The introduction of transesophageal echocardiography (TEE) as a 'semi-invasive' technique with few complications in clinical practice represents a major diagnostic advance in the evaluation of cardiac disease. The close anatomic vicinity of the transducer to the heart and thoracic aorta allows the use of high frequency devices with better resolution compared to transthoracic echocardiography. Furthermore, the diagnostic gain when evaluating structures that are poorly visualized by the transthoracic approach such as the left atrial appendage, interatrial septum, prosthetic valves and the thorac

Acute aortic dissection (AAD) is an important emergency that should be identified promptly. The classification of AAD follows two different systems: Stanford (which defines lesions as types A, on the ascending aorta, or B, on the descending aorta) and DeBakey, which also accounts for the extension of the aortic dissection. We present a notable case of a 63-year-old male who presented with a history of abrupt abdominal pain radiating to the dorsal region for endovascular treatment. He was oliguric with symmetric pulses in the superior limbs and reduction of pulses in the left lower limbs, with

Reoperations on the proximal thoracic aorta are increasingly observed after previous aortic or cardiac operations. Redo proximal aortic surgery remains challenging with an increased mortality compared to first-time operations. For a successful redo proximal aortic surgery in a patient with complex pathological conditions, the surgical procedure and cardiopulmonary bypass (CPB) should be simplified as much as possible. Herein, we report our experience of proximal aortic reoperations in which the strategy consisted of an axillo-axillary (jugular) and a femoro-femoral CPB in combination with mini

BACKGROUND: We evaluated the clinical and anatomic presentations of acute type A aortic dissection according to patient age.
METHODS: We retrospectively reviewed 235 consecutive patients who underwent acute type A dissection repair between January 2000 and December 2014. The influence of age on anatomical and clinical presentation, surgical management in the entire cohort and also after exclusion of patients with known connective tissue disorders was assessed using logistic regression.
RESULTS: Males presented with type A acute aortic dissection at a younger age than females. Acute onset wit

Acute aortic dissection (AAD) is a fatal cardio-vascular emergency initiated by an intimal breach in the aorta, leading to the delamination of its wall layers and the development of true and false lumina. Typically, Stanford Type B dissections are amenable to medical management unless complications develop. This report describes a 45-year-old hypertensive male who presented with acute, severe, constricting chest pain. Physical assessment identified a notable inter-arm blood pressure discrepancy. Emergency Computed Tomography Angiography (CTA) confirmed an acute Stanford Type B AAD, originating

A successful case of two-staged operation for Stanford type B acute dissecting aneurysm complicated with total occlusion of the distal abdominal aorta was reported. A 62-year-old male patient with a long history of systemic hypertension developed acute severe chest, back and bilateral legs pain. An enhanced CT demonstrated Stanford type B dissecting aneurysm with occlusion of the left renal artery and bilateral common iliac arteries. On the day of admission, an emergent right axillo-femoral bypass operation using 8 mm ringed EPTFE graft was undertaken to rescue the ischemic legs. The patient's

Thoracoabdominal impalement injuries involving the aorta represent an extremely rare but highly life-threatening trauma scenario. We report the case of a 50-year-old construction worker who sustained an impalement injury after falling approximately 8m onto an upright standing wooden hammer handle, resulting in thoracoabdominal penetration. The injury resulted in multiple trauma, including damage to the lungs, diaphragm, spleen and kidneys as well as a contained dissection of the thoracic aorta. The patient was treated within the framework of an interdisciplinary, time-critical management appro

BACKGROUND: In patients with Stanford type B dissection who have been treated successfully with medical hypotensive therapy during the acute phase, a large number have incurred the risk of surgery during their chronic phases because of enlargement of the dissected aorta. The purpose of this study was to determine the indications for surgical treatment of acute type B dissection by studying chronic-phase enlargements of aortic dissections in patients treated successfully with medical hypotensive therapy during the acute phase.
METHODS AND RESULTS: In 41 patients with type B dissection who had

Persistent false lumen in chronic type B aortic dissection complicates endovascular repair. The hot wire technique offers a precise method for septal fenestration, enabling endovascular treatment when standard approaches are not feasible. We present a case of a 65-year-old man with prior ascending aorta replacement and elephant trunk, who developed a large descending aortic aneurysm with residual dissection and a false lumen in which the elephant trunk resided, prohibiting conventional thoracic endovascular aortic repair. Septal fenestration allowing continuous flow was created using the hot w

Dissection morphology is important when considering landing zones for thoracic endovascular aortic repair. Landing proximally in a healthy aorta, free of dissection and intramural hematoma, is generally recommended. For the most common location of the proximal entry tear near the left subclavian artery, this generally requires landing in zone 2. However, zone 2 landing is associated with higher rates of stroke and need for left subclavian revascularization. Distal landing zone 5 is associated with much higher rates of spinal cord ischemia, without significant benefit over zone 4 or bare metal

A 75-year-old woman was diagnosed with type B acute aortic dissection 14 years ago and 3-channeled aortic dissection 7 years ago. She received total arch replacement 6 years ago and descending aortic replacement with double barrel anastomosis technique for distal anastomosis 5 years ago. Computed tomography( CT) revealed giant thyroid tumor and thoracoabdominal aortic aneurysm( 58 mm in diameter). She suffered from back pain during her follow-up period. CT revealed ruptured thoracoabdominal aortic aneurysm. First, the false lumen of descending aorta was closed by thoracic endovascular aortic r

We report the case of a 70-year-old man who presented with a large dissection of the ascending aorta related to coronary artery bypass grafting 3 years earlier. One graft was patent; however, a reverse venous graft to the right coronary artery was occluded, and that artery was substantially stenotic. The patient was a high-risk surgical candidate, so we treated a large dissection successfully by endovascular means with a thoracic endograft. This case suggests that this approach can be applied safely and effectively to other high-surgical-risk patients who have complex ascending aortic disease.

A 63-year-old Japanese man with no significant medical or family history was admitted for sudden consciousness disturbance, left hemiparesis and shock. Physical examination showed internal carotid artery asymmetry on palpation and significant differential blood pressure in both upper arms. A thoracic CT scan showed classic aortic dissection from the ascending aorta through the common iliac artery, classic dissection of the left common carotid artery and an intramural haematoma in the right common carotid artery. The patient's consciousness level gradually improved, and he underwent emergency s

PURPOSE: To describe the use of a hybrid frozen elephant trunk technique to treat a patient suffering from a multisegmentally diseased, dextropositioned aortic arch and right descending aorta.
TECHNIQUE: The technique is illustrated in a 58-year-old woman who was diagnosed with chronic type A aortic dissection in a right aortic arch and descending aorta; a lusoric left subclavian artery passed behind the esophagus. Aneurysmal dilatation of the arch and descending aorta required treatment. A frozen elephant trunk procedure was performed in a single stage via a median sternotomy using a hybrid

Autosomal dominant polycystic kidney disease is one of the most common hereditary diseases, and frequently has well defined extrarenal manifestations. Very few cases of aortic aneurysms associated with this disorder are described in literature. We report a 42-year-old male with autosomal dominant polycystic kidney disease presenting with dissecting aneurysm of the thoracic aorta.

BACKGROUND: Stanford type B aortic dissection is one of the aortic catastrophes with a high mortality and morbidity that needs immediate or delayed treatment, either surgically or endovascularly. This comprehensive review article addresses the current status of open, endovascular and hybrid treatment options for type B aortic dissections with the focus on new therapeutic perspectives.
METHODS: Evaluation of currently available evidence based on randomized and registry data and personal experience.
RESULTS: All type B dissections require prompt medical treatment to prevent aortic rupture. Acu

Thoracic aortic aneurysm (TAA) is characterized by dilation of the aorta that can lead to dissection or rupture. Degradation of elastic fibers is a consistent histopathological feature of TAA that likely contributes to disease progression. Pentagalloyl glucose (PGG) shows promise for stabilizing elastic fibers in abdominal aortic aneurysms, but its efficacy and mechanical effects in the thoracic aorta are unknown. We simulated TAAs using elastase (ELA) to degrade elastic fibers in the mouse ascending aorta and determined the preventative and restorative potential of PGG. Biaxial mechanical tes

Endovascular repair has evolved to become a viable mainstream treatment for aortic pathology in both acute and elective settings. As technology advanced, traditional anatomical barriers were progressively tackled using new devices and novel procedures, and there are now multiple options available to the vascular surgeon. In the abdominal aorta, advances in endovascular aneurysm repair have been in the treatment of hostile aortic necks using new sealing concepts and ancillary procedures, and in branch preservation using fenestrations and snorkels. Access challenges have been met with a percutan

BACKGROUND: Individuals with Loeys-Dietz syndrome 3 are at risk for thoracic aortic dissection even if their aortas are not substantially dilated. A pathogenic variant of SMAD3 is a genetic trigger, but the cellular programs that underlie the heightened aortic risk remain poorly understood.
METHODS: We ascertained the cardiovascular profile of 57 individuals in a 4-generation family with a missense variant in SMAD3 (p.Arg287Gln). Aortic medial architectures were quantified from aortas harvested during thoracic aortic replacement surgery, with 2-dimensional and 3-dimensional profiling of the e

OBJECTIVE: The objective of our study was to report our hospital and long-term results after reinterventions on the thoracic aorta.
METHODS: Between 1986 and 2011, 224 reoperations on the proximal thoracic aorta after previous aortic surgery were performed in our institution. The number of reinterventions quadrupled during the course of the study period. Mean patient age was 58.1 years, and 174 patients (77.7%) were male. An urgent/emergency operation was performed in 39 patients (17.4%). Indications for surgery included degenerative and chronic postdissection aneurysm (n = 166), false aneury
