A patient with Ménière's disease is presented in whom several transtympanic electrocochleographic recordings were made during the course of the disease. Variations in the amplitude of the Summating Potential are related to clinical status. These findings lend support to the theory that the enhanced negative SP in Ménière's disease is produced by endolymphatic hydrops. The scientific evidence for this theory is discussed.

The symptoms of Meniere's disease (MD) are generally considered to be related to endolymphatic hydrops (EH). There are many recent reports supporting the possibility that vasopressin (VP) is closely linked to the formation of EH in Meniere's disease. Based on this, we developed a clinically relevant animal model of Meniere's disease in which a VP type 2 receptor agonist was administered after electrocauterization of the endolymphatic sac. We report live imaging of the internal structure, and functional changes of the inner ear after electrocauterization of the endolymphatic sac and administrat…

The ampullar endolymphatic potential (AEP) and the endolymphatic Ca2+ activity were measured with double-barred Ca2+ selective microelectrodes in the experimental hydrops of guinea pigs. They showed 3.16 +/- 0.42 mV (n = 9) and (4.10 +/- 1.22) x 10(-4) M, respectively. The AEP decreased significantly at about 6 min after intravenous administration of 10 mg/kg furosemide. Its minimal value was 3.04 +/- 0.53 mV at about 40 min. But there was no significant difference between that and the value before injection. The Ca2+ activity increased significantly and reached (7.10 +/- 1.32) x 10(-4) M at a

The objective of the study was to compare the outcomes of a series of diagnostic parameters in Ménière's disease (MD) patients with the extent of endolymphatic hydrops (EH) as shown by magnetic resonance imaging (MRI) performed after intra-tympanic gadolinium administration using 18 patients (13 males and 5 females, age 25-78 years, median age 54.3 years) with definite MD. A 0.6-ml solution of Gadobutrol (1 mmol/ml) diluted 1:7 in saline was injected through the inferior-posterior quadrant of the tympanic membrane, using a 22-gauge spinal needle. The patient was kept with the head rotated 45° …

BACKGROUND: Auditory and vestibular symptoms and signs are common in patients with migraine, yet little is known about the pathogenesis of these symptoms and signs.
OBJECTIVE: To perform clinicopathological correlation in a patient with migraine, sudden deafness, and delayed endolymphatic hydrops.
METHODS: A patient with long-standing migraine with aura developed sudden hearing loss in the left ear at the age of 50 years and Ménière disease on the right side at age 73. At age 76, he had a flurry of sudden drop attacks typical of otolithic crisis. He died of unrelated causes at age 81. The br…

Taurine is the second most abundant free amino acid in the brain where its osmoregulatory function is well established. Taurine-deprived kittens show retinal pathology leading to blindness. In the inner ear, taurine has been reported to be the most abundant free amino acid although its role in inner ear function is not known. Immunohistochemistry was employed here to investigate the localisation of taurine in normal cochleae of the guinea pig compared with two different conditions: experimentally induced endolymphatic hydrops and after oral administration of glycerol. In normal cochleae, by li…

OBJECTIVE: The diagnosis of Ménière's disease (MD), characterized by idiopathic endolymphatic hydrops (ELH), remains a clinical priority. Many ancillary methods, including the auditory and vestibular assessments, have been developed to identify ELH. The newly emerging delayed magnetic resonance imaging (MRI) of the inner ear after intratympanic gadolinium (Gd) has been used for identifying ELH in vivo. We aimed to investigate the concordance of audio-vestibular and radiological findings in patients with unilateral MD.
METHODS: In this retrospective study, 70 patients with unilateral definite …

Immunohistochemical investigations of the guinea pig cochlea, using a specific antibody to the inducible isoform of NO synthase (iNOS/NOS II), have been performed 3 weeks after closure of the right endolymphatic duct (n=7). Endolymphatic hydrops, the morphological substrate of Meniere's disease, became evident by distension of the Reissner's membrane. iNOS expression could be noted in endothelium, spiral ganglion cells, in nerve fibers, in supporting cells of the organ of Corti and cells of the spiral ligament. Temporal bones of non-operated controls (n=6) as well as of sham-operated animals (…

There is ample clinical and animal evidence that, in most patients, Meniere's disease is an immune-mediated disorder in a small, misplaced, malfunctioning endolymphatic sac that causes endolymphatic hydrops, the essential pathology of this disease. If this is so, then the potent anti-inflammatory steroids are the ideal first treatment for Meniere's disease and other immune-mediated diseases of the inner ear, and streptomycin perfusion is the ideal second treatment for those who do not respond to dexamethasone perfusion. The diagnosis, selection of patients, and details of operation with the re…

Both electrocochleography and glycerol dehydration have an important diagnostic and prognostic role in the management of patients with endolymphatic hydrops, whether idiopathic as in Meniere's disease or secondary to certain diseases of the otic capsule. Abnormal electrocochleographic responses in a "normal" ear can be used to predict when Meniere's disease will become bilateral. There is a significant correlation between the effects of glycerol and of endolymphatic sac surgery on the pars inferior, at least over the span of several years. The subject hearing gains, the objective improvements …

Hypofibrinolysis was found in five patients with Menière's disease that all started with sudden hearing loss. A connection between hypofibrinolysis and Menière's disease is proposed. The possibility that the symptoms of Menière's disease and endolymphatic hydrops is due to an inner ear vascular disorder, is discussed. It is concluded that, if the hypothesis is correct, it can lead to a causal therapy in some cases of Menière's disease.

Ménière's disease (MD) is characterized by loss of balance and hearing disorders. Although there is known to be endolymphatic hydrops involved in the pathological process, the pathogenesis of the disease is still largely unclear. Approximately half of patients with MD suffer from depressive symptoms and high levels of several stress hormones were observed in MD and depression, simultaneously. Recently, gut microbes have been shown to influence the function of the central nervous system in humans through their metabolites. We studied the intestinal microbiota of 10 patients with MD and 11 healt…

Previous clinical and histological studies have suggested that the aetiology of Meniere's disease should be sought not only in the endolymphatic hydrops, but also in pathological changes of the brain and brain stem. Moreover, neuropsychological studies have indicated some centralization of the disease, whether it be primary or secondary. In order to elucidate this problem further we have examined the temporal bones and brains of 4 patients with Meniere's disease, who died of diseases unrelated to the inner ear or the central auditory and vestibular pathways. Endolymphatic hydrops was found in …

The endolymphatic sac (ES) is thought to have close relations to pathogenesis of endolymphatic hydrops. Here is reported a retrospective study of 41 patients (42 ears) with Meniere's disease who underwent MRI prior to endolymphatic sac surgery. Based on proton-density weighted image (PDI) and T2-weighted image (T2), the ES including an endolymphatic duct (ED) were estimated whether it was detectable or not. Fourteen ESs were detected on both images (Group A), 14 ESs were detected only on PDI (Group B), and the remaining 14 ESs were not detected on either image (Group D). The actual shape of th…

The endolymphatic sac is a part of the homeostasis-regulating system of the membranous labyrinth of the inner ear. Disturbances in the function of the endolymphatic sac are believed to be involved in the genesis of different inner ear disorders, such as endolymphatic hydrops and Ménière's disease. To make studies of the ion- and fluid-regulating mechanisms of the sac possible, a method to culture the tissue in vitro was developed. Epithelial cells and fibroblasts were morphologically characterised in the cell cultures with light and electron microscopy as well as immunohistochemically using an…
