Treatment of chronic pain by spinal cord stimulation.

Chronic pain after injury of the nervous system is difficult to treat. This report describes our experience in the treatment of chronic pain by spinal cord stimulation (SCS) in four patients (three men and one woman, with ages ranging from 61 to 73 yr). One patient had chronic intractable pain due to a spinal cord injury, one had post-herpetic neuralgia, one had failed back surgery syndrome, and one had brachial plexus and spinal cord injuries. A permanent spinal cord stimulator was implanted in the patient with spinal cord injuries. In the other three patients, a permanent spinal cord stimula

A target-specific electrode and lead design for internal globus pallidus deep brain stimulation.

In nearly all deep brain stimulation (DBS) applications, the same quadripolar electrode design is used for different anatomical targets even if shape and volume differences exist between nuclei. Taking into account the electrode location within the internal globus pallidus (GPi) and the size of the GPi, 2 electrodes were designed in order to improve the therapeutic benefit, to minimize side effects from DBS and to obtain a more homogeneous electric field distribution. The electrodes were evaluated numerically by using a stereotactic model measuring the correlation between the electric field an

Autonomic and cytokine mechanisms of acute electroacupuncture in a rodent model of functional dyspepsia.

BACKGROUND: Cytokines have been presumed to play an important role in the pathophysiology of functional dyspepsia (FD). Electroacupuncture (EA) has been used for FD treatment; however, its mechanisms remain largely unknown. This study aimed to (1) compare the plasma levels of cytokines, including tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-6, and IL-10, in "FD" rats with normal control rats; (2) investigate whether EA, using chronically implanted electrodes, could inhibit the release of these cytokines; and (3) explore the correlation of cytokine levels with plasma norepinephrine (N

Mechanism of the inhibitory effect of histamine on amygdaloid-kindled seizures in rats.

PURPOSE: The mechanism of the inhibitory effect of histamine on amygdaloid-kindled seizures was investigated in rats. METHODS: Under pentobarbital anesthesia, rats were fixed to a stereotaxic apparatus, and bipolar electrodes were implanted into the right amygdala. A guide cannula made of stainless steel tubing was implanted into the right lateral ventricle. Electrodes were connected to a miniature receptacle, which was embedded in the skull with dental cement. EEG was recorded with an electroencephalograph; stimulation of the amygdala was applied bipolarly every day by a constant-current sti

Effects of experimental myocardial infarction on the ST segment response to tachycardia.

Prior studies have shown that tachycardia results in ST segment depression in dogs with chronic, gradual coronary occlusion. This response was compared with that produced by acute, total occlusion of the left anterior descending artery. Ten dogs served as controls; in another 10 dogs, an ameroid constrictor was implanted about the left anterior descending artery. This artery was acutely ligated in a third set of 10 animals, and in a final set of 10, the distal left anterior descending coronary vasculature was embolized by latex injection. Tachycardia was produced by atrial pacing from rates of

Effect of Underwater Insertion on Intracochlear Pressure.

Background: The importance of intracochlear pressure during cochlear electrode insertion for the preservation of residual hearing has been widely discussed. Various aspects of pre-insertional, intra-insertional, and post-insertional relevant conditions affect intracochlear pressure. The fluid situation at the round window during electrode insertion has been shown to be an influential factor. Aims/Objectives: The aim of the study was to compare various insertion techniques in terms of the fluid situation at the round window. Material and Methods: We performed insertion of cochlear implant elect

Assessment of endocardial leads for permanent cardiac pacing.

Catheter tip dislocation is the main complication of permanent pacemaker implantation using the subclavian vein. In this study of 50 patients we compared the incidence of complications using two endocardial electrodes specially designed to prevent tip dislocation. One catheter had a helical endocardial electrode and the other had retractable nylon barbs. The helical electrode was superior in every respect and we recommend its use for pacing through the subclavian vein.

Cochlear implant surgery through oval window: A novel approach in a child with facial nerve aberration.

Cochlear implantation is safe for simple case presentations. However, facial nerve (FN) aberrations may impede insertion of the electrode through a round window and increase the risk of iatrogenic FN injury. We developed a novel method of cochlear implantation, a present the case of a child with FN aberrations. Inferior displacement of a horizontal segment of FN lay on the surface of th e promontory, obscuring the round window. The oval window was used and the electrode successfully inserted. No postoperative complications occurred, and significant improvement in speech perception was achieved

Permanent cardiac pacing using anterior pectoral veins.

We have used the anterior pectoral veins for inserting permanent transvenous cardiac pacemaker electrodes in 260 patients. Though this route is used in other centres it has not been published. We found it to be a reliable alternative to the more usual routes of pacemaker insertion, with a combined perforation and displacement rate of 9.6 per cent. We describe this technique of electrode insertion and compare the complication rates with those of other routes of implantation used in our department. All of the pacemaker implantations in this study were performed by doctors in the cardiology depar

Longitudinal changes of resting-state networks in Parkinson's disease.

Deep brain stimulation (DBS), but also the sole implantation of the electrodes and dopaminergic medication, can reduce symptoms in Parkinson's disease (PD) patients. Furthermore, an effect on network activity is known for all three options separately. However, long-term effects have rarely been investigated. Therefore, in the present study, we focus on the long-term impact of dopaminergic medication on whole-brain network activity following DBS electrode implantation. Therefore, we extracted resting state networks (RSNs) of 20 PD patients (4 females, (59.00 ± 9.72 years) from magnetoencephalog

Implantation of an endocardial tined lead to prevent early dislodgment.

Early electrode dislodgment from the ventricular apex is a major complication associated with transvenous cardiac pacing. A new lead with flexible tines proximal to the tip electrode has been developed to minimize acute dislodgment incidence. The bipolar tined lead was implanted in 79 patients. This experience was compared to 48 implants of a standard bipolar endocardial lead. Stimulation threshold, sensing, and resistance measurements from both leads were comparable. Although the cephalic vein was the preferred venous route, the jugular vein was needed more often for tined lead insertion than

Tutorial: a computational framework for the design and optimization of peripheral neural interfaces.

Peripheral neural interfaces have been successfully used in the recent past to restore sensory-motor functions in disabled subjects and for the neuromodulation of the autonomic nervous system. The optimization of these neural interfaces is crucial for ethical, clinical and economic reasons. In particular, hybrid models (HMs) constitute an effective framework to simulate direct nerve stimulation and optimize virtually every aspect of implantable electrode design: the type of electrode (for example, intrafascicular versus extrafascicular), their insertion position and the used stimulation routin

Effectiveness of diaphragmatic stimulation with single-channel electrodes in rabbits.

Every year, a large number of individuals become dependent on mechanical ventilation because of a loss of diaphragm function. The most common causes are cervical spinal trauma and neuromuscular diseases. We have developed an experimental model to evaluate the performance of electrical stimulation of the diaphragm in rabbits using single-channel electrodes implanted directly into the muscle. Various current intensities (10, 16, 20, and 26 mA) produced tidal volumes above the baseline value, showing that this model is effective for the study of diaphragm performance at different levels of electr

[Influence upon the longitudinal growth of extremities by direct current in animal experiments (author's transl)].

Direct current under certain conditions will be show a positive effect on the longitudinal growth of long bones during growing-age. Examinations were made in 8 to 10 week old rabbits in a right to left attempt. The electrodes were implanted in the midshaft of tibia. The stimulation was performed by direct current of 20 and 30 microamperes. The quantitative evaluation of 12 oxytetracyclinelabled pairs of bone showed a statistically significant increase of longitudinal growth on the electrically treated side as compared with control. Possible reasons leading to bone growth by electrical stimulat

Cochlear implantation in otosclerotic deafness.

The otosclerotic process commonly involves the otic capsule and may cause quite widespread demineralisation which leads to a progressive and often profound bilateral sensorineural hearing loss. In this situation cochlear implantation may be the only effective treatment. This chapter considers the pathology of this hearing loss, the effects of cochlear obliteration on implantation, and the effects of demineralisation of the otic capsule on placement of the electrode and nonauditory stimulation. A study is reported from 4 cochlear implant centres in the UK and the Netherlands of 53 patients with

Surgical aspects of cochlear implantation: mechanisms of insertional trauma.

The development of hybrid electroacoustic devices has made conservation of residual hearing an important goal in cochlear implant surgery. Our laboratory has recently conducted anatomical studies directed toward better understanding mechanisms underlying loss of residual hearing associated with electrode insertion. This paper provides an overview of observations based on microdissection, scanning electron microscopy and temporal bone histology relating to inner ear injury that may occur during implant surgery. Trauma to cochlear structures including lateral wall tissues, the basilar membrane,

ECoG spiking activity and signal dimension are early predictive measures of epileptogenesis in a translational mouse model of traumatic brain injury.

The latency between traumatic brain injury (TBI) and the onset of epilepsy (PTE) represents an opportunity for counteracting epileptogenesis. Antiepileptogenesis trials are hampered by the lack of sensitive biomarkers that allow to enrich patient's population at-risk for PTE. We aimed to assess whether specific ECoG signals predict PTE in a clinically relevant mouse model with ∼60% epilepsy incidence. TBI was provoked in adult CD1 male mice by controlled cortical impact on the left parieto-temporal cortex, then mice were implanted with two perilesional cortical screw electrodes and two similar
