Role of granulocytes in experimental Streptococcus sanguis endocarditis.

We investigated the role of granulocytes during the induction and course of experimental Streptococcus sanguis endocarditis in rabbits by depleting blood granulocytes with nitrogen mustard. The induction of the endocarditis was not influenced by granulocytopenia: the 50% infectious dose was 5.4 X 10(4) colony-forming units in normal and granulocytopenic rabbits. However, granulocytopenia influenced the curse of the endocarditis, as shown by a significant increase in the number of colony-forming units per gram of vegetation (P less than 0.02) from 24 to 72 h after the injection of 10(5) colony-

Endomyocardial biopsy in infants and children: technique; indications and results.

By modification of a miniaturized technique for transcatheter endomyocardial biopsy, it was possible to insert a bioptome through a 6 French catheter in 15 infants and children aged 40 days to 17 years and weighing 3.5 to 55kg. Eight right and 7 left ventricular biopsies were performed, and the specimens were examined by light and electron microscopy. In one child we failed to obtain a specimen. There were no complications. Diagnoses to be either proved or excluded were hypertrophic or dilated cardiomyopathy, endocardial fibroelastosis, glycogen and lipid storage disease, tumours and myocardit

Effects of isoprenaline on functional capillary density in the subendocardial and subepicardial layer of the rat myocardium.

The functional capillary density in subepicardial and subendocardial layers of rat heart was measured during rest and during isoprenaline-induced (5.0 microgram X kg-1 X min-1, i.v. over 3 minutes) cardiac stimulation. For determination of the number of perfused capillaries, a fluorescent dye (thioflavine S) was infused into the left atrium; 1, 3, 5 and 10 sec, respectively, after starting dye application, hearts were excised and rapidly cooled down to -50 degrees C. In histological sections capillaries which had been perfused during the dye infusion could be identified and counted. An increas

The genetics and pathology of discrete subaortic stenosis in the Newfoundland dog.

Breeding experiments confirm that discrete subaortic stenosis (SAS) in Newfoundland dogs is a specific inherited trait. Specificity of the morphogenetic abnormality is not complete, however, since matings between Newfoundlands with SAS occasionally produced pups with valvular and subvalvular pulmonic stenosis as well as SAS. The spectrum of severity of SAS ranged from a subclinical forme fruste to a severe form causing death before maturity. Well-developed subvalvular stenotic rings consisted of a base of loosely arranged fibrous connective tissue and a subendocardial region of cartilagenous t

The regulation of endothelial-to-mesenchymal transition in endocardial cushion development: Signaling pathways and transcription factors.

Endocardial cushions, derived from the atrioventricular canal (AVC) and outflow tract (OFT), serve as the primordia for the formation of the … me. This biological event is strictly governed by regional signals originating from the myocardium, endocardium, and mesenchyme. Various signaling pathways, including TGFβ/BMP, Wnt, Notch, Hippo/YAP, PI3K/AKT, VEGF, and mechanical stress, are involved in the modulation of endocardial cell proliferation, migration, and mesenchymal transition. Transcription fac …

Disseminated Gonococcal Infection Presenting as Septic Wrist Arthritis and Endocarditis.

Disseminated gonococcal infection (DGI) is an uncommon manifestation of Neisseria gonorrhoeae infection and may rarely involve the endocardium. We report the case of a 48-year-old man who presented with progressive right wrist pain and swelling, back pain, intermittent fever, weight loss, and diarrhea. MRI demonstrated inflammatory changes involving the L3-L4 facet joints and sacroiliac joints. Culture from wrist aspiration grew N. gonorrhoeae, whereas blood cultures remained negative. Additional infectious evaluation showed a negative HIV screen, nonreactive syphilis testing, a negative hepat…

A case report of dyspnea that revealed a congenital left cardiac diverticulum of discovery in adulthood with a fatal course.

INTRODUCTION AND IMPORTANCE: Congenital cardiac diverticula are rare malformations involving the myocardium, the endocardium, and occasionally the pericardium. They have variable presentations and are often incidental findings. CASE PRESENTATION: A 61-year-old man with no significant history presented worsening exertional dyspnea. Examination revealed obesity but was otherwise unremarkable. Initial tests were normal, but chest radiography showed cardiomegaly and ECG indicated diffuse microvoltage. Echocardiography revealed a large left ventricular diverticulum (7 × 8 cm) with a wide neck (4 c…

Differential requirement for DICER1 activity during the development of mitral and tricuspid valves.

Mitral and tricuspid valves are essential for unidirectional blood flow in the heart. They are derived from similar cell sources, and yet congenital dysplasia affecting both valves is clinically rare, suggesting the presence of differential regulatory mechanisms underlying their development. Here, we specifically inactivated Dicer1 in the endocardium during cardiogenesis and found that Dicer1 deletion caused congenital mitral valve stenosis and regurgitation, whereas it had no impact on other valves. We showed that hyperplastic mitral valves were caused by abnormal condensation and extracellul…

Nephronophthisis type I, left ventricular non-compaction cardiomyopathy and reduced cilia motility-atypical manifestations of one disease.

Nephronophthisis, an autosomal recessive cystic kidney disease, represents genetically heterogenous group of diseases that lead to end-stage kidney disease in children and young adults. The typical clinical manifestations are polyuria, enuresis and growth failure. Left ventricular non-compaction cardiomyopathy is a rare form of cardiomyopathy, is determined by the disturbance of embryogenesis of the endocardium and myocardium. Primary ciliary dyskinesia is a genetically heterogeneous recessive disorder of motile cilia characterised by chronic lung disease and nonpulmonary manifestations, usual…

Method for preclinical pathology evaluation and analysis of cardiovascular implantable electronic device implant sites.

UNLABELLED: Cardiovascular implantable electronic devices (CIEDs) typically incorporate leads that directly contact the endocardium. Post-explant pathology evaluation of formalin-fixed CIED lead implant sites and downstream organs (i.e., lungs) can provide useful safety data to the US Food and Drug Administration; however, current regulatory guidelines do not mandate how the safety data are collected. In this paper, we outline a protocol for preclinical pathology evaluation of leads associated with CIEDs, which includes formalin fixation of the heart and lungs, gross evaluation, and qualitativ…

The Development and Regeneration of Coronary Arteries.

PURPOSE OF REVIEW: In this review, we aim to summarize and discuss the cellular origins of the coronary endothelial cells during development and neovascularization in the adult stage after cardiac injury. RECENT FINDINGS: Recent work identified three different developmental origins for coronary endothelial cells: proepicardium, endocardium, and sinus venosus. However, the level of contribution by each source remains debated. During heart injury and regeneration, although multiple cell types such as endothelial progenitor cells, epicardial cells, and endocardial cells were reported to contribu…

Update on assessment and management of primary cardiac involvement in systemic sclerosis.

Primary cardiac involvement is a common and severe complication of systemic sclerosis, which may affect all of the hearts' structural components, including pericardium, myocardium, endocardium, cardiac valves, and conduction system. While cardiac disease can be clinically silent and only diagnosed in autopsy, new imaging modalities such as speckle-tracking echocardiography and cardiovascular magnetic resonance may reveal various abnormal findings in the majority of patients. Cardiovascular magnetic resonance evaluation should include assessment of left and right ventricular function, inflammat…

Deletion of Fstl1 (Follistatin-Like 1) From the Endocardial/Endothelial Lineage Causes Mitral Valve Disease.

OBJECTIVE: Fstl1 (Follistatin-like 1) is a secreted protein that is expressed in the atrioventricular valves throughout embryonic development, postnatal maturation, and adulthood. In this study, we investigated the loss of Fstl1 in the endocardium/endothelium and their derived cells. APPROACH AND RESULTS: We conditionally ablated Fstl1 from the endocardial lineage using a transgenic Tie2-Cre mouse model. These mice showed a sustained Bmp and Tgfβ signaling after birth. This resulted in ongoing proliferation and endocardial-to-mesenchymal transition and ultimately in deformed nonfunctional mit…

Surgical considerations in the treatment of cardiac myxoma.

Misinterpretation of confusing cardiac, constitutional, and embolic symptoms delayed the diagnosis of cardiac myxoma and caused two of 18 patients to undergo acute operations during cardiogenic shock with pulmonary edema. In recent cases echocardiographic screening of unclear cardiac symptoms gave the correct diagnosis early. Despite the simple surgical procedure (excision of tumor and underlying endocardium), the postoperative course was complicated by cardiac failure, arrhythmias, and systemic reactions. Prosthetic valve thrombosis and malignancy caused two early deaths. Two patients died la…

An ALMS1 variant disrupts proximal centriole organization and promotes a myofibroblast-like phenotype that is regulated by THY1.

Primary endocardial fibroelastosis (pEFE) is characterized by accumulation of elastin-rich extracellular matrix (ECM) in the left ventricular endocardium in the absence of structural defects, but its pathophysiology remains unclear. We investigated dermal fibroblasts from a pEFE patient (proband) harboring a loss-of-function ALMS1 variant. Compared with control fibroblasts, proband cells displayed a myofibroblast-like phenotype, with increased ECM proteins and markers of fibroblast activation and endothelial-to-mesenchymal transition. Conversely, the thymus cell surface antigen (THY1) was down…

Cardiac calcified amorphous tumor in the right atrium: a rare cardiac neoplasm.

BACKGROUND: Cardiac calcified amorphous tumors (CATs) represent rare, nonneoplastic intraluminal heart masses, with limited case reports in … red blood cells. The final diagnosis confirmed a cardiac CAT. CONCLUSIONS: CATs, rarely occurring endocardium-based pseudotumors, comprise calcium nodules and amorphous fibrin material. Typically presenting as a calcified pedunculated mass, they may arise in any heart chamber, with a significant propensity for distal embolism. Differentiating CATs from calc …

Left-sided infective endocarditis presenting with pulmonary involvement and liver abscess: A case report.

KEY CLINICAL MESSAGE: Infective endocarditis is an important differential diagnosis in patients with persistent fever and chills not responding to antibiotics and involvement of multiple organs. It can present without any specific signs such as valvular murmurs and no growth on blood cultures. Therefore, considering an echocardiography can be crucial and helpful in establishing the diagnosis. ABSTRACT: Infective endocarditis (IE), a rare disease with high mortality, arises from microbial infection affecting the heart valves and endocardium. It exhibits diverse symptoms and can involve various…

Uhl's disease: An uncommon presentation of a rare disease.

Uhl's disease, also known as Uhl anomaly, is a rare disease secondary to selective but uncontrolled apoptosis of right ventricular myocytes during the perinatal period, after complete cardiac development, leading to the absence of right ventricular myocardium and the direct apposition of endocardium to epicardium without a myocardial layer in between, resulting in right ventricular failure. The present paper describes a case of Uhl's disease with an uncommon presentation. A 28-year-old man was admitted with dyspnea and cyanosis. Transthoracic echocardiography showed severe dilation of the righ…

Notch signaling in cardiac development.

The Notch signaling pathway has been demonstrated to play a critical role during mammalian cardiac development based on recent findings from gene-targeted mice. In addition, mutations in the Notch signaling pathway have been associated with human congenital heart defects such as Alagille syndrome, bicuspid aortic valve disease, calcification of the heart valves, and ventricular septal defects. Recently, it was demonstrated that Notch activation in the endocardium regulates ventricular myocardial development and that the Notch downstream target genes Hey1 and Hey2 are required for the establish…
