Clinical context and communication in shared decision-making about major surgery: Findings from a qualitative study with colorectal, orthopaedic and cardiac patients.

Increasing numbers of older people undergo major surgery in the United Kingdom (UK), with many at high risk of complications due to age, co-morbidities or frailty. This article reports on a study of such patients and their clinicians engaged in shared decision-making. Shared decision-making is a collaborative approach that seeks to value and centre patients' preferences, potentially addressing asymmetries of knowledge and power between clinicians and patients by countering medical authority with greater patient empowerment. We studied shared decision-making practices in the context of major su

Innovative AI models for clinical decision-making: predicting blastocyst formation and quality from time-lapse embryo images up to embryonic day 3.

Accurate embryo assessment on embryonic day 3 of assisted reproductive technology (ART) is crucial for deciding whether to continue the culture until day 5 (blastocyst stage) or opt for earlier transfer or cryopreservation. Prolonged culture often improves pregnancy outcomes in patients with multiple high-quality embryos, but may offer limited benefits for older patients or those with few available embryos. In Japan, where donor eggs are rarely used, cleavage-stage vitrification remains common in poor-prognosis cases, making early embryo assessment clinically relevant. To address this clinical

The Impact of Patient-Generated Health Data From Mobile Health Technologies on Health Care Management and Clinical Decision-Making: Narrative Scoping Review.

BACKGROUND: Long-term health conditions and multimorbidity are increasing globally, placing an unsustainable pressure on health care systems. Mobile health (mHealth) technologies enable the collection of patient-generated health data outside clinical settings, offering the potential to support personalized care and inform clinical decision-making. However, the ways in which mHealth patient data are being used in clinical practice remain unclear. OBJECTIVE: This study aimed to map and synthesize the existing literature on how patient-generated mHealth data are reportedly being used and influen…

Sentinel lymph node biopsy status improves adjuvant therapy decision-making in patients with clinical stage IIB/C melanoma: A population-based analysis.

BACKGROUND: Given the results of the recent KEYNOTE-716 trial, the performance of sentinel lymph node (SLN) biopsy for patients with clinical stage IIB/C melanoma has been questioned. OBJECTIVE: Determine the utility of SLN status in guiding the recommendations for adjuvant therapy. METHODS: Patients with clinical stage IIB/C cutaneous melanoma who underwent wide local excision and SLN biopsy between 2004 and 2011 were identified from the Surveillance, Epidemiology, and End Results database. Two prognostic models, with and without SLN status, were developed predicting risk of melanoma-specif

Exploring motivations and resistances for implementing shared decision-making in clinical practice: A systematic review based on a structure-process-outcome model.

OBJECTIVE: Shared decision-making (SDM) as a multicollaborative approach is vital for facilitating patient-centred care. Considering the limited clinical practice, we attempted to synthesize the motivations and resistances, and investigate their mutual relationships for advancing the implementation of SDM. METHODS: A comprehensive systematic review using Preferred Reporting Items for Systematic Review and Meta-Analysis guidelines was performed. 'Shared decision making' was searched as the mesh term through PubMed, Web of Science and EBSCO from 2000 to 2021, and the quality of literature was a

Rationale for therapeutic decision-making in locally advanced and metastatic radioactive iodine (RAI)-refractory differentiated thyroid cancer, starting from a clinical case.

Iodine uptake and organification are the hallmarks of thyroid cells differentiation. The loss of these characteristics in thyroid cancer leads to radioactive iodine refractoriness, a rare condition that bears a low survival rate and poor prognosis. We present a 52-year-old patient presenting dry cough and dyspnea in the supine position. Imaging examinations revealed a thyroid nodule with a high suspicion of malignancy in the right thyroid lobe, multiple laterocervical and mediastinal lymph nodes, lung, bone, and brain metastases. Fine needle aspiration cytologic features have advocated for pap

Predictors of treatment in acute coronary syndromes in the elderly: impact on decision making and clinical outcome after interventional versus conservative treatment.

BACKGROUND: Management of elderly patients with acute coronary syndromes (ACS) is not standardized and physicians discretion for invasive versus conservative strategies lacks sufficient evidence. The purpose of this analysis was, therefore, to evaluate treatment strategies and outcomes of elderly patients with ACS and to highlight reasons for the treatment decision in a consecutive series of elderly patients. METHODS: This is a retrospective analysis of 1,001 elderly patients (>75 years) presenting with ACS. Patients were identified on the basis of their final discharge diagnosis. Baseline da

A rare case of double orifice mitral valve with perimembranous ventricular septal defect: Application of three-dimensional echocardiography for clinical decision making.

Double orifice mitral valve (DOMV) is an uncommon anomaly of surgical importance characterized by a mitral valve with a single fibrous annulus with two orifices opening into the left ventricle (LV). Subvalvular structures, especially the tensor apparatus, invariably show various degrees of abnormality. Associated congenital heart defects are common, though DOMV can occur as an isolated anomaly. Two-dimensional echocardiography is useful for diagnosis but combining it with real-time three-dimensional echocardiography helps in a more detailed evaluation of mitral valve and subvalvular structures

Impact of professional experience and practice setting on clinical decision-making and anxiety in peri-implantitis management: a cross-sectional survey study.

BACKGROUND: Peri-implantitis management is clinically demanding and may be accompanied by professional anxiety; however, the extent to experience level and practice setting shape treatment decisions and anxiety has not been fully clarified. Previous surveys have described clinicians' knowledge and attitudes toward peri-implantitis, yet evidence integrating therapeutic preferences with clinician anxiety remains scarce. This study assessed Turkish periodontists' awareness and treatment priorities for peri-implant diseases and examined the influence of professional experience, postgraduate exposu

From Translation Milestones to Clinical Breakthroughs: Decision-Making Hubs and Translational Strategies for RNA-Binding Proteins in the Immunotherapy of Haematological Malignancies.

Immunotherapy for haematological malignancies-including CAR-T and immune checkpoint inhibitors-is limited by resistance driven by rapid adaptive changes at the tumour-immune interface that transcriptional regulation cannot address. This review introduces 'translational checkpoints' as a synthesized working framework for overcoming resistance by targeting RNA-binding proteins (RBPs) that govern rapid protein synthesis in tumour and immune cells. We address three core questions: how tumours evade immune surveillance, how immune dysfunction arises, and how to develop targeted interventions. We co
