Comorbidity of psychiatric and alcohol/drug disorders: interactions and independent status.

Dual diagnosis as interpreted in clinical psychiatric practice is often not what the term means-two independent disorders. Commonly, addiction disorders are attributed to self medicating of "underlying" psychiatric disorders, there by, a contingency status. However, studies and clinical addiction practice show that drug and alcohol addiction must be afforded an independent status before accurate assessment and assignment of interactions in dual diagnosis can be made in clinical conditions. Studies also show rates for psychiatric comorbidity are low in addiction populations and rates for addict

The kinetics of drug-membrane interactions in human erythrocytes from neonates.

The kinetics of drug-membrane interactions of erythrocytes from neonates were compared with those from adults by monitoring the time course of the shape transformations and vesicle release caused by drugs, using a light microscope--video recording technique. Both crenation caused by lysophosphatidylcholine (LPC) and cupping caused by chlorpromazine (CPZ) took place more slowly in the neonatal cells than in the cells from adults. The equilibrium concentrations of LPC and CPZ in erythrocytes did not differ significantly between the neonates and adults, however. The slower responses of the neonat

Drug interactions with ethanol. Effects on body temperature and motor impairment.

Single doses of d-amphetamine, chlorpheniramine or diazepam were combined with ethanol under two conditions: (i) in drug-naive mice and (ii) in mice which had been given a single dose of ethanol 72 hr previously. Ethanol was administered orally at doses of 6.0, 3.0 or 1.5 g/kg; doses of d-amphetamine, chlorpheniramine or diazepam were given intraperitoneally. Three parameters were measured; changes in rectal temperature, forced motor coordination, as evaluated by rotarod performance and concentrations of ethanol in blood. d-Amphetamine and chlorpheniramine attenuated the hypothermia induced by

Multi-feature machine learning for enhanced drug-drug interaction prediction.

Drug-drug interactions are a major concern in healthcare, as concurrent drug use can cause severe adverse effects. Existing machine learning methods often neglect data imbalance and DDI directionality, limiting clinical reliability. To overcome these issues, we employed GPT-4o Large Language Model to convert free-text DDI descriptions into structured triplets for directionality analysis and applied SMOTE to alleviate class imbalance. Using four key drug features (molecular fingerprints, enzymes, pathways, targets), our Deep Neural Networks (DNN) achieved 88.9% accuracy and showed an average AU…

Adverse Drug-Drug Interaction Between Phenobarbital and Fluconazole in Two Dogs.

Phenobarbital (PB) is an antiseizure medication widely used in dogs that is metabolized by hepatic cytochrome P450 (CYP) enzymes. Fluconazole, a commonly prescribed antifungal medication, inhibits several CYP isoenzymes and can impair PB metabolism. Genetic polymorphisms such as the CYP2C41 gene deletion can alter CYP activity and influence drug interactions, although not well characterized in dogs. We describe two epileptic dogs on chronic PB treatment that developed marked sedation and ataxia, and increased serum PB concentrations after receiving fluconazole. Both dogs were homozygous for th…

Pharmacokinetic and pharmacodynamic drug-drug interaction of Nomilin with atorvastatin in hyperlipidemic mice.

Atorvastatin (Atv) is widely used to lower cholesterol levels and treat hyperlipidemia in clinical application. Nomilin (Nom) is a kind of limonoids, which is found and isolated from the citrus herbs of Rutaceae family, which are widely used as patent medicines, functional foods, and nutritional supplements in many countries. In previous studies, Nom has the effect of anti-obesity and curing other metabolic diseases. Nevertheless, in recent years, the drug-drug interaction (DDI) caused by the administration of drugs with synergistic effects have raised worldwide concerns. To investigate the DD

Clinically relevant bidirectional drug-drug interaction between midostaurin and voriconazole.

Midostaurin is often prescribed with azole antifungals in patients with leukaemia, either for aspergillosis prophylaxis or treatment. Midostaurin is extensively metabolized by cytochrome (CYP) 3A4. In addition, it inhibits and induces various CYPs at therapeutic concentrations. Thus, midostaurin is associated with a high potential for drug-drug interactions (DDIs), both as a substrate (victim) and as a perpetrator. However, data on midostaurin as a perpetrator of DDIs are scarce, as most pharmacokinetic studies have focused on midostaurin as a victim drug. We report a clinically relevant bidir…

Ziritaxestat Drug-Drug Interaction with Oral Contraceptives: Role of SULT1E1 Inhibition.

In vitro signals indicate that ziritaxestat is a weak cytochrome P450 (CYP) 3A4 inhibitor and inducer. Therefore, potential drug-drug interactions (DDIs) with oral contraceptives were examined at a time when ziritaxestat was under development for treatment of fibrotic diseases. This open-label, crossover (fixed sequence) DDI study enrolled healthy, nonpregnant women aged 18-65 years (n = 15) who were using highly effective contraception, such as a nonhormonal intrauterine device, bilateral tubal occlusion, or sexual abstinence. A single dose of oral contraceptive (0.03 mg ethinyl estradiol (EE…

Drug-Drug Interaction between Tacrolimus and Vonoprazan in Kidney Transplant Recipients.

Kidney transplant recipients with tacrolimus-based immunosuppressive therapy are often treated with proton-pump inhibitors (PPIs) to prevent gastric ulcer complications. Vonoprazan, a potassium-competitive acid blocker, is a novel PPI possessing different metabolic pathways from conventional PPIs (e.g., omeprazole, lansoprazole and rabeprazole). However, no data are available on the change in blood concentration of tacrolimus after switching rabeprazole, a conventional PPI, to vonoprazan coadministration in the initial period of post-transplantation. This is a retrospective study of 18 kidney

A Possible Drug-Drug Interaction Between Eliquis and Amiodarone Resulting in Hemopericardium.

According to the ARISTOTLE trial, apixaban was superior to warfarin because it was associated with fewer strokes, systemic embolism, and bleeding. Hemopericardium was one of the major bleeding events reported in this trial. However, the percentage of patients that developed hemopericardium was not stated in the trial results. We present a case of hemopericardium in an 80-year-old man admitted for dyspnea, cough, and lower extremity edema. He was recently diagnosed with paroxysmal atrial fibrillation and started on apixaban for stroke prevention. Prior to admission, he was taking metoprolol suc

Drug-drug interaction database for safe prescribing of systemic antifungal agents.

INTRODUCTION: A drug-drug interaction (DDI) describes the influence of one drug upon another or the change in a drug's effect on the body when the drug is taken together with a second drug. A DDI can delay, decrease or enhance absorption or metabolism of either drug. Several antifungal agents have a large number of potentially deleterious DDIs. METHODS: The antifungal drug interactions database https://antifungalinteractions.org/was first launched in 2012 and is updated regularly. It is available as web and app versions to allow information on potential drug interactions with antifungals with…

Unintentional sulfonylurea toxicity due to a drug-drug interaction: a case report.

BACKGROUND: Sulfonylureas are widely used for type 2 diabetes mellitus, but these medications carry a risk of hypoglycemia. Drug-drug interactions that inhibit sulfonylurea metabolism and thus increase systemic exposure can cause unintentional sulfonylurea toxicity. CASE PRESENTATION: A 56-year-old man presented with severe, recurrent hypoglycemia. He had a history of type 2 diabetes mellitus and was taking the sulfonylurea gliclazide with no prior episodes of hypoglycemia. The onset of his hypoglycemia occurred within days after starting voriconazole and subsequently fluconazole for a fungal…
