Acute Type A Aortic Dissection Causing Collapse of Previous Stent Graft.

A 69-year-old man presented to the emergency department with back pain and paraplegia. Computed tomography showed acute type A aortic dissection with a primary tear right below the origin of the left subclavian artery. He had undergone endovascular aneurysm repair 13 months earlier, and the previously inserted endovascular graft had totally collapsed. Total arch replacement covering the primary entry tear was performed. Femoral pulse was undetected before the operation but was monitored postsurgically after cardiopulmonary bypass was weaned. The patient was discharged from the hospital on foot

Extended total arch replacement via the L-incision approach: single-stage repair for extensive aneurysms of the aortic arch.

OBJECTIVES: Total arch replacement via the L-incision approach (a combination of left anterior thoracotomy and upper median sternotomy) can be used to achieve more extensive replacement. METHODS: In the period between 2002 and 2014, 279 total arch replacement procedures were performed. After excluding cases of acute aortic dissection and cases involving concomitant, hybrid or frozen elephant trunk procedures, patients who underwent isolated total arch replacement via an L-incision (n = 29) and via median sternotomy (n = 143) were identified and the data pertaining to their cases were analysed

Systematic review of outcomes of combined proximal stent grafting with distal bare stenting for management of aortic dissection.

OBJECTIVE: Available data on outcomes of combined proximal stent grafting with distal bare stenting for management of aortic dissection are limited. This is a systematic review of outcomes of this approach. METHODS: Studies involving combined proximal stent grafting with distal bare stenting for management of aortic dissection were systematically searched and reviewed. RESULTS: A total of 4 studies were included, with 108 patients treated for acute (n = 54) and chronic (n = 54) aortic dissection. Technical success rate was 95.3% (range, 84-100). The 30-day mortality was 2.7% (range, 0%-5%).

[Aortic root replacement for aortic dissection and aortic regurgitation due to aortitis syndrome].

Aortic aneurysms and aortic regurgitation (AR) with aortitis syndrome are occasionally reported in young women. We report a case of aortic dissection with severe AR in an 8-year-old girl. The patient underwent aortic root replacement with a composite graft. Pathological report revealed aortitis syndrome and steroid therapy was continued to suppress further inflammatory vascular reaction.

Treatment of delayed retrograde dissection after endovascular stenting of thoracoabdominal aortic aneurysm: graft-to-endograft anastomosis.

INTRODUCTION: Endovascular treatment of thoracic aortic aneurysms may be associated with procedure-related early and late complications. REPORT: We describe the case of a 70-year-old man who developed acute retrograde dissection of the ascending aorta 30 days after endovascular stent grafting of a thoracoabdominal aortic aneurysm and prior revascularization of the left subclavian artery. The ascending aorta and aortic arch were successfully replaced with a Dacron graft; the distal part of the graft was partially anastomosed to the previously placed endograft. DISCUSSION: Surgical repair of d

The modified Bentall procedure for aortic root replacement.

The Bentall procedure is a surgical repair of an ascending aortic or aortic root aneurysm in combination with aortic valve disease. Less commonly, it is used to repair aortic dissection affecting the aortic root and valve. During the procedure, a composite aortic valve graft is used to replace the proximal ascending aorta and aortic valve. The procedure is performed through a median sternotomy during cardiopulmonary bypass. In this modification of the original procedure, coronary artery circulation is maintained by removing a full-thickness "button" of aorta surrounding the coronary ostia, mak

[Total arch graft implantation with open stent-graft placement for aortic arch aneurysm or dissection].

We have developed and routinely performed the total arch graft implantation with open stent-graft placement for aortic arch aneurysm or dissection. This method is easy and respiratory-troubleless method, because of ascending and arch replacement with 4 branched graft and descending graft implantation with stent-graft from opened aortic arch. This method is useful for the patient with aortic arch diseases concomitant with coronary artery bypass candidate, respiratory dysfunction and aortic dissection.

[Lethal aortic dissection in a 13-year-old boy with a vascular Ehlers-Danlos syndrome].

UNLABELLED: Vascular Ehlers-Danlos syndrome(EDS) is at high risk of death by arterial rupture. CASE REPORT: A 13-year-old boy with vascular EDS and aortic dissection was admitted in pediatric emergency care unit. The children died after surgery by massive hemorrhage and prosthetic rupture. DISCUSSION: The precocity of this vascular accident EDS is uncommon. It occurred despite clinical and echocardiographic follow-up. The death of the children confirmed the difficulties in surgery of vascular manifestation, even if appropriate and rapid management was already done. CONCLUSION: Patients with

Chronic aortic dissection complicated by disseminated intravascular coagulation: successful treatment with endovascular stent-grafting.

PURPOSE: To report endovascular repair of a chronic aortic dissection complicated by disseminated intravascular coagulation (DIC). CASE REPORT: A 61-year-old man developed DIC associated with a chronic Stanford type B aortic dissection that occurred during cardiac catheterization 12 years earlier. At the current admission, computed tomography showed a partially thrombosed false lumen extending from the aortic arch to the left common iliac artery. On angiography, entry and re-entry tears were identified at the right subclavian and left common iliac arteries, respectively. After stent-graft imp

Total and subtotal aortic replacement for extensive aortic dissection in patients with or without Marfan's syndrome.

OBJECTIVES: Extensive aortic dissection with multiple entries often found in Marfan's syndrome patients ultimately requires replacement of the whole aorta. We present a surgical strategy and results for total and subtotal aortic replacement. METHODS: Subjects were 18 patients, including 14 Marfan's patients undergoing total (n = 13) or subtotal (excluding aortic arch) aortic replacement (n = 5), for DeBakey type I aortic dissection (n = 13) and DeBakey type IIIb aortic dissection with annuloaortic ectasia (n = 5) between February 1991 and April 2001. Mean age was 39.9 +/- 0.8 years--34.9 +/-

[Nearly total aortic replacement for DeBakey type-I aortic dissection in a patient with Marfan's syndrome].

A successful nearly total aortic replacement for DeBakey type-I aortic dissection in a patient with Marfan's syndrome is reported. A 36-year-old woman developed a DeBakey type-I aortic dissection in October 1987. We replaced the ascending aorta and resuspended the aortic valve urgently, when she had not been diagnosed with Marfan's syndrome. In August 1989, we replaced the distal portion of the descending aorta and the infra-renal abdominal aorta because of dilatation in the diameter of these regions. In September 1990, she had sudden severe chest pain. Computed tomography and Doppler ultrason

Laparoscopic attenuation of a congenital extrahepatic portosystemic shunt in a dog-a thin-film banding for splenophrenic shunt: A case report.

A 6-year-old castrated male Shih-Tzu dog weighing 6. 5 kg presented with chief complaints of pollakiuria and urine dribbling. He had a history of urolithiasis for 3 years, which was confirmed by the presence of ammonium urate in the urinary stone analysis, performed 2 years prior to the presentation. Blood examination showed high values of fasting ammonia, post-prandial bile acid, and low blood urea nitrogen. Microhepatica and urolithiasis were identified on plain radiography and ultrasonography. A computed tomography angiography demonstrated a shunting vessel, diameter up to 9.6 mm, originate

Lip Reconstruction by Double Layer and Double Flap New Combination Technique: A Case Series.

In the past, lip reconstruction after ablative surgery has been performed by primary closure and more recently by free flap transfer technique. Cheek's skin flap has been used to reconstruct the lower lip cutaneous portion. This study presents a reconstructive method for the vermillion and the lip's cutaneous portion using the Goldstein-Robotti techniques (for the vermillion) and the buccinator flap to reconstruct the cutaneous lip portion and the perioral muscles. This procedure allows a complete reconstruction with a double layer technique for defects of more than one-third of both lips, tog

Transperineal Coccygeal Muscle-Guided Endoscopic Approach for Early Distal Control of the Internal Iliac Vein in High-Complexity Pelvic Exenteration: Technical Tricks and Initial Experiences.

BACKGROUND: High-complexity pelvic exenteration requires extensive lateral dissection and often en bloc resection of the internal iliac vessels to secure margins. However, achieving safe, precise access to the distal pelvic sidewall through a confined operative field is technically demanding and hazardous. IMPACT OF INNOVATION: We developed a transperineal coccygeal muscle-guided endoscopic approach primarily to establish a reliable, anatomically guided pathway to the lateral pelvic compartments. Using the coccygeal muscle as a landmark facilitates precise identification of the distal interna
