[Typical tumors of the petrous bone].

In the region of the petrous bone, inner acoustic canal and cerebellopontine angle, a variety of different tissues can be found, such as bony, epithelial, neural and vascular structures. Tumorous or tumor-like lesions, vascular or bony malformations or other pathologies can therefore be found in all of these areas. We discuss various frequently occurring tumorous or tumor-like pathologies including congential lesions, such as mucoceles, inflammatory disorders including osteomyelitis, pseudotumors and Wegener's granulomatosis. Benign non-neoplastic lesions, such as cholesteatoma, cholesterol gr

Measles virus in otosclerosis and the specific immune response of the inner ear.

Histologic and immunohistochemical studies of otosclerotic lesions have shown that there is a chronic inflammatory reaction of the otic capsule with bone resorption resulting from vascular invasion accompanied by inflammatory cells. During the active lytic stage of otosclerosis, paramyxoviral structures have been identified by electron microscopy and measles virus antigen expression by immunohistochemistry. Recently, measles virus related sequences have been detected in tissue of otosclerotic lesions. Because the otosclerotic focus has a close relation to the perilymphatic space, the expressio

[Ménière's disease].

Ménière's disease is based upon a clinical definition: the traditional Ménière's triad, a physiopathological definition, and the presence of hydrops. If all cases of Ménière's disease suffer from hydrops, not all cases of hydrops suffer from Ménière's disease. Recent progress thanks to electrocochleography, determination of the summating potential and its possible reversability under the effect of glycerol, enables a more reliable diagnostic approach, which furthermore includes the usual clinical and para-clinical examinations. Medical treatment has several objectives: the action of water on t

Triple semicircular canal occlusion for the treatment of Ménière's disease.

CONCLUSION: Triple semicircular canal occlusion (TSCO) controls vertigo, is easy to perform, and could be used as an alternative procedure for the treatment of Meniere's disease in selected patients who complain mainly of intractable vertigo. OBJECTIVE: To seek an effective alternative surgical procedure for treating Ménière's disease in selected patients with intolerant rotational vertigo. PATIENTS AND METHODS: Three patients with Ménière's disease who underwent unsuccessful endolymphatic sac decompression or mastoid shunt, then underwent TSCO. Vertigo control and vestibular and auditory fu

Hearing loss in patients with enlarged vestibular aqueduct: air-bone gap and audiological Bing test.

The Bing test is based on the principle that occlusion of the external auditory meatus improves the perception of bone-conducted sounds unless there is a conductive hearing impairment. An air-bone gap has been reported in patients with large vestibular aqueduct (LVA) syndrome without apparent middle ear dysfunction. We therefore performed the Bing test on nine patients with this syndrome to evaluate whether it is associated with an air-bone gap or middle ear dysfunction. Bone conduction thresholds did not change significantly during the Bing test in any patient. Because an air-bone gap is obse

Lipopolysaccharide-induced expression of reactive oxygen species and peroxynitrite in the guinea pig vestibular organ.

The purpose of this study was to investigate the occurrence of reactive oxygen species and peroxynitrite in the vestibular organ of the guinea pig following inoculation with bacterial lipopolysaccharide (LPS). The animals were injected transtympanically with 1 mg of LPS 24 h after the intraperitoneal injection of 0.1 mg LPS. Forty-eight hours after the inoculation, varying degrees of degeneration of the vestibular end organs were observed. Immunohistochemical study revealed immunoreactivity to xanthine oxidase (which generates O-2) in the vestibular organ after inoculation with LPS. Immunohist

Clinical management and outcome of Meniere's disease in children and adolescents: A systematic review.

INTRODUCTION: Meniere's disease (MD), or idiopathic endolymphatic hydrops, has been well-established in adults. However, due to the lack of understanding of pathomechanism and the progression of MD in children, managing this condition remains a challenge. The review aims to investigate the clinical management and outcome of treatments in children and adolescents with MD. METHODS: A systematic literature review was conducted in accordance with PRISMA guidelines to identify articles that assessed the outcome of treatment among children with MD. Information regarding the child's demographic data

Therapeutic strategies in the treatment of Menière's disease: the Italian experience.

PURPOSE: Ménière's disease (MD) is an inner ear disorder of unknown etiology, whose pathological substrate is the endolymphatic hydrops. Different treatments have been proposed; however, evidence of their effectiveness is lacking. The aim of this study was to evaluate by a questionnaire which medical and surgical treatments are used in Italy for the treatment of MD and to compare them with those proposed in other countries. METHODS: An electronic questionnaire of 40 questions was formulated and sent to Italian otolaryngologist (ENT) divided into two groups: Group 1 ("generalists" 60.8%) and G

Surgical treatment of peripheral vertigo and vertiginous diseases.

Although our understanding of the mechanisms of vertigo and pathophysiology of vertiginous disorders has increased, diagnosis and treatment of various vertiginous diseases is challenging. The objective for the treatment of a vertiginous disease is to eliminate the underlying pathology either with maneuvers or drugs. In vertiginous diseases, surgery is performed either to eliminate the underlying pathologic event or to create stability in the incoming vestibular signals. It is not always possible to treat the underlying disease. Therefore, surgery is usually performed for the relief of vertigo.

Retrolabyrinthine vestibular neurectomy with simultaneous monitoring of eighth nerve action potentials and electrocochleography.

Since November 1978, we have used retrolabyrinthine vestibular neurectomy in 29 of 42 cases as the primary procedure to relieve vertigo in Meniere's disease. The results indicate that 67% of patients had no vertigo postoperatively, while 21% were much improved. Hearing was maintained within 20 dB of the preoperative level in 78%. Simultaneous intraoperative electrocochleography and eighth nerve action potentials were used to monitor auditory function in the last fourteen cases using an Amplaid Mark V evoked potential signal processor. It appears that the intraoperative electrocochleography aft

Meniere's disease: the selection and assessment of patients for surgery using electrocochleography.

Transtympanic electrocochleography has been used to measure the action potential (AP), summating potential (SP), and cochlear microphonic (CM) in patients with Menière's disease. In this condition the widening of the AP/SP complex is thought to be due to an enhanced negative SP which shows no adaptation and this may be the result of an increased asymmetry produced by endolymphatic hydrops. Measurements have also been made using two groups of agents -- those designed to increase the cochlear blood flow (the metabolic activator naftidrofuryl and inhalations of carbon dioxide) and those designed

Dark cell pathology in Menière's disease.

The dark cells in the cristae of the semicircular canals were studied histopathologically in 9 temporal bones from individuals with Menière's disease (MD group), 7 temporal bones with endolymphatic hydrops from individuals without a history of Menière's disease (non-MD hydrops group), and 10 normal temporal bones (control group). The density of the dark cells was significantly lower in specimens in the MD group versus the non-MD hydrops group (Wilcoxon's ranking test, t = 90.5, p less than 0.01) and control group (Wilcoxon's ranking test, t = 50.0, p less than 0.01), and many of the dark cells

Prevention of vertigo in Ménière's syndrome by means of transtympanic ventilation tubes.

The insertion of a transtympanic ventilation tube prevented the occurrence of vertiginous attacks in 82% of a series of 28 patients suffering typical Ménière's syndrome with incapacitating vertigo resistant to medical treatment. The follow-up time exceeds 2 years for all cases. It ranges from 2 to 4 years with an average of 2.5 years. This treatment had no effect on drop attacks. Also, hearing loss does not seem to be influenced. The presence of a transtympanic ventilation tube has not caused any major complications in these patients. Obstruction or loss of the tube was in some cases followed

Surgical management of vestibular Meniere's disease with endolymphatic mastoid shunt.

The early stages of Meniere's disease may consist only of vestibular symptoms. These patients with vertigo alone may be incapacitated by this condition. A great many of these patients are unresponsive to medical therapy and tend to progress to cochleovestibular disease. Early surgery on such individuals may decrease vertigo and give the added benefit of preserving hearing. This paper reports the results of endolymphatic mastoid shunts performed on 24 patients with normal hearing who had medically uncontrolled vestibular Meniere's disease. The patients underwent surgery between 1978 and 1981, a

Surgical Management of Intractable Meniere's Disease.

Meniere's Disease (MD) is an inner ear disorder characterized by spontaneous recurrent vertigo, fluctuating sensorineural hearing loss, aural fullness and low-pitch tinnitus. Therapeutic management of MD includes dietary restriction and medical therapy. A minority of cases is characterized by frequent vertigo attacks, progressive hearing loss and persistent tinnitus even through the continuous medical treatments; this condition is called intractable MD and requires a therapeutic escalation from non-invasive medical treatment to surgical intervention. Invasive procedures include endolymphatic s
