High rates of reoperation following operative fixation of open patella fractures.

PURPOSE: Patella fractures are common and may result in significant morbidity. Open patellar fractures are less common, and there are few investigations into their outcomes and complications. This study aimed to determine acute outcomes after operative treatment of open versus closed patella fractures. METHODS: This study was a retrospective cohort study from a single academic Level One tertiary trauma center. Patients who underwent open reduction internal fixation of open and closed patella fractures with a minimum of 6 months of follow-up from 2013 to 2018 were included. Those who underwent

Management of subtrochanteric fractures by nail osteosynthesis: a review of tips and tricks.

INTRODUCTION: Subtrochanteric fractures of femur remain most challenging injuries faced by orthopaedic surgeons. The implant choice shifted from extra medullary to intramedullary nails over the last two decades. Effort to achieve anatomical reduction before nail insertion for better outcomes has been emphasized well in literature. We reviewed various surgical techniques for reduction of fracture in more biological way that can be used to overcome the deforming forces of the muscles acting on fracture fragments. CONCLUSION: Regardless of the closed vs open reduction for subtrochanteric fractur

Assessment of Bosworth-type fracture by external oblique radiographs.

INTRODUCTION: Bosworth described an unusual fracture dislocation of the ankle with fixed posterior fracture dislocation of the fibula. This ankle fracture variant is often not recognized in initial radiographs and requires a computed tomographic scan for verification. It is usually not reducible by the closed method, and repeated trials induce more damage. The purpose of this study was to verify the usefulness of simple external oblique radiographs for diagnosis of Bosworth-type fracture. METHODS: We reviewed the 327 patients who were diagnosed as unilateral malleolus ankle fracture in 2002 t

Irreducible ankle fracture-dislocation due to tibialis anterior subluxation: a case report.

Irreducible ankle fracture-dislocations are rare. Several cases of irreducible ankle fracture-dislocation have been reported in published studies secondary to the tibialis posterior tendon, deltoid ligament, or extensor digitorum longus tendon blocking the reduction. We report a case of irreducible ankle fracture-dislocation resulting from posteromedial subluxation of the tibialis anterior tendon around a medial malleolar fracture fragment. Ultimately, the ankle required open reduction of the incarcerated tendon to reduce the joint and proceed with internal fixation of the associated fracture.

The Butterfly Fragment in Wedge-Shaped Femoral Shaft Fracture: Comparison of Two Different Surgical Methods.

OBJECTIVE: Our study compared the results of wedge-shaped femoral shaft fracture following intramedullary (IM) nailing with or without fixation of the third fragment. METHODS: We retrospectively reviewed patients presenting with femoral shaft fracture with AO/OTA type 32-B from 2011 to 2016. Patients were divided into two groups: closed reduction without touching the third fragment and open reduction with fixation of the third fragment. The fragment ratio, fragment length, nail size, dynamization or not, mRUST scores, union rate, and union time were compared between the two groups. Risk facto

A comparison of ketamine sedation and general anaesthesia for manipulation of paediatric forearm fractures.

The purpose of the study was to compare the use of ketamine sedation and general anaesthesia for manipulation of paediatric wrist and forearm fractures. A retrospective analysis was performed of patients under 16 years treated at our centre between October 2014 and October 2015. Exclusion criteria were open fractures and fractures with complete displacement. Outcomes measured were fracture reduction, the quality of the cast, fracture redisplacement, further surgical intervention and use of theatre time. 66 children were manipulated over the study period; 31 received ketamine sedation and 35 ge

The Reverse Oblique Supracondylar Humerus Fracture: Description of a Novel Fracture Pattern.

BACKGROUND: Sagittal profile descriptions of supracondylar humerus fractures are limited. We describe a reverse oblique (RO) pattern in which the distal fragment has a prominent anterior spike that is displaced posterior to the proximal fragment. This pattern presents a challenge during closed manipulation utilizing traditional reduction maneuvers. The purpose of this study is to compare the clinical and radiographic characteristics of the RO and non-RO patterns of supracondylar humerus fractures. We hypothesized that the RO pattern would be associated with greater soft tissue trauma. METHODS

Sternoclavicular joint injuries: a literature review.

Traumatic sternoclavicular (SC) joint injuries are rare. The goal of this literature review was to evaluate the available literature on the treatment of traumatic anterior and posterior SC (fracture-) dislocations in adults and children. In adults, anterior dislocations are most commonly treated by nonoperative means. Closed reduction can be attempted but is associated with a high recurrence rate. For posterior dislocations, closed reduction is usually more successful. Children usually present with displaced physeal fractures, as true dislocations probably do not occur in a skeletally immature

Open Treatment of Closed Talus Dislocation without Fracture: A Case Report and Literature Review.

INTRODUCTION: Talus dislocation without a concomitant fracture of the malleoli is a rare injury. CASE REPORT: We present a case of a closed anteromedial talus dislocation in a 25-year-old male motorcyclist due to a high-energy trauma, resulting from a fall following a road traffic accident. The talus could not be reduced by closed means making open reduction performed through a dorsal approach necessary. At 6-month follow-up, the patient has little pain and the range of ankle motion was satisfactory with some signs of bone contusion. CONCLUSIONS: Talus dislocation is a rare injury that occur

Intra-Articular Dislocation of the Patella With Associated Hoffa Fracture in a Skeletally Immature Patient.

Since 1887, approximately 50 cases of an intra-articular patellar dislocation have been reported in the worldwide literature. The vast majority of patients required an open reduction of the patella or closed reduction under general anesthesia. This injury has never been reported in association with a coronal shear fracture of the femoral condyle. A 14-year-old boy presented to our institution with his left knee locked in flexion after a direct blow. Radiographs showed the patella rotated on its horizontal axis and lying in a transverse position within the knee joint, as well as a concomitant f

Anterior fracture-dislocations of the shoulder: pitfalls in treatment.

Fracture-dislocations of the shoulder can present difficulties in treatment. Two cases are described to illustrate the complications that can occur when an inexperienced examiner attempts a closed reduction of a two-part anterior fracture-dislocation of the shoulder. Careful clinical and radiologic evaluation (A-P and transcapular or axillary lateral X-rays) followed by appropriate management can help to prevent these problems from occurring. If there is evidence that the fracture is displaced by manipulation, immediate open reduction and internal fixation are recommended.

Treatment of acute lunate and perilunate dislocations.

In 55 wrists with dorsal perilunate dislocations, trans-scaphoid dorsal perilunate fracture dislocations and volar lunate dislocations, an anatomic reduction was achieved by closed manipulation in 67% of the cases. After anatomic reduction, 59% of the wrists lost anatomic position during the first six weeks of treatment despite adequate external immobilization. Closed treatment alone was successful in achieving and maintaining an anatomic reduction only 27% of the time. Open treatment was successful in gaining anatomic reduction 75% of the time and was uniformly successful in maintaining the r

[Minimal invasive treatment of massively dislocated radial neck fractures in children by percutaneous joystick reposition and Prevot nailing].

Undisplaced fractures of the proximal radius can generally be treated conservatively with good results. In children, spontaneous correction of some angular deformities can be expected during growth. Nevertheless, more severely displaced fracture types may require reduction in children, too. Open reduction and internal fracture fixation have shown to lead to a loss in range of motion frequently. A technique of percutaneous fracture reduction is demonstrated that can provide good results in cases when closed techniques have failed. A Kirschner wire is used to manipulate the fracture fragments pe

[Treatment of femoral shaft fractures with expandable intramedullary nail].

BACKGROUND: Femoral shaft fractures are usually seen in the young population as a result of high energy traumas and are often accompanied by major organ injuries. In this paper, we aimed to assess the clinical results of expandable femoral intramedullary nails in the treatment of 20 femoral shaft fractures. METHODS: The average age was 34.7. One fracture was the result of a gunshot wound, type 3A open fracture, and the other 19 fractures were closed. Under fluoroscopic control, all patients underwent elective closed reduction and internal fixation with intramedullary expandable femoral nails

Fracture-separation of the distal humeral epiphysis. Long-term follow-up of five cases.

Fracture-separation of the distal humeral epiphysis is a rare injury described in young children. The diagnosis is often difficult because most of the ossification centre are not visible. We report a long-term follow-up study of five cases, evaluated after an average of 38 years. At the time of fracture, all the patients were more than 3 years old. Three patients had been treated with overhead traction, closed reduction and immobilisation in a long-arm plaster cast, one patient with closed reduction and immobilisation and another one, initially not diagnosed, had been treated after 2 weeks wit

Primary exploration for radial nerve palsy associated with unstable closed humeral shaft fracture.

BACKGROUND: The treatment of radial nerve palsy caused by closed humeral shaft fracture is a matter of debate. The purpose of this study is to evaluate the outcome of early surgical management of radial nerve palsy in patients with unstable closed humeral shaft fractures and to determine whether patients with this injury should be surgically explored. METHODS: Medical records of 15 consecutive patients who had undergone open reduction and internal fixation for unstable closed humeral shaft fractures were reviewed. Injury mechanisms, radiographs, intraoperative findings of the injured nerve, a

Elastic Intramedullary Nailing of Pediatric Both-Bone Forearm Fractures.

BACKGROUND: Elastic intramedullary nailing of both-bone pediatric forearm fractures is used to treat unstable fractures that fail conservative management with closed reduction and casting, open injuries, or injuries with neurovascular compromise. DESCRIPTION: Small incisions are used to enter the medullary canal of the distal end of the radius and proximal part of the ulna, avoiding injury to the adjacent physis. The elastic nail is advanced to the fracture site under fluoroscopic guidance. The fracture is reduced with traction and manipulation, and the nail is passed across the fracture site
