonacquisition costs associated with subcutaneous administration of epoetin alfa were determined in a Canadian hemodialysis unit. A time-and-motion technique was used to determine the nursing time for preparation and administration. Fixed anemia

The Delivery of Kidney Care in Rural or Sparsely Populated Settings.

Social determinants of health and limited access to medical care contribute to underdiagnosis and late presentation of chronic kidney disease (CKD). Rural areas face unique challenges in providing optimal care for patients with CKD due to poverty, limited health care access, and geographical barriers. This review addresses the challenges of caring for rural patients with CKD and discuss potential strategies to increase health care equity. Furthermore, we examine rural disparities in patients with CKD, patients with kidney failure on in-center or home dialysis, and patients with kidney transpla
