[Convulsions in neonates. Clinical, electrographic, etiopathogenic and prognostic aspects].

121 cases of neonatal convulsions, including 79 cases of status epilepticus and 42 cases of isolated convulsions are studied. The great number of premature infants (40/121 newborns born before a gestational age of 37 weeks) must be pointed out. The clinical symptoms and EEG activity occurring during the first 24 hours following the onset of convulsions are analysed. Their descriptions confirm previous ones. Comparison of the populations of preterm and term infants demonstrates that the pre- and post-ictal neurological states are equally severe in both groups and that there is no difference bet

A controlled trial of pyridoxine supplementation in children with febrile convulsions.

A total of 107 children who had been hospitalized following a febrile convulsion were enrolled into the trial. By random allocation, 55 children were treated with pyridoxine hydrochloride (20 mgs twice daily) and the remaining 52 children were treated with a placebo until there had been either a further convulsion or a year had passed without recurrence. Eighty children were adequately followed up and of these, 17 had a recurrent febrile convulsion while receiving medication. Recurrences occurrences occurred in 7 of the 38 children receiving pyridoxine and in 10 of the 42 children receiving pl

[Diagnostic significance of sleep EEGs in children with febrile convulsions].

The sleep EEGs of 52 children suffering from febrile seizures were recorded. Their waking EEGs were free of epileptic activity. Only a few patients fell asleep without medication, the majority received 2 mg/kg body weight of promazine hydrochloride. Epileptic activity occurred in the sleep EEG of 15 out of 52 patients (29%). Exclusively short generalized paroxysms of high amplitude, mostly irregular and partially abortive 3-4/sec spike-wave complexes and occasional 2.5/sec sharp-slow-wave complexes were recorded. Epileptic discharges were detected in the B stage in 11 patients, in the C stage

Febrile convulsions. What happens to the infant admitted to the hospital.

We present our experience with children with febrile convulsions admitted to a large teaching hospital. Elective admission on a routine basis increases the tendency for unnecessary additional tests. Hospital admission should be reserved for children with severe multiple febrile seizures, or the underlying serious disease or where parental anxiety and other social circumstances indicate. Workups should be done only in selected cases consistent with clinical findings. The yield of such workup is marginal, even in a hospitalized child with multiple febrile seizures.

Continuous phenobarbital treatment after a 'simple febril convulsion'.

In only a small proportion of young children with brief, generalized, febrile convulsions do afebrile seizures develop, but this fraction is several times the prevalence of epilepsy in an unselected population. The risk of another febrile convulsion is approximately 30%. Febrile status epilepticus during a subsequent infection is a potential source of serious morbidity and mortality. Intermittent phenobarbital administration during subsequent, febrile illnesses confers little protection against recurrent, febrile convulsions. Continuous phenobarbital administration during the preschool years i

Primary immunisation and febrile convulsions in Oxford 1972-5.

A three-year study of febrile convulsions in Oxford with comprehensive notification from general practice and hospitals showed a 3% risk for all children of suffering at least one febrile convulsion by the age of 5 years. Children were most at risk between 6 and 27 months, and febrile convulsions were most likely to be prolonged in children aged 9-15 months. The association between febrile convulsions and primary immunisations in the preceding 28 days was compared in case and control children, matched for age and sex. Results suggested that such association was a chance relationship with age.

Continuous phrophylcatic anticonvulsants in selected childrens with febrile convulsions.

The recurrence rate of febrile convulsions is assessed in 108 children who had had either prolonged or repeated initial fits or had both a positive family history and persisting neurological disorder. Only 7 of 40 (17%) who received continuous prophylactic phenobarbitone or primidone for at minimum of 18 months had another fit compared with 40 of 68 (59%) children who were untreated (p less than 0.0005). Indications for continuous therapy are discussed.
