Cranial nerve involvement with carcinoma of prostate.

Eleven patients with known prostate cancer presented with cranial nerve deficits and skull metastases during the course of their disease. All were treated with radiation therapy to the base of skull. Of the 11 patients, 10 (91%) responded to therapy. Four of the patients in the responder group had complete resolution of the cranial nerve deficits. This response lasted until their death. One patient had a complete response but later relapsed and is still alive. He is the only survivor of the 11 patients. Partial responses were achieved in 5 patients. These patients had either improvement but no

High-resolution CT of the normal and abnormal fallopian canal.

A two-part anatomic and clinical high-resolution computed tomographic (HRCT) study of the fallopian canal was conducted. From the correlation of HRCT images of eight specimen temporal bones with their corresponding anatomic sectional images, it was evident that the full length of the fallopian canal can be accurately visualized. An axial section demonstrates the labyrinthine segment, geniculate ganglion fossa, and proximal part of the tympanic segment, whereas a Stenver projection is used for the tympanic segment, second knee, and mastoid segment. In clinical studies axial sections simultaneou

[Facial nerve monitoring in the parotidectomy].

INTRODUCTION: Facial palsy (FP) is a feared complication of parotidectomy. Facial nerve monitoring (FNM) has been poorly evaluated since it was created at the beginning of the 90's. The authors described a retrospective series and discussed it reviewing the literature data. SUBJECTS AND METHOD: From January 2002 to January 2009, 75 parotidectomies were performed in 72 patients (34 males, 38 females), without FNM in 28 cases (group 1) and with FNM in 47 cases (group 2). RESULTS: In group 1, 9 FP were noted, 5 were transient and 4 were definitive. In group 2, 12 FP were noted, 9 were transient

Facial nerve outcome in non-vestibular schwannoma tumour surgery.

PROBLEMS/OBJECTIVES: Tumour size, intra-operative electrophysiologic thresholds and postoperative facial nerve function have been demonstrated to be important predictors of ultimate facial nerve function after vestibular schwannoma surgery. In general little attention has been given to the prediction of outcome of facial nerve function in non-vestibular schwannoma tumour surgery of the cerebellopontine angle (CPA). METHODOLOGY: A prospective study was performed to assess the predictive value of patient, tumour histology and electrophysiologic factors in the estimation of ultimate facial nerve

Extratemporal facial rehabilitation.

Treatment of the paralyzed face has made significant advances over the past 30 years. Evaluation of the paralyzed face with the concepts of early and delayed extratemporal facial rehabilitation and a review of the multiple methods of re-animation are discussed. Dynamic reconstruction is preferred to static methods except under special circumstnces. The many modalities of treatment include nerve grafting or crossover, regional muscle transposition or free muscle grafts, muscle interdigitation, and nerve implantation with the static methods of fascial stripping and rhytidectomy used in exception

The "elephant man" of Cambridge. a case report of neurofibromatosis.

The case is presented of a 65-year-old man with neurofibromatosis manifesting facial and skeletal features resembling those of the "elephant man" described by Sir Frederick Treves. Autopsy revealed not only a pheochromocytoma (a common accompaniment of neurofibromatosis), but an enlarged infarcted spleen and a subphrenic abscess. These findings have not been described previously in a patient with neurofibromatosis.

A Large Intratemporal Facial Nerve Schwannoma Presenting as an Occluding External Auditory Canal Mass.

Facial nerve schwannomas are rare, benign, slow-growing tumors that can occur in any segment of the facial nerve, although 71% of cases are intratemporal. Surgical resection can lead to facial nerve injury. Facial function recovery after reanimation is usually not better than House-Brackmann (HB) grade III. Thus, for cases of intratemporal facial nerve schwannomas (IFNSs) with favorable facial function (HB grade I or II), observation by periodic magnetic resonance imaging is the mainstay of management. Here, we present a case of a large IFNS with normal facial function in which the mass fully

Comparison of postoperative complications and facial nerve recovery rates after conventional and partial superficial parotidectomy of benign parotid tumours: a prospective study.

The aim of this study was to compare postoperative complications and facial nerve (FN) recovery rates between conventional superficial parotidectomy (CSP) and partial superficial parotidectomy (PSP) as surgical treatment for benign parotid tumours. A prospective study was conducted on 55 consecutive patients who underwent either CSP (n=35) or PSP (n=20) for benign parotid tumours. The primary outcomes were FN injury, FN recovery rates, and Frey syndrome. Secondary outcomes were operative time, hospital stay, sialocele, haematoma, and auricular numbness. The degree of FN injury was evaluated at

Middle ear adenoma with facial palsy: A case report and a review of the literature.

A 52-year-old man presented to our emergency department with an acute onset of right-sided facial nerve (FN) palsy of House-Brackmann grade V. Electroneurography (ENoG) was conducted with no response at the right FN, as compared with the left FN (0%). We performed a biopsy of the right middle ear mass and histological studies showed the tumor to be neuroendocrine tumors (NET) of the middle ear. We resected the tumor with canal wall down mastoidectomy and reconstructed the posterior meatal wall with soft tissue. Three months after surgery, the FN paralysis had improved with House-Brackmann grad

Stereotactic radiosurgery for facial nerve schwannomas: meta-analysis and clinical review.

OBJECTIVE: Although several small individual series on stereotactic radiosurgery (SRS) for facial nerve schwannomas (FNSs) have been published, we aim to systematically aggregate data from the literature as well as from our institution to better understand the safety and efficacy of SRS for FNS. DATA SOURCES: PubMed English language search for keywords "facial nerve schwannoma" AND "radiation therapy" published from January 1995 to 2014. Data from our institution were also included in the analysis. STUDY SELECTION: Minimum study inclusion criteria included tumor treatment outcomes yielding 1

Facial nerve monitoring during parotidectomy: a systematic review and meta-analysis.

OBJECTIVES: To determine the effectiveness of intraoperative facial nerve monitoring (FNM) in preventing immediate and permanent postoperative facial nerve weakness in patients undergoing primary parotidectomy. DATA SOURCES: PubMed-NCBI database from 1970 to 2014. REVIEW METHODS: A systematic review and meta-analysis of the literature was conducted. Acceptable studies included controlled series that evaluated facial nerve function following primary parotidectomy with or without FNM (intraoperative nerve monitor vs control). Primary and secondary end points were defined as immediate postopera

Fifteen inches from cancer: early recognition of facial lesions by the dentist.

Skin cancer is an increasing malignancy worldwide, and dentists are in a good position to observe their patients' faces for worrisome lesions. Most skin cancers are caused by actinic damage. The dentist must appreciate the characteristics of sun damage and be familiar with the clinical signs and symptoms of the more common premalignant and malignant facial lesions to appropriately evaluate the face. We review the features of freckles and moles, as well as those of seborrheic keratosis. Sun damage is discussed to aid in understanding the etiological role of ultraviolet exposure in the developme

[Progressive facial palsy. 2 recent cases].

Following 2 recent observations of progressive facial paralysis, the authors discuss the diagnostic problems encountered and the compressive etiology observed frequently. Extrinsic nerve damage calls for rapid care but should not cause the existence of neurinomas of the 7th nerve, more frequent than suspected, to be overlooked. Facial paralysis is the most frequent inaugural symptom, however the neurinomas of the 7th nerve may be preceeded by a pure cochleovestibular syndrome or a tumoral syndrome of the middle ear. They can appear at any place on the nerve, however the tympanomastoidal locati

Galeal-pericranial flaps in head and neck reconstruction. Anatomy and application.

Head and neck deformities of congenital, traumatic, or neoplastic cause often require reconstruction. At the University of Virginia over the past 14 years, we have used galeal, temporalis fascial, and pericranial flaps to correct these defects in more than 150 patients. Dissection of these flaps on both cadavers and reconstructive patients had demonstrated new anatomic findings different from those reported in standard textbooks. The galea is the most superficial layer of fascia. The pericranium is the next tissue layer. It is continuous above and separate from the temporalis muscle fascia. We

Vestibular neurectomy.

Vestibular neurectomy technique used for the standard middle fossa approach and for a total translabyrithine VIII nerve section is described. The middle fossa approach has been used in 49 cases of mainly Meniere's disease and basically the same approach for nerve decompression has been used in 19 cases of facial paralysis, 15 being due to Bell's palsy, 2 to petrous pyramid cholesteatoma and 2 to facial nerve Schwannoma. In unilateral Meniere's disease with no further function in hearing, vestibular neurectomy gives good results in alleviating vertigo.
