Automated detection of rapid eye movements in children.

We present an automated multiple-step tool to identify Rapid Eye Movements (REMs) in the polysomnogram, based on modeling expert criteria. It begins by identifying the polysomnogram segments compatible with REMs presence. On these segments, high-energy REMs are identified. Then, vicinity zones around those REMs are defined, and lesser-energy REMs are sought in these vicinities. This strategy has the advantage that it can detect lesser-energy REMs without increasing much the false positive detections. Signal processing, feature extraction, and fuzzy logic tools are used to achieve the goal. The

Psychophysiological differences in schizophrenics with and without delusional misidentification syndromes: a P300 study.

There is a debate on whether delusional misidentification syndromes (DMSs) and schizophrenia are distinct disorders. Information-processing deficits have been found in both. Since the P300 component of event-related potentials (ERPs) reflects attention and working memory (WM) mechanisms, the P300 elicited during a WM test was studied in schizophrenic patients with DMS in comparison to schizophrenic patients without DMS and controls. Nine schizophrenic patients with DMS, 11 without DMS and 11 healthy controls were tested with a computerized version of the digit span test of the Wechsler batteri

Genetic linkage analysis of a novel syndrome comprising North Carolina-like macular dystrophy and progressive sensorineural hearing loss.

AIM: To characterise the phenotype and identify the underlying genetic defect in a family with deafness segregating with a North Carolina-like macular dystrophy (NCMD). METHODS: Details of the family were obtained from the Moorfields Eye Hospital genetic clinic database and comprised eight affected, four unaffected members, and two spouses. Pedigree data were collated and leucocyte DNA extracted from venous blood. Positional candidate gene and genetic linkage strategies utilising polymerase chain reaction (PCR) based microsatellite marker genotyping were performed to identify the disease locu

Congenital fibrosis syndrome associated with central nervous system abnormalities.

BACKGROUND: Congenital fibrosis of extraocular muscles (CFEOM) is a complex strabismus syndrome that typically occurs in isolation and results from dysfunction of all or part of cranial nerves III (CNIII) and IV (CNIV) and/or the muscles that these nerves innervate. Only a few patients with CFEOM and additional central nervous system malformations have been reported. We describe four additional patients with CFEOM associated with central nervous system (CNS) abnormalities. METHODS: Four patients who presented with congenital restriction of eye movements in association with neurological abnorm

Attention and successful episodic encoding: an event-related potential study.

Event-related potentials (ERPs) were used to delineate the cerebral processes occurring when information is encoded into episodic memory and to determine how these processes are affected by divided attention. ERPs were recorded during encoding under focused or divided attention, and were selectively averaged on the basis of their retrieval during later free recall and recognition tests (with remember-know judgments). Items retrieved with conscious recollection of the encoding episode (remembered, recalled) were distinguished at encoding from later missed items by an enhanced left fronto-tempor

Naso-oesophageal probes decrease the frequency of sleep apnoeas in infants.

The objective of the study was to determine whether a naso-oesophageal probe modifies sleep and cardiorespiratory patterns in infants with repeated obstructive apnoeas. Two polygraphic recording sessions were conducted in random order for 2 nights on 35 infants suspected to have repeated obstructive sleep apnoeas. One sleep study was performed with a pH probe inserted through the nasal passage down to the distal portion of the oesophagus. The other session was conducted without any naso-oesophageal probe (the baseline study). For the 25 infants who presented repeated obstructive apnoeas during

Rod and cone function in the Nougaret form of stationary night blindness.

BACKGROUND: Recently, a mutation (Gly38Asp) was identified in the alpha subunit of rod transducin in members of the Nougaret pedigree affected with dominantly inherited stationary night blindness. OBJECTIVE: To evaluate retinal function in patients with the Gly38Asp gene defect. DESIGN: Ocular examinations, including specialized measures of rod and cone function. SETTING: A clinical research facility in Boston, Mass. PATIENTS: A father (aged 48 years) and son (aged 25 years) with the Gly38Asp mutation. MAIN OUTCOME MEASURES: Psychophysical thresholds to white and narrowband lights and ful

Is excessive daytime sleepiness with periodic leg movements during sleep a specific diagnostic category?

Thirty-four patients who presented with excessive daytime sleepiness (EDS) and who showed an elevated number of periodic leg movements during sleep (PLMS) were studied. None of these patients reported other symptoms or presented sleep laboratory manifestations of narcolepsy or of breathing disorders during sleep. A diagnosis of restless leg syndrome, head trauma or a past history of psychopathology or infectious diseases known to cause EDS were also ruled out. In addition, none of the patients reported a history of drug or alcohol abuse, chronic sleep deprivation or irregular sleep-wake schedu

Sedative, memory, and performance effects of hypnotics.

The sedative, amnestic, and performance disruptive effects of benzodiazepine (Bz) receptor selective and non-selective hypnotics were studied in 23 healthy, normal subjects, aged 26.8 +/- 1.0 years. Triazolam (0.25 and 0.50 mg), zolpidem (10 and 20 mg) and placebo were administered, double-blind, at bedtime in a repeated measures design. During an awakening 90 min later (at approximate peak concentration of each drug) a 30-min performance battery which included memory, vigilance, and psychomotor tasks was completed. Each drug and dose impaired memory (both immediate and delayed), vigilance, an

Unilateral conjugate gaze palsy due to a lesion of the abducens nucleus. Clinical and neuroradiological correlations.

We report a case of left-sided horizontal gaze palsy, ipsilateral adduction weakness, and left peripheral facial weakness, all of which indicate the lesion in the left median pontine tegmentum. The enhanced MRIs revealed a discrete left median pontine tegmental lesion, involving the abducens nucleus, MLF, and facial nerve knee. This lesion spared the area of the left PPRF. Among these structures, the area of the abducens nucleus seems to be responsible for the unilateral horizontal gaze palsy. We are not aware of any previous precise neuroradiological documentation of unilateral paralysis of c

Simultanagnosia: a defect of sustained attention yields insights on visual information processing.

Simultanagnosia, in which subjects report a piecemeal visual experience, offers an important probe of human attention. We studied 2 subjects with simultanagnosia following bilateral superior occipital strokes. Compared with controls, they could orient attention to spatial targets in visual, auditory, and mixed-modal conditions. A different task required immediate response to the appearance or disappearance at unpredictable intervals of any element in a random-dot CRT display. The subject tested could detect less than 50% of 1,600 events, and had increased "mirages" and prolonged reaction times

[The retinopathy of ophthalmoplegia plus (Kearn's syndrome)].

The retinopathy of ophthalmoplegia plus (Kearns syndrome) is characterised by an extreme variability and a relative mildness of the visual disturbances, contrasting with the gravity of the ophthalmoscopic lesions. A study of three new cases, as well as an analysis of the literature, tend to demonstrate that the retinal lesions are mainly localised in the pigment-epithelial cells (mitochondrial anomalies?). This peculiar pathogenesis could explain why the retinopathy in this syndrome differs from classical pigmentary retinopathy.

SPAR-EEG: Selective Pass-Wise Artifact Reduction for Wearable Single-Channel EEG Denoising.

Single-channel electroencephalography (EEG) is attractive for wearable neurotechnology and brain-computer interface (BCI) applications, including assistive interfaces and clinical monitoring, but artifact suppression is difficult when auxiliary channels, artifact labels, or user-selected clean baseline segments are unavailable. We introduce SPAR-EEG, a self-contained framework for Selective Pass-wise Artifact Reduction in single-channel EEG. The framework applies three artifact-specific attenuation passes to each EEG epoch: a variational mode decomposition (VMD)-based pass for high-frequency e

Eye blinking characteristics as potential biomarkers in Isolated REM sleep behavior disorder during visuospatial attention tasks.

INTRODUCTION: Isolated REM sleep behavior disorder (iRBD) is a prodromal stage of alpha-synucleinopathies, such as Parkinson's disease (PD). Although altered eye blinking patterns are recognized as a clinical feature in PD, no studies have investigated its prodromal stage. This study aimed to investigate eye blinking characteristics in iRBD patients during a visuospatial attention task. METHODS: A total of 131 iRBD (aged 67.1 years) and 50 healthy controls (HCs; aged 66.2 years) participated. Patients who developed PD (PD-CV) or dementia with Lewy bodies (DLB-CV) were grouped as converters, w
