Fracture of the Anteromedial Facet of the Coronoid is More Common Than Previously Thought in Combined Fractures of the Coronoid and Radial Head.

BACKGROUND: The terrible triad injury involves an ulnohumeral dislocation, radial head fracture, and coronoid process fracture. According to traditional teaching, these injuries are strongly associated with anterolateral coronoid tip fractures and can be addressed via a lateral approach to the elbow. However, recent small clinical series suggest that some terrible triad injuries have larger coronoid fractures involving the anteromedial facet. It is important to understand how often these larger coronoid fractures occur because anteromedial facet fractures may need a different approach and diff

Elbow Arthroplasty: From Normal to Failure.

Total elbow arthroplasty is currently an established surgical treatment for several pathologies of the elbow. Although initially used primarily in the treatment of rheumatoid arthritis, indications for total elbow arthroplasty have expanded and now include trauma, primary and secondary osteoarthritis, fracture nonunion, and following neoplasm resection. Desired outcomes of elbow arthroplasty include decreasing patient pain, restoration of function and mobility, and prevention of or treatment for instability. In comparison with total elbow arthroplasty, radial head replacements are most commonl

[Progress on diagnosis and treatment of the terrible triad of elbow joint].

The terrible triad of elbow is a kind of complex elbow fracture dislocation, after reduction, it should get a concentric circles joint reduction and elbow stability, if radial and coronoid process fractures is less piece, the conservative treatment can be performed, but regularly follow up is mandatory. If surgical treatment was chosen, radial head fractures and the lateral collateral ligament complex must be repaired. Single lateral approach can be used and also can be combined with anteromedial approach in surgery. Some problems are still controversial in the treatment of coronoid process fr

Fractures of the radial head and neck.

The majority of simple fractures of the radial head are stable, even when displaced 2 mm. Articular fragmentation and comminution can be seen in stable fracture patterns and are not absolute indications for operative treatment. Preservation and/or restoration of radiocapitellar contact is critical to coronal plane and longitudinal stability of the elbow and forearm. Partial and complete articular fractures of the radial head should be differentiated. Important fracture characteristics impacting treatment include fragment number, fragment size (percentage of articular disc), fragment comminutio

Triceps fascial tongue exposure for total elbow arthroplasty: surgical technique and case series.

During a posterior approach to the elbow, the triceps muscle body and tendon insertion must be addressed during exposure. Several methods exist including performing an olecranon osteotomy. The triceps fascial tongue exposure, originally described by Van Gorder in 1940, provides an excellent view of the joint while causing minimal trauma to the extensor mechanism facilitating tendon healing, and easy intraoperative conversion from fracture fixation to a total elbow arthroplasty if a fracture is not able to be reconstructed. Using a standard posterior approach to the elbow, a triceps tongue is c

Complex elbow instability.

Complex elbow instability consists of dislocation of the ulnohumeral joint with a concomitant fracture of one or several of the bony stabilizers of the elbow, including the radial head, proximal ulna, coronoid process, or distal humerus. Recurrent instability is not often associated with simple dislocation, but an improperly managed complex dislocation may be a prelude to chronic, recurrent elbow instability. Complex instability is significantly more demanding to manage than simple instability. Radial head, coronoid, and olecranon fracture associated with dislocation each must be assessed and

Buttress plate fixation of coronoid process fractures via a medial approach.

PURPOSE: To assess the clinical and radiographic outcomes of coronoid process fractures surgically managed with buttress plate fixation via a medial approach. METHODS: A retrospective review of all coronoid fractures surgically fixed in our institution using a buttress plate technique via a medial approach between June 2012 and April 2015 by the senior author was performed. These fractures were all sizeable fractures contributing to persistent elbow instability in terrible triad or varus posteromedial rotatory instability injury patterns. A prospective telephone questionnaire was conducted to

Fractures of capitellum: a review of 14 cases treated by open reduction and internal fixation with Herbert screws.

Fourteen patients with displaced fractures of the humeral capitellum were treated by open reduction and internal fixation of the capitellar fragments with Herbert screws. As per Bryan and Morrey classification, there were seven type I fractures, one type II fracture, three type III fractures, and three non-unions. Patient outcomes were evaluated using the Mayo elbow performance score. The follow-up period ranged from three to seven years (mean 4.8 years). All patients had a stable, pain-free elbow with good range of motion at follow-up. There was no evidence of avascular necrosis or degenerati

[The coexistence of fractures of the capitellum and the radial head: a rare case].

The coexistence of fractures of the capitellum and the radial head in the elbow joint is a rare entity. A 36-year-old man presented with complaints of pain, swelling, and limited range of motion in the right elbow after a fall. Radiography, computed tomography, and three-dimensional reconstruction of computed tomography images revealed displaced and comminuted fractures in the capitellum and the radial head. The fractures were treated with open reduction via a lateral approach and osteochondral fragments were fixed with Herbert screws. Clinical and radiographic results were excellent at the en

Monteggia fracture-dislocation in children.

Monteggia fracture-dislocations are uncommon in childhood. Correct early diagnosis is essential to avoid elbow dysfunction and the necessity for open reduction of the radial head. This retrospective analysis of 220 forearm fractures in children shows that 50% of Monteggia fractures were misdiagnosed by accident and emergency (A&E) department senior house officers (SHOs) and 25% were misdiagnosed by senior radiologists. Instruction in correct clinical and radiological examination of the elbow joint in children to junior A&E doctors, with early review of suspicious cases by senior medical staff,

Clinical and radiological evaluation of surgical treatment of medial epicondyle fractures of humerus in children. A two-centre study.

The present study analyses the outcome of open reduction and internal fixation (ORIF) of humerus medial epicondyle fracture with the use of Kirschner (K) wires, and determine the effect of elbow dislocation. The study included 112 patients operated on in 2005-2016. Of these, 81presented with an isolated medial epicondyle fracture (mean age 11.6 years), and 31 with an elbow dislocation (mean age 11.9 years). Out of 112 patients tested, 98 achieved an excellent treatment result, ten good and a mean Mayo Elbow Performance Score (MEPS); no significant differences were observed between dislocated a

[Bilateral elbow dislocation related to Essex-Lopresti injury].

Elbow dislocation is second in frequency, after the shoulder, whereas bilateral dislocation is uncommon, even less than dislocations with concurrent associated fractures. One of the least frequent associations is the Essex-Lopresti injury which consists of a fracture of the radial head affecting the distal radioulnar joint with injury to the interosseous membrane. This is a case of bilateral elbow dislocation, one of the elbows associated with the Essex-Lopresti injury. During treatment, the premature closed reduction prevails, previously making sure the elbow is stable, the premise which will

Evaluation of effect and safety of arthroscopic surgery with three different operative approaches in patients with terrible triad of the elbow: A comparative study.

BACKGROUND: The terrible triad of the elbow (TTE) is a difficult injury, and the usual TTE consists of posterior dislocation of the elbow, radial head fracture, and coronoid fracture. The target of this retrospective study is to explore the effect, postoperative complications, and prognostic factors in patients with TTE undergoing arthroscopic surgery with three different operative approaches. METHODS: Patients with TTE underwent arthroscopic surgery were treated using lateral, anterior medial, or combined lateral and anterior medial approaches, respectively. In order to analyze the postopera

Comminuted radial head fractures treated by the Acumed anatomic radial head system.

OBJECTIVE: The treatment of comminuted radial head fractures is still challenging. A radial head replacement is more effective in comminuted radial head fractures. The aim of this paper was to present the medium-term results of the Acumed anatomic radial head system (AARHS). METHODS: This study was performed on 12 patients with traumatic elbow fracture and instability between 2008 and 2011 of whom 12 were reviewed at a mean follow-up of 60.8 months (19 to 77 months). The evaluation included a record of pain, function, muscle strength, contracture and rotation. The outcome was assessed using t

Distal humerus fractures in elderly patients: results after open reduction and internal fixation.

PROBLEM: Fractures of the distal humerus are difficult to treat. In elderly patients, diminished bone mineral quality and increased trauma-associated joint destruction may make stable joint reconstruction even more problematic. Furthermore, comorbidities and poor tolerance of joint immobilization might be additional factors which influence elbow function negatively. Until now, disagreement has existed on how to treat these fractures in elderly patients. Recommendations range from conservative treatment to primary total elbow replacement. So far, reports in the literature on whether or not open

Early Callus as an Adverse Prognostic Sign Following Radial Head Replacement for Trauma.

Radial head fractures are common elbow injuries. Radial head replacement (RHR) is a well-established surgical option for non-reconstructible fractures, either in isolation or alongside associated elbow trauma, such as a terrible triad injury, which is a complex injury characterized by elbow dislocation and coronoid and radial head fractures. The procedure aims to restore the stabilizing function of the radial head, which plays a key role in maintaining longitudinal and valgus stability and an auxiliary role in the varus stability of the elbow. Radial head arthroplasty has gained popularity in

Open Reduction and Internal Fixation Versus Radial Head Arthroplasty for Mason III Radial Head Fractures: Appraising the Current Literature Evidence.

Fractures of the radial head are common and account for one-third of elbow fractures. Management has evolved over the past few decades as have the techniques and implants used to treat them. However, no standardized treatment protocol exists because of the complexity with which these fractures may present. The complex, unstable, displaced, and multi-fragmentary fractures, also known as Mason type III fractures, remain one of the most challenging fractures to treat, especially if associated with other elbow injuries. There are various surgical treatment options available, including open reducti…

Lateral condylar fracture and ipsilateral ulnar shaft fracture: Monteggia equivalent lesions?

Displaced ulnar shaft fractures are frequently associated with radiohumeral dislocation, producing the Monteggia fracture-dislocation. Fractures not previously thought to coexist were seen in a young boy following a severe automobile-pedestrian accident. The displaced ulnar shaft fracture was associated with a displaced lateral condylar fracture with preservation of the radiocapitellar joint and capsule. This fracture was treated with rigid internal fixation in spite of the open nature of the ulnar shaft fracture. This treatment permitted early range of motion of the elbow with early use of am
