Conventional versus accelerated collagen cross-linking for keratoconus.

: Collagen cross-linking (CXL) is a procedure that primarily aims to increase corneal stiffness. Although used for a variety of conditions, it is most commonly applied to the treatment of keratoconus. Collagen cross-linking involves irradiation of the cornea with ultraviolet A (UVA) irradiation after it has been soaked with riboflavin (vitamin B), a photosensitizer. In conventional treatment, based on the Dresden protocol, a minimum corneal thickness threshold of 400 μm is recommended and UVA (370 nm) irradiation of 3 mW/cm irradiance is applied for 30 min, resulting in a cumulative dose of 5.…

Microbial keratitis following accelerated corneal collagen cross-linking.

A deep stromal infiltrate with hypopyon appeared in central cornea of right eye of a 15-year-old boy postoperatively after 2 days, who underwent uneventful accelerated corneal collagen crosslinking (C3R) with riboflavin and ultraviolet-A (UVA) for the treatment of keratoconus. Staphylococcus aureus keratitis was confirmed by the microbiological studies, which guided intense treatment with topical and systemic antibiotics. Before C3R, the best corrected visual acuity (BCVA) in the ocular dexter was 20/30 with the refraction of - 1.00 DS/-5.00 DC × 30° with drop to 20/400 following the infection…

Collagen microstructural factors influencing optic nerve head biomechanics.

PURPOSE: Previous studies have suggested that the lamina cribrosa (LC) and its surrounding sclera are biomechanically important in the pathogenesis of glaucoma, but many were limited by assumptions of tissue isotropy and homogeneity. Here, we used an improved biomechanical model driven by experimental measurements of scleral and LC collagen fiber organization to more accurately evaluate optic nerve head (ONH) biomechanics. METHODS: Collagen fiber organization was quantitatively mapped across human ONH cryosections (three normal and three glaucomatous) using small-angle light scattering (SALS)…

Topography-based keratoconus progression after corneal collagen crosslinking.

PURPOSE: The aim of this study was to report 2 cases with topographic keratoconus progression despite stability for a long-term period after corneal collagen crosslinking (CXL). METHODS: This is a case report. RESULTS: Two patients with progressive keratoconus underwent CXL treatment. Four and 5 years postoperatively, a topographic examination revealed an increase in the keratometric values indicating keratoconus progression. CONCLUSIONS: Post-CXL, topographic keratoconus progression may occur several years after, despite stability for a long-term period.

Newer protocols and future in collagen cross-linking.

Corneal Cross-Linking (CXL) is an established surgical procedure for the treatment of corneal disorders such as corneal ectasia and keratoconus. This method of treatment stabilises the corneal structure and increases rigidity, reducing the requirement for corneal transplantation. Since its development, many scientific studies have been conducted to investigate ways of improving the procedure. Biomechanical stability of the cornea after exposure to UV-A light, and the effect of shortening procedure time has been some of the many topics explored.

Computational analysis of bovine alpha-1 collagen sequences.

Bovine collagen alpha-1 is a naturally occurring extracellular matrix protein found in tendons and other connective tissues. It plays a vital role in cell growth, differentiation, attachment, and migration. Recent findings have established that collagen alpha-1 is involved in osteogenesis imperfecta phenotype in cattle but deep information about other members of this large family is not available so far. So with a view to finding a new edge and attempt to figure out a correlation among the well attributed Bovine alpha-1 collagen sequences are executed and analyzed. To do so, comparative analys…

Serum levels of tenascin-C in collagen diseases.

Tenascins are a family of large multimetric extracellular matrix (ECM) proteins. Among them, large molecular weight variant tenascin-C is known to be specifically expressed in pathological conditions. However, no link between tenascin-C and collagen diseases has been established. The aim of our study was to determine the serum tenascin-C levels in patients with various collagen diseases, and to evaluate the possibility that serum levels of tenascin-C can be a useful marker for collagen diseases, correlating with the pathogenesis. Serum tenascin-C levels of 33 patients with scleroderma (SSc), 1…

[Bile duct reconstruction using 3-dimensional collagen tubes].

INTRODUCTION: In recent years, with widespread laparoscopic cholecystectomy and liver transplantation, complications involving the biliary system are increasing. All current techniques have a high risk of recurrence or high-morbidity. MATERIAL AND METHODS: A 3-dimensional collagen bile duct modified with agarose hydrogel was developed to substitute the affected extrahepatic bile duct. It was used in 40 guinea pigs and the histology and physiology was studied at 4 weeks, 3 and 6 months after transplantation. CONCLUSIONS: The graft shows to have a high potential in applications to treat hepato…

Cervical collagen concentration within 15 months after delivery.

OBJECTIVE: Cervical collagen concentration decreases during pregnancy. The increased risk of preterm birth after a short interpregnancy interval may be explained by an incomplete remodeling of the cervix. The objective of this study was to describe the changes in cervical collagen concentration over 15 months after delivery. STUDY DESIGN: The collagen concentrations were determined in cervical biopsy specimens that were obtained from 15 women at 3, 6, 9, 12, and 15 months after delivery. RESULTS: The mean cervical collagen concentrations were 50%, 59%, 63%, 65%, and 65% of dry weight (SD, 4.…

Clinical phenotypes associated with type II collagen mutations.

COL2A1 mutations give rise to a spectrum of phenotypes predominantly affecting cartilage and bone from the severe disorders that are perinat … clues. The underlying hypothesis is that there are novel phenotypes caused by mutations in type II collagen that extend from premature arthritis through to more severe bone dysplasias. The importance of finding a COL2A1 mutation lies in the subsequent ability to accurately assess recurrence risks and offer information regarding disease natural history. Mo …

Free-floating collagen fibers in interstitial mycosis fungoides.

We present a case of interstitial mycosis fungoides showing pseudodovascular clefts with "free-floating" collagen fibers surrounded by neoplastic T lymphocytes. Such a finding further expands the histopathologic spectrum of mycosis fungoides and could be taken into account in its differential diagnosis from granuloma annulare, inflammatory morphea, and interstitial granulomatous drug reaction.

Polymerized type I collagen reduces chronic cyclosporine nephrotoxicity.

BACKGROUND: Polymerized type I collagen (P-collagen) has been successfully used to reduce human hypertrophic scars due to its anti-fibrotic and anti-inflammatory properties. We therefore carried out a study to determine if P-collagen reduces functional and structural injury in chronic cyclosporine [cyclosporine A (CsA)] nephropathy. METHODS: Four groups of six male Wistar rats fed with a low sodium diet were treated with vehicle, P-collagen (0.8 mg/day, i.p.), CsA (15 mg/kg) or CsA + P-collagen for 15 days. Mean arterial pressure, renal blood flow and glomerular filtration rate were measured …
