Bridging policy and practice: Determinants of place of death for palliative care recipients in Taiwan.

PURPOSE: Given the impact of place of death (PoD) on end-of-life quality and policy planning, this study examined the distribution and determinants of PoD among older adults receiving palliative care in Taiwan, with a focus on service models and referral-to-death intervals. METHODS: This population-based study analyzed linked data from the Health and Welfare Data Science Center in Taiwan for 177,403 decedents aged ≥ 65 years who received palliative care and died between 2010 and 2020. Multivariable logistic regression was employed to examine associations between PoD and individual characteris

Diagnosis of Parkinson's Disease Based on Voice and Speech Using Machine Learning and Deep Learning: A Systematic Umbrella Review.

OBJECTIVES: Early diagnosis of Parkinson's disease (PD) is complicated. Speech impairment, as an early symptom of PD, offers a noninvasive, scalable biomarker for remote assessment. Speech-based machine learning has shown promise, but methodological quality of existing evidence remains unclear. This review examines the previous reviews on the performance of machine learning models, their strengths and weaknesses, and future research opportunities in diagnosing PD using speech. METHODS: We conducted this umbrella review in accordance with PRISMA guidelines, systematically searching PubMed, Sco

Fertility planning for women in medicine: a call for an active approach.

BACKGROUND: Women in medicine often delay childbearing due to the competing priorities of training, finding a partner, and waiting for the "right time." Unfortunately, the steep decline in female fertility during a woman's mid to late 30s also often coincides with the completion of residency and/or fellowship training. Data show that women in medicine elect to have children later in life and that at least 1 in 4 female physicians suffer from infertility, a higher rate than in nonphysicians. It is essential to escalate the dialogue regarding family planning for female physicians and trainees.

The Natural History Study and Biomarker Collection of the Clinical Research Consortium for the Study of Cerebellar Ataxia (CRC-SCA).

Hereditary ataxias are progressive neurodegenerative disorders primarily affecting the cerebellum. Since 2009, the Clinical Research Consortium for the Study of Cerebellar Ataxias (CRC-SCA) has studied the natural history of common types of spinocerebellar ataxias (SCAs). The CRC-SCA is a 17-site academic collaboration supported by the National Ataxia Foundation. In 2024, the CRC-SCA expanded its scope by incorporating newly identified late-onset ataxias, including repeat expansion mutations in RFC1 and FGF14 causing Cerebellar Ataxia with Neuropathy and Vestibular Areflexia Syndrome (CANVAS)

Midwifery Practices for the Conversion of Foetus With Breech Presentation: A Qualitative Study.

BACKGROUND: Although the conversion of the foetus with breech presentation to cephalic presentation is well-supported in the literature in terms of its benefits for maternal and neonatal health, there is insufficient knowledge regarding the specific midwifery practices involved in this process. The purpose of this study is to identify the methods used by midwives to convert fetuses with breech presentation to cephalic presentation and how they feel about performing these methods. METHODS: The study employs a descriptive exploratory qualitative design. Individual in-depth interviews were condu

Update on Tumor Surveillance for Children with Hereditary Pheochromocytoma/Paraganglioma Syndromes.

Hereditary pheochromocytoma/paraganglioma syndromes (HPPS) are a collection of conditions caused by variants in genes producing subunits of the succinate dehydrogenase (SDH) complex or related proteins. These conditions are characterized by substantial lifetime risks for developing pheochromocytomas, paragangliomas, and other tumors. Affected individuals who develop these tumors may experience severe, acute, and chronic problems. Indeed, aggressive, malignant, and/or disseminated tumors may result in death. Tumor surveillance enables early intervention, which, in turn, should lead to improved

Developmental dynamics of the single nucleus regulatory landscape of pig hippocampus.

The hippocampus is a brain region associated with memory, learning and spatial navigation, its aging-related dysfunction is a common sign of Alzheimer's disease. Pig is a good model for human neurodegenerative disease, but our understanding of the regulatory program of the pig hippocampus and its cross-species conservation in humans remains limited. Here, we profiled chromatin accessibility in 33,409 high-quality nuclei and gene expression in 8,122 high-quality nuclei of the pig hippocampus at four postnatal stages. We identified 510,908 accessible chromatin regions (ACRs) in 12 major cell typ

Gaps in Ambulatory Patient Safety for Immunosuppressive Specialty Medications.

OBJECTIVES: New specialty drugs such as biologics are now available in record numbers, presenting increased safety risks for people with immune-mediated diseases. However, comprehensive assessments of patient safety for these drugs are lacking. We examined performance on key patient safety measures, such as screening for latent tuberculosis (LTBI), hepatitis B virus (HBV), and hepatitis C virus (HCV), for new users of a broad group of specialty medications. METHODS: Data were extracted via electronic health record data warehouses of a large university health system using structured queries, a

Inflammation and its discontents: the role of cytokines in the pathophysiology of major depression.

Recognition that inflammation may represent a common mechanism of disease has been extended to include neuropsychiatric disorders including major depression. Patients with major depression have been found to exhibit increased peripheral blood inflammatory biomarkers, including inflammatory cytokines, which have been shown to access the brain and interact with virtually every pathophysiologic domain known to be involved in depression, including neurotransmitter metabolism, neuroendocrine function, and neural plasticity. Indeed, activation of inflammatory pathways within the brain is believed to

Nucleotide sequence of the rat skeletal muscle actin gene.

The actins constitute a family of highly conserved proteins found in all eukaryotic cells. Their conservation through a very wide range of taxonomic groups and the existence of tissue-specific isoforms make the actin genes very interesting for the study of the evolution of genes and their controlling elements. On the basis of amino acid sequence data, at least six different mammalian actins have been identified (skeletal muscle, cardiac muscle, two smooth muscle actins and the cytoplasmic beta- and gamma-actins). Rat spleen DNA digested by the EcoRI restriction enzyme contains at least 12 diff

Diagnosis of Pulmonary Embolism in Pregnancy.

Pulmonary embolism (PE) is a significant cause of obstetric morbidity and mortality. However, overdiagnosis related to excessive use of diagnostic testing is also associated with long-term major health issues, including impact on future pregnancies and subsequent health care. Accurate diagnosis of PE depends on the knowledge of prevalence of PE in the pregnant population, the a priori probability of a PE based on specific findings in a given patient, and understanding of the accuracy of computed tomography pulmonary angiography (CTPA), the dominant diagnostic modality employed for this diagnos

Attitudes, Beliefs, and Values of Italian Osteopaths Toward the Management of Patients With Tension-Type Headache: A Categorical Content Analysis.

OBJECTIVE: Tension-type headache (TTH) is among the 20 leading causes of disability worldwide. Due to the difficulty in diagnosis and the variable association of other symptoms, the management of this clinical condition is often unsatisfactory. Osteopathic manipulative treatment combined with exercise, diet, and general relaxation therapies may be valid for TTH. This study investigated osteopaths' common attitudes, beliefs, and values when treating and managing individuals experiencing TTH. METHODS: This is a qualitative study using thematic analysis with elements of grounded theory. Osteopat

The conect4children (c4c) Consortium: Potential for Improving European Clinical Research into Medicines for Children.

The need for information about new and existing drugs used in children was recognized in the European Union (EU) with the implementation of the Paediatric Regulation in 2007. In 2017, the 10-year review of the Paediatric Regulation identified barriers to the conduct of clinical trials, including delays in setting up and completing paediatric trials. Across Europe, the difficulties with clinical research are compounded by variation within countries and between countries. Ethics and regulatory review have national specificities. This paper describes the Collaborative Network for European Clinica

Tenodesis Grasp Detection in Egocentric Video.

OBJECTIVE: Cervical spinal cord injury (cSCI) can impair motor function in the upper limbs. Video from wearable cameras (egocentric video) has the potential to provide monitoring of rehabilitation outcomes at home, but methods for automated analysis of this data are needed. Wrist flexion and extension are essential elements to track grasping strategies after cSCI, as they may reflect the use of the tenodesis grasp, a common compensatory strategy. However, there is no established method to evaluate wrist flexion and extension from egocentric video. METHODS: We propose a machine-learning-based

Extrapolation of Adult Efficacy to Pediatric Patients With Chemotherapy-Induced Nausea and Vomiting.

Chemotherapy-induced nausea and vomiting (CINV) is a common treatment-related adverse event that negatively impacts the quality of life of cancer patients. During pediatric drug development, extrapolation of efficacy from adult to pediatric populations is a pathway that can minimize the exposure of children to unnecessary clinical trials, improve efficiency, and increase the likelihood of success in obtaining a pediatric indication. The acceptability of the use of extrapolation depends on a series of evidence-based assumptions regarding the similarity of disease, response to intervention, and
