OBJECTIVE: Chylothorax complicates the postoperative course of patients after congenital heart surgery. Innominate vein thrombosis and stenosis have been associated with postoperative chylothorax. Revascularization and angioplasty can be accomplished using transcatheter techniques. We report our experience with this procedure for the management of postoperative chylothorax.
DESIGN: This is a retrospective case series of patients who underwent catheter revascularization and/or angioplasty of the innominate vein following cardiac surgery at our institution from January 1, 2008 through April 9, …

BACKGROUND: Chylothorax after surgery for congenital heart disease is rare. We wanted to compare the different presentations of chylothorax in patients who received median sternotomy or lateral thoracotomy.
PATIENTS AND METHODS: We retrospectively studied pediatric patients with congenital heart disease who received palliative or corrective surgeries and developed postoperative chylothorax between January 1992 and July 2003. Patients were divided into two groups by the type of surgery: median sternotomy and lateral thoracotomy. The average daily fluid amount (mL/kg/24 hours), latency period, …

Postoperative chylothorax is an uncommon complication after lung cancer resection. However, chylous leakage caused by iatrogenic injury of aberrant mediastinal lymphatic vessels often presents massive drainage and fails conservative treatment, requiring reoperation. A retrospective analysis was performed on three patients who underwent reoperation for refractory massive chylothorax after radical resection of right upper lobe lung cancer between October 2022 and November 2023 in our center. Intraoperatively, chylous fistulas were consistently detected at the junction of the upper margin of the …

BACKGROUND Congenital chylothorax is a rare condition characterized by the accumulation of lymph in the pleural space. It is the most common cause of neonatal pleural effusion, with low incidence and male predominance. When it is complicated by other conditions, diagnosis and management become challenging. This report aims to present a rare case of bilateral congenital chylothorax with a complicated clinical course, highlighting the associated diagnostic and management challenges. CASE REPORT We report a male Kosovar neonate presenting on the third day of life with respiratory distress requiri…

The objective of this study, which is part of a retrospective case series, was to report on single-session, minimally invasive thoracic duct ligation (TDL), cisterna chyli ablation (CCA), and partial pericardiectomy (PPE) for dogs with chylothorax. A total of 9 client-owned dogs with chylothorax participated in the study. A right-sided or bilateral thoracoscopic and left-flank laparoscopic approach was carried out in sternal recumbency for TDL and CCA, respectively. Subsequent partial pericardiectomy was done thoracoscopically via a paraxiphoidal approach. Combined (TDL, CCA, and PPE), minimal…

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma and often presents with an aggressive clinical course. We report the case of an 84-year-old woman with dementia who presented with a rapidly growing mediastinal mass. Imaging revealed compression of the superior vena cava - an oncological emergency requiring immediate intervention - along with a large pleural effusion, which was confirmed to be a chylothorax, a rare complication of lymphoma. Radiotherapy was started on the day of admission, alongside corticosteroid therapy. A biopsy confirmed peripheral B-…

Chylothorax is a severe complication following the Fontan procedure, causing significant morbidity and mortality due to nutritional depletion and fluid loss. We present a case involving a six-year-old girl with tricuspid atresia, atrial septal defect (ASD), ventricular septal defect (VSD), and severe pulmonary stenosis (PS), presenting with fever, non-productive cough, and increased work of breathing. Cyanosis was noted, improving with oxygen. Imaging revealed bilateral pleural effusion, with pleural fluid analysis confirming chylothorax. Despite normal laboratory reports, retrograde transveno…

Chylothorax is a rare complication after double lung transplantation. We report a case of a 55-year-old man with idiopathic pulmonary fibrosis. He underwent a double lung transplantation with venoarterial extracorporeal membrane support. The surgery was uncomplicated; however, his postoperative course was complicated with a refractory chylothorax that started postoperative day 4. Medical management could not control the chylothorax, including nil per os, total parenteral nutrition, and octreotide administration. After failed percutaneous embolization via lymphangiography and surgical ligation …

A 70-year-old man was admitted to our hospital with a thoracic aortic aneurysm. He underwent elective thoracic endovascular aneurysm repair with left common carotid artery-left subclavian artery bypass via the left supraclavicular approach. During the bypass, the thoracic duct was injured and ligated. On postoperative day 3, the patient complained of dyspnea. Chest radiography revealed a massive right-sided pleural effusion. On postoperative day 5, he was diagnosed with right-sided chylothorax and underwent chest tube insertion. The next day, a left-sided chylothorax was noted, and chest tube …

Postoperative chylothorax after wedge resection without mediastinal lymph node dissection is extremely rare. Herein, we report two cases: one case of postoperative chylothorax that developed after wedge resection and another of expanded collaterals detected during thoracoscopic surgery. We believe the cause of this complication to be the history of neck and pulmonary ligament dissections during pulmonary surgery.

Chylothorax is characterized by the accumulation of chyle in the pleural space, of which the most common cause is trauma or neoplasm. Although chylothorax accounts for a small proportion of clinical pleural effusions, prompt recognition is needed to avoid malnutrition, immunodeficiency, and fibrothorax. We report 2 patients with superior vena cava obstruction caused by tunneled venous catheters resulting in chylothorax and demonstrate the potential safety of tunneled pleural catheters for prolonged chylothorax drainage in an outpatient setting with rigorous follow-up. Changes in pleural fluid …

We report the case of a 61-year-old female with advanced gastric cancer and mediastinal lymph node metastasis who developed chylothorax. The patient presented with cough, dyspnea and pain in the left lower limb, back, and hips. Her lower limb symptoms were attributed to cellulitis. Computed tomography revealed right-sided pleural effusion, multiple lymph node swelling, and thickening of the gastric wall. Following pleural aspiration, the effusion was identified to be chyle. Cytopathologically, numerous adenocarcinoma cells were detected in clumps, and subsequent esophagogastroduodenoscopy reve…

A neonate, who had undergone primary repair of esophageal atresia with tracheo-esophageal fistula, developed right pleural effusion in the postoperative period. It was initially misdiagnosed as an anastomotic leak, but later confirmed to be chylothorax. Conservative treatment failed. Application of biological fibrin glue (sealant) on the mediastinum through a thoracotomy was curative.

This retrospective study was carried out at eight Neonatal Intensive Care Units (NICU) Centers worldwide on 33 newborns presenting at birth with pleural, pericardial, or abdominal chylous effusions. Diagnosis of chylous effusion is based on findings of fluid with a milk-like appearance, a concentration of triglycerides in pleural effusion >1.1 mmol/l, and a total cell count >1,000 cells/ml with a predominance of >80% lymphocytes. Thirty-three newborns met the inclusion criteria and were studied. Six subjects who presented at birth with fetal effusion were treated by in-utero pleuro-amniotic sh

BACKGROUND: Chylothorax after esophagectomy remains a devastating postoperative complication. Prophylactic mass ligation of the thoracic duct during the extensive resection of esophageal cancer is used to prevent the chylothorax, but there are few published data about its effect. This randomized controlled study was designed to evaluate the preventive effect of thoracic duct mass ligation on postoperative chylothorax.
METHODS: Six hundred fifty-three patients undergoing transthoracic esophagectomy for cancer were randomly assigned to two groups: 328 patients received esophagectomy (preservati…

Lymphorrhea is an uncommon but potentially life-threatening complication following surgical procedures which strongly influences the outcome of surgical patients. Persistent lymphocutaneous fistula is mainly associated with an increased risk of local infections and wound complications. Chylous ascites and chylothorax can result in malnutrition, metabolic deterioration and immunosuppression that increase postsurgical morbidity. Diagnosis can easily be made by the characteristic milky-beige appearance of chylous fluid. Laboratory investigation of the drainage fluid (chylomicrons, triglycerides) …

A 52-year-old man with chronic myeloid leukaemia (CML) who had been receiving dasatinib 100 mg/day for 13 years presented with progressive dyspnea. Chest radiography revealed bilateral pleural effusions, and further evaluation confirmed the diagnosis of chylothorax. Dasatinib-induced chylothorax was suspected, and discontinuation of the drug led to clinical improvement. Dasatinib is a tyrosine kinase inhibitor (TKI) used for the treatment of CML and Philadelphia chromosome-positive acute lymphoblastic leukaemia (Ph + ALL). Although pleural effusion is a relatively common adverse effect, chylot…
